Overview A sad movie or a wedding can make your tears flow. But expressing emotion is just one of the many functions of tears. Tears protect your eyes and lubricate them, an essential part of clear, comfortable vision. Tears reduce the risk of eye infection and, with each blink of the eyelids, help clear your eyes of any debris. When your eyes become irritated from dust or are bothered by wind, smoke or fumes, extra tears form to help wash away the foreign material. However, dry eyes caused by decreased production of fluids from your tear glands can prevent tears from performing their useful functions and affect your vision. An imbalance in the substances that make up tears also can make your eyes become dry.

Causes
Lack of humidity in your home or workplace may be the source of your dry eyes. Get an hygrometer to measure the humidity, then add a humidifier to raise the level. Computer use or other close work where you need to concentrate can cause or exacerbate dry eye syndrome. When you concentrate you don't blink as often and therefore your eyes aren't being lubricated. Try to blink more frequently and take frequent breaks. Contact lenses are another possible cause. Talk to your eye specialist.

Warm compress Soak a washcloth in hot water and place it over your eye for 10 minutes two times a day. This method may stimulate your glands to produce more oil, which will keep your tears from evaporating as quickly.

I have dry eyes, is this a problem for LASIK candidates?
It is common to experience dry eyes for a short period after LASIK IntraLASIK and sometimes PRK and its cousin LASEK . Virtually everyone who has LASIK or IntraLASIK will experience dry eyes, but the important issue is to what degree and for how long. For the vast majority of refractive surgery recipients, the degree of dry eye symptoms is not significant enough to cause difficulties. Even those who do have some trouble most often find the dry eye symptoms resolve within the normal six-month healing period. For an unfortunate few patients, LASIK induced dry eyes is a long-term complication that, although manageable, may be very disruptive.

Risk Factors for Dry Eye Found Among Elderly Some Conditions, Medications May Increase Risk of Dry Eye Syndrome By Jennifer Warner WebMD Medical News Reviewed By Brunilda Nazario, MD on Monday, March 08, 2004 March 8, 2004 Elderly people with allergies or diabetes may face a higher risk of developing dry eye syndrome due to the medications they use, a new study reports. Researchers say dry eye syndrome is a common source of discomfort among the elderly and can seriously affect quality of life. But little is known about what risk factors are associated with the condition, which causes symptoms including eye irritation, itching, or burning and blurred vision. Although increasing age is the biggest risk factor for dry eye syndrome, the study suggests that people with allergies or diabetes who use antihistamines or diuretics or those with poor overall health may face the greatest risk of developing dry eye syndrome. In contrast, the risk of dry eye syndrome is lower in people who use ACE inhibitors (drugs commonly prescribed to treat high blood pressure or kidney problems in people with diabetes) or drink alcohol.

Dry Eye Syndrome Dry Eye Syndrome is a collection of symptoms that result from insufficient quantity or quality of tears. Symptoms may include dry, red, irritated eyes. Often people suffering from dry eyes will feel a "gritty, sandy" sensation in their eyes. They may even have seemingly paradoxical watering eyes since the eyes will water or tear excessively as a reflex response to the dryness and irritation. The cause is usually due to either a decreased production of "normal" tears or increased evaporation of tears related to poor quality of the tear film. What Causes Dry Eye Syndrome? Several factors can contribute to dry eyes, including: Blepharitis Often dry eye symptoms can result from inflammation of the eye lid margins, which may be caused by a bacterial infection ( Staphylococci ). This condition is called blepharitis and can compromise the quality of the tear film causing tears to evaporate more quickly. The bacteria produce waste material which can cause a mild toxic reaction leading to chronic red, irritated eyes. The first step in treating this condition is to initiate better lid hygiene. There are many ways in which to do this, but warm compresses, or "lid scrubs,"

Description of KCS
Keratoconjunctivitis sicca (KCS) or "dry eye" describes the changes in the eye which result from lack of tear production. To understand "dry eye" it is helpful to know how tears help keep the cornea healthy. The cornea is the optically clear portion of the eye that allows entry of light into the eye. Like all living tissue, the cornea requires a supply of oxygen and energy to remain healthy. Oxygen and nutrients are supplied to most tissues by the blood that moves through the area in blood vessels. The healthy cornea has no blood vessels, if it did it wouldn't be clear, so the oxygen and nutrients are supplied through the three-layered 'tear film.' The outer most layer of the tear film is an oily layer supplied by glands in the eyelids. This layer helps prevent evaporation of the next aqueous layer. The middle layer is the liquid aqueous layer produced by the main tear gland and a gland in the third eyelid. This is the layer that is decreased in dry eye. The innermost layer in direct contact with the cornea is a mucous layer produced by glands located in the folds of the eyelid. The mucus layer helps the aqueous layer adhere to the surface of the cornea. A breakdown in the tear film and a loss of the aqueous layer causes dry eye. This loss results in dryness to areas of the corneal surface or in more advanced cases, drying to the entire corneal surface. When the cornea is deprived of oxygen and nutrients through the tear film, it rapidly undergoes destructive changes. These changes result in brown pigmentation, scar tissue growth, ulcer development, and blood vessel growth across the cornea leading to partial vision loss.

DIAGNOSIS OF KERATOCONJUNCTIVITIS SICCA
Diagnosis is made by determining how long the condition has been on-going, what the doctor sees when the patient is examined and a number of testing procedures. These tests include the Schirmer tear test that measures how much of the watery layer is produced in one minute. Fluorescein stain (a bright green stain) is used to define possible breaks in the corneal surface and the rate of the tear breakup. In addition, Rose Bengal stain (a reddish pink stain) may be used to evaluate the health of the outer layer of the cornea called the epithelium. Examination of cells is sometimes recommended to define the state of health of the conjunctiva. This test has both a prognostic and therapeutic value.
WHAT ARE THE CAUSES OF KERATOCONJUNCTIVITIS SICCA?
A number of causes have been reported for dry eye. These include hypothyroidism, infections of the tear glands such as canine distemper virus and immune mediated diseases that attack the tear glands. Loss of nerve impulses to the gland due to longstanding ear infections will cause dry eye in some cases. Another frequent cause of dry eye is a toxic effect produced by some sulfa-containing drugs. Some of these drugs may have been given for the treatment of other diseases. It may not be possible to change the patient's medications and occasionally dry eye is treated despite being caused by other drugs. In many cases the cause of dry eye remains unknown yet treatment can still be instituted.
