The lens of the eye is transparent, allowing light to pass through it and be focused on the retina. A cataract is an opaqueness or cloudiness in the lens that prevents light from reaching the retina. A cataract usually starts out as very small opaque areas, then becomes larger. How rapidly the cataract becomes larger varies greatly depending on many factors including the location of the cataract within the lens, the age of the animal, and the cause of the cataract. The size of the cataract will effect how much vision your pet has.
A very serious side effect of uncontrolled or poorly controlled diabetes is cataract formation. Diabetes-induced cataract formation is much more common in dogs than in cats. Not all diabetic dogs develop cataracts, but most do. Some dogs form cataracts so quickly that their vision is impaired and that's why the owner takes them to the vet, and then the diabetes is diagnosed. For other dogs, cataracts may form within weeks of diagnosis, or it may take months, or even years for cataracts to develop.
If your vet tells you that your pet is developing a cataract, or has cataracts, it is very important that you get a referral to a certified veterinary ophthalmologist and have the specialist examine your pet's eyes. The ophthalmologist has special education, training, and equipment that will allow him or her to give your pet's eyes a full examination. The lens is not the only structure involved - the health of the entire eye should be evaluated.

An estimated 12 million Americans have dry eyes. This condition is the inability to produce enough tears to keep the surface of the eyes lubricated and comfortable.
Tears are made by tissue in the lacrimal glands. The major lacrimal glands lie under the skin and extend into the eye socket. Tears reach the surface of the eye via small openings from the gland. Tears drain from the eye's surface by flowing into other tiny openings (lacrimal puncta), then into canals in the eyelids (lacrimal ducts), into a small pouch (lacrimal sac) and finally into the nose.
Normally, blinking helps the eyelids spread a tear film over the eye. If blinking is prevented, dry spots develop on the surface of the eye and vision decreases.

What is Dry Eye?
Tears serve to lubricate the eye and they are produced around the clock, but when insufficient moisture is produced stinging, burning, scratchiness and other symptoms are experienced and may be referred to as Dry Eye, Keratitis Sicca, Keratoconjunctivitis Sicca (KCS) or Xerophthalmia. When we blink, tears form a film which spreads over the eye, making the surface smooth and optically clear and enabling good vision. There are three layers in the thin film of tears: an oily layer, a watery layer and a layer of mucus, each with specific function. The outermost, oily layer is produced by small glands at the edge of the eyelid (meibomian glands) and the main purpose of this layer is to smooth the tear surface and reduce evaporation.

Punctal Plugs For Dry Eyes
Often people give up contact lens wear because their eyes are too dry. While there are a number of things that can cause dry eye including environmental factors, age, certain medications, eyelid conditions, etc., there are steps that can be taken to address the dry eye and keep you in contact lenses. When treating dry eye, regardless of contact lens wear, a stepwise approach is often the most successful. Initially, using lens re-wetting drops or artificial tears formulated for contact lenses can help moisturize the eye. The most common mistake here is not using them enough. Using drops four to six times a day is a good start. The next step in this incremental process can be the insertion of punctal plugs. There are two tear drainage ducts for each eye. If the problem is that not enough tears are remaining on the surface of the eyes, we can place plugs in the ducts to maintain more of the moisture on the eye. These plugs are made of a soft silicone plastic which may be removed if necessary, but which often allow patients to remain in contact lenses by increasing the tears that stay on the eye. If you feel you have dry eyes and have had to discontinue contact lens wear or decrease your wear time because of dryness, make an appointment to come in for a dry eye evaluation.

    Answer Dry eyes are a very common problem, thought to affect approximately 60 million Americans. The cause is usually unclear, but there appears to be an imbalance between tear production and tear volume drainage through the nasolacrimal ducts. The tear film is made up of a mucous layer against the eye, a middle aqueous (watery) layer, and an outer lipid (oily) layer. All three components are critical to a normal tear film. If any of the three layers of the tear film are deficient, the eye may suffer symptoms of dry eye.
    People with dry eye syndrome have the following symptoms: burning; stinging; redness of the eyes; and tearing. The excessive tearing is felt to be a reflex attempt by the lacrimal (tear) gland to overcome the poor quality of the tears that are produced. Tearing that produces these symptoms usually occurs in conditions that more rapidly evaporate tears from the eye, such as being outdoors in a high wind. Heat, low humidity, and the presence of smoke may compound the problem. Unfortunately, in most cases the cause of dry eye syndrome is unknown. However, there are numerous diseases and medications that may be associated with dry eye.

    A condition known as dry eye occurs when tear film abnormalities prevent the eyes from being comfortable. Although dry eye can occur in both men and women at any age, women are most often affected. Tear production normally decreases as we age. Dry eye can also be associated with arthritis, and occasionally with a dry mouth. Medications sometimes cause dry eye by reducing tear secretion. Since these medications are often necessary, the dry eye condition may have to be tolerated or treated with "artificial tears." A wide variety of common medications that can cause dry eye include diuretics; betablockers; antihistamines; sleeping pills; medications for "nerves"; pain relievers; or alcohol. Be sure to tell your ophthalmologist the names of all the medications you are taking. People with dry eye are often more prone to the toxic side-effects of eye medications, including artificial tears. There are two kinds of tears: those that constantly lubricate the eye and those that are produced as a response to irritation or emotion. The healthy eye constantly produces tears that lubricate. When a foreign body irritates the eye or when a person cries, more tears are produced. Excess tearing from dry eye may sound like a contradiction, but your tears must have the right balance of oil, water, and mucus to be able to protect your eyes. Without this balance, your eyes become dry and irritated and produce too many tears.

    Peer Review Status: Internally Peer Reviewed People usually know how to care for dry skin, but they may be unsure how to treat dry eyes, which can be equally or even more uncomfortable. In the winter months, with lower humidity, dry eye problems increase for some people. There are various types of dry eye syndrome, and between 10 and 15 percent of people in the United States have dry eye problems, said Kenneth Goins, M.D., associate professor of ophthalmology and visual sciences in the University of Iowa Roy J. and Lucille A. Carver College of Medicine. "Many different factors can affect the tear film quality and amount, which leads to secondary changes in the eye and loss of vision," said Goins, who specializes in treating patients with corneal, external and refractive eye diseases at UI Hospitals and Clinics. The amount of lubricating tears produced by the eyes can vary depending on whether the eye is at rest or under stress, such as while reading. People with severe dry eyes usually complain of pain, redness, decreased vision and foreign body sensation.

    Q. Are dry eyes a problem after transplant? Dry and painful eyes are one of the more common complaints mentioned by our LTFU patients. It can be a major problem for people after transplant, especially for those with GVHD. Symptoms of dry eyes include:
    • itching irritation burning redness blurred vision that improves with blinking discomfort after periods of concentration a feeling that something is in your eyes sensitivity to light
    Untreated or severe forms of Dry Eye Syndrome can lead to permanent eye damage. If dry eyes are bothering you, go to an ophthalmologist for a complete eye exam including a Schirmer's test and a slit lamp exam. Q. What can be done about my dry eyes? Things that you can do that may relieve dry eyes include:
    • drinking a lot of water blinking frequently taking frequent breaks from intense concentration using preservative free eye drops using a humidifier (with distilled water only)
    Some situations that can make dry eyes worse are hot or dry climates, wind, living at high altitudes, cigarette smoke, dust or pollen and air-conditioning.
