  • Always give the medications that your veterinarian has prescribed at the correct dose and at the correct times.
  • Contact your veterinary practice if you are concerned that your animal is having a relapse, or if it appears to react abnormally following treatment.
  • Keep your animal on the strict diet that your veterinarian recommends and AVOID feeding high fat foods because fat intake can make the condition much worse. If your pet is a bin scrounger or likely to steal food - stop him/her . Many "snacks" are relatively high in fat - so cut them out !!

Most of us would probably agree that meta-analysis is a good thing, but not when we cannot use the results or we fail to reach a satisfying level of comfort in using its results. Bandolier was delighted when it saw a meta-analysis on the use of probiotics for antibiotic-associated diarrhoea [1]. There were seven randomised trials, but like so many meta-analyses all it told us was a lot of statistical guff, with the fact that the combined effect relative risk was 0.3966 with a 95% confidence interval of 0.275 to 0.571.
Not only does the over-precision of these figures worry us (who really worries about the fourth significant figure of the relative risk in Bridgend on a wet Wednesday afternoon?), but what use is a relative risk without some anchor? What is the incidence of diarrhoea with antibiotics? What is the rate of diarrhoea without probiotics?
So Bandolier decided to use this as a test case to see how much work would be needed to turn a good meta-analysis without utility into one that could be used.

Bovine viral diarrhoea is a complex disease of cattle. It affects all ages of cattle, some of its most important effects result from infection of the unborn foetus. Work is being carried out at Strathclyde University in conjunction with the department of Veterinary Medicine at the University of Glasgow Veterinary School, and the Institute for Animal Health, Compton, to define the effects of the disease on a herd of cattle, and to produce a mathematical model that can be compared with several specific herds whose disease status is known. The goal of the project is to quantify the effect of the disease on a herd, and thence extrapolate to the whole of the UK; the effectiveness of various control regimes, at present either culling certain groups of animals, or the use of a vaccination policy, is also to be assessed.

What is Traveller's Diarrhoea?
Any food or water from an infected source can cause traveller's diarrhoea. Symptoms of traveller's diarrhoea usually begin about three to four days after arrival. It can continue for up to ten days after return. The risk of traveller's diarrhoea is very much related to the destination, level of accommodation and travel habits of the traveller. Those who only go to 5* hotels will be at a lower risk of traveller's diarrhoea than those eating and living in local accommodation and restaurants. There is some evidence to suggest that those who travel frequently (at least every six months) to developing countries - do build up a limited defense against Traveller's Diarrhoea. However, this is not a life long immunity.

Diseases Traveller’s diarrhoea Traveller’s diarrhoea (diarrhea) is acquired through ingestion of faecal contaminated food or water, or both. Both cooked and uncooked foods might be implicated if they have been improperly handled. Especially risky foods include raw or undercooked meat and seafood and raw fruits and vegetables. Tap water, ice, and unpasteurized milk and dairy products can be associated with increased risk of TD; safe beverages include bottled carbonated beverages (especially flavoured beverages), beer, wine, hot coffee or tea, or water boiled and appropriately treated with iodine or chlorine. The onset of traveller’s diarrhoea is usually within the first week of travel, but can occur at any time during the visit and even after returning home. Manifestations: It typically results in four to five loose or watery stools per day. The duration of diarrhoea is 3 to 4 days. Approximately 10% of the cases persist longer than 1 week. Approximately 15% of ill people experience vomiting, and 2% to 10% have diarrhoea accompanied by fever or bloody stools, or both. Traveller’s can experience more than one episode of diarrhoea during a single trip. It rarely is life threatening. Diagnose and treatment: Most cases of diarrhoea are self-limited and require only simple replacement of fluids and salts lost in diarrhoeal stools. This is best achieved by use of an

Diarrhoea may be defined as the more frequent passing of stools, which are more fluid than usual. It should be noted that this definition reflects a significant change from normal. The fluidity is important: stools contain about 100-200ml of fluid; in a patient with severe diarrhoea this can increase to 10 litres in 24 hours.
Incidence
It is difficult to obtain accurate figures for the incidence of all self-limiting minor conditions, as most surveys are the result of self-reporting. However, 19 per cent of respondents to a British survey reported diarrhoea in the last year and 4 per cent in the previous two weeks.

There has been a significant growth in the number of people travelling to foreign countries. It was estimated that around 55 million visits abroad took place last year. This figure is set to double by 2010. People travel abroad mainly on holiday, however the last decade has seen a steady growth in travel for business, charity-sponsored events, sporting and educational reasons. There has been an increase in elderly travellers, people with chronic diseases, pregnant women and families with young children. Travel to tropical, culturally diverse and remote countries that do not have fully developed public health infrastructures or medical services, has become more common. Warm climates, and places with poor sanitation and hygiene systems increase the risk of travellers' diarrhoea.
Causes and transmission
Diarrhoea is caused by bacteria, viruses, protozoa and algae-like organisms, which vary in prevalence at particular locations and times of the year.

Diarrhoea is the most common complaint of travellers in their first 2 weeks away from home. This can be for many reasons, but is primarily thought to be related to the bacteria in the non-purified water of the developing countries. Normally, if a person gets diarrhoea the first thing they do is visit their doctor or GP who will make an accurate diagnosis and prescribe an appropriate course of treatment. High up the mountain this is unlikely to be an option, so self-diagnosis is vital. Although diarrhoea is rarely fatal, it causes they body to lose valuable water supplies, and in a mountain environment this can lead to an increased risk of altitude and cold related illnesses.
Bacterial infection is not the only cause of diarrhoea - viruses, protozoa, some medicines, food intolerances and allergies can also be held responsible. This means that there is no 'magic bullet' which will cure all types and strains. It is especially important to recognise this, as if you take antibiotics in the hope that it will be the cure-all end-all you may inadvertently kill off any good bacteria which the body is using to fight the infection. Diarrhoea is complex, so you should descend and seek professional help and if it persists for more than a week, or is accompanied any of the following:

Prevention
As diarrhoea is a very unpleasant condition to have halfway up a mountain some people choose to take preventative medicines to protect against diarrhoea, but doctors are divided as to whether this is a good idea. You should be aware of the importance of countering the dehydration consequent upon diarrhoea by drinking plenty of fluids, preferably a rehydration fluid containing salt and glucose. Antidiarrhoeal preparations, including antimotility drugs, can provide an adult with symptomatic relief; however, they can also cause undesirable side-effects and an authoritative opinion should be sought before they are used.     Infection is almost exclusively through contaminated food and water. Food and water may be contaminated with sewage or may be handled by people with poor hand hygiene.     Diarrhoea
    Diarrhoea is abnormally liquid bowel motions, not those which are slightly loose or frequent. Some children may pass several motions a day, while others go only a few times a week. Both are normal. Diarrhoea usually starts suddenly, and is often associated with colicky, coming-and-going tummy pains, which tend to flit around different parts of the tummy. This is quite different from appendicitis , where the pain is in one place and constant. It can be caused by travel or over-eating of a particular food (e.g. fruit or chocolate), but if your child is also vomiting, then it is very likely to be caused by a bowel virus infection. Most cases settle on their own without any special treatment, but if dehydration is setting in, then get medical advice without delay. You will know your child is dehydrated if:
    • He looks drawn and pinched in the face He has sunken eyes and a dry mouth His urine production slows down as his body tries to conserve fluid. If your baby does not wet his nappy for six hours, he may well be dehydrated
