What is DiGeorge syndrome? The Thymus and Parathyroid Glands The thymus gland is located behind the breastbone and is responsible for the maturation of T-cells to fight infections. The four parathyroid glands are located adjacent to the thyroid gland in the neck and regulate calcium in the blood through the production of parathyroid hormone. The history of the syndrome, previously referred to as DiGeorge, includes the following discoveries:
  • In the mid 1960s, an endocrinologist named Angelo DiGeorge, MD, recognized that a particular group of clinical features frequently occurred together, including the following:
    • hypoparathyroidism (underactive parathyroid gland), which results in hypocalcemia (low blood calcium levels) hypoplastic (underdeveloped) thymus or absent thymus, which results in problems in the immune system conotruncal heart defects (i.e., tetralogy of Fallot, interrupted aortic arch, ventricular septal defects, vascular rings)

Case 127 A 24-day-old Term Infant with Seizures

Published on line in December 1997
PATIENT HISTORY: This 24-day-old term infant had an uncomplicated prenatal history and developed failure to thrive and seizures. He received 60cc of unirradiated packed red blood cells for anemia of unknown etiology and prophylactic antibiotic therapy for possible sepsis as evidenced by seizures. Following transfusion therapy, a chest x-ray and echocardiogram revealed an enlarged heart and truncus arteriosus with a large ventricular septal defect. The infant was admitted to Children's Hospital of Pittsburgh (CHP) in congestive heart failure. On physical exam the infant appeared normal. Seizure activity remitted with calcium supplementation. The infant developed an erythematous papular rash, predominantly on the face, trunk and upper extremities. Skin biopsy revealed histologic features suggestive of a drug rash, with mild eosinophilia in the papillary dermis and vascular channels. Fluorescent in situ hybridization of the buccal mucosa found a 22q11 deletion, confirming the diagnosis of DiGeorge syndrome. The infant received broad spectrum antibiotics and antifungal therapy for possible sepsis. A tracheal aspirate grew E. cloacae and a urine culture grew Candida species. All other cultures failed to identify organisms.

Word: Word Starts with Ends with Definition DiGeorge syndrome is a hereditary disease A genetic disorder , or genetic disease is a disease caused, at least in part, by the genes of the person with the disease. There are a number of possible causes for genetic defects:
  • They may be caused by an unwelcome mutation, as are most cancers.
  • There are genetic disorders caused by the accidental duplication of a chromosome, as in Down syndrome, or repeated duplication of part of a chromosome as in Fragile X syndrome.
  • The defective genes are often inherited from the person's parents. In this case, the genetic disorder is known as a hereditary disease . This can often happen unxpectedly when two healthy carriers of a defective recessive gene reproduce, but can also happen when the defective gene is dominant.

General Discussion DiGeorge syndrome is a rare immunodeficiency disorder characterized by various congenital abnormalities that develop because of defects that occur during early fetal development. These defects occur in areas known as the 3rd and 4th pharyngeal pouches, which later develop into the thymus and parathyroid glands. Developmental abnormalities may also occur in the 4th branchial arch.

Chromosome deletion that causes DiGeorge Syndrome found in mice HOUSTONA mouse model for studying a congenital heart disease has provided clues about the genetic cause and a possible way to prevent the disorder. Researchers at Baylor College of Medicine and Howard Hughes Medical Institute (HHMI) in Houston created the mouse model to study DiGeorge syndrome, a genetic disorder that causes potentially lethal heart defects. They identified the deleted portion of the chromosome that produces the defects and found that duplicating this chromosome region can prevent the defects. Their study is reported in the Sept. 23 issue of the scientific journal Nature. DiGeorge syndrome occurs in one of every 4,000 babies. About eight percent of patients with the disorder die from a defect in the aorta that prevents the lower part of the body from receiving oxygenated blood. The defect must be surgically repaired during the first few days of life. Other patients with DiGeorge syndrome might develop related heart problems that require surgery within the first several months, facial abnormalities, mild retardation and psychiatric problems. In humans, the disorder has been traced to a missing portion of chromosome 22. This chromosome deletion, known as del22q11, spans approximately 25 genes, although which one of those missing genes causes the syndrome remains unknown. The deletion occurs on only one of the two chromosomes 22; the other chromosome in the pair is normal.

ABSTRACT Background The DiGeorge syndrome is a congenital disorder that affects the heart, parathyroid glands, and thymus. In complete DiGeorge syndrome, patients have severely reduced T-cell function.
