  • What is dry eye syndrome? Dry eye syndrome is the decline of the quality or quantity of tears bathing the eye. The patient experiences constant pain from eye irritation, and a sandy or gritty sensation that, if untreated, can lead to scarring or ulceration of the cornea, and thus loss of vision.
  • What do we know about tears? In many cases, dry eye results from disorders of the various glands which work together to produce normal tears. Tears themselves are a complex combination of substances which form three layers on the eye. The very thin outer layer contains lipids from the meibomian glands in the eyelid, to reduce evaporation. The lacrimal glands produce the middle watery layer that keeps the salinity and the acidity of the tears at proper levels. This middle layer also carries antibodies and other immune defense agents to defend the eye against infection. The inner mucous layer helps the tear film "stick" to the cornea and stay intact.

    Overview Dry eye syndrome is one of the most common problems treated by eye physicians. Over ten million Americans suffer from dry eyes. It is usually caused by a problem with the quality of the tear film that lubricates the eyes. Tears are comprised of three layers. The mucus layer coats the cornea, the eye’s clear outer window, forming a foundation so the tear film can adhere to the eye. The middle aqueous layer provides moisture and supplies oxygen and other important nutrients to the cornea. This layer is made of 98 percent water along with small amounts of salt, proteins and other compounds.     Dry Eye:
    One of the most common diagnoses in our clinic is dry eye. The phrase "dry eye" encompasses many different ocular disorders that result in a dry eye condition. In order to understand some of these conditions, one has to learn some of the basics about the eye surface. The ocular surface is covered with a thin tear film, which serves many functions. Not only does it keep the surface moist, it plays a critical role in bending light rays toward the retina which leads to a nice, sharp visual image. The tears also contain proteins which help fight against infection. Nutrients are also delivered to the cornea from the tears. The main supply of oxygen to the cornea diffuses through the tear film. Obviously, the tear film plays a important role. The tear film is made up of three layers. The outermost layer is an oily layer released from glands on the eyelid margins. This oily layer spreads out over the rest of the tear film and prevents evaporation. Conditions such as blepharitis which results in abnormal or decreased oil secretions can lead to dry eye in which the tear production is adequate but rapid tear break-up (evaporation) is present. The innermost layer of the tear film is the mucin layer, which is made by special cells that are located on the surface of the eye. This mucous layer allows the tear film to spread out evenly and also binds up bacteria and other foreign matter. Severe scarring of the eye surface leads to a deficiency of the mucin. This occurs in severe burns (chemical or thermal) of the eye, trachoma (an infection seen commonly in the middle east), autoimmune diseases (pemphigoid), drug reactions (Stevens-Johnson syndrome) and some eye drop medications used for a long period of time.

    The natural tears that your eyes produce are composed of three layers:
    • the outer oily layer
    • the middle watery layer
    • the inner mucus layer
    Dry eye is the term used to describe eyes that do not produce enough tears or tears with the proper chemical composition in any of these layers. All three layers of tears, the outer oily layer, the middle watery layer and the inner mucus layer are produced by different parts of the eye and are necessary for proper lubrication. Dry eye is most often a result of eyes' natural aging process. Most peoples' eyes tend to become drier as they age, but the degree of dryness varies and some people have more problems than others. In addition to age, dry eye can result from:
    • problems with normal blinking
    • certain medications like antihistamines, oral contraceptives and antidepressants
