This can be caused many different organisms such as E.Coli, Shigella, Entamoeba histolytica, Salmonella, Campylobacter, Giardia, Cryptosporidia, Cyclospora and Vibrio cholerae. All these organisms are spread through the faecal/oral route. Remember loose motions can also result from a change in diet including, for example, spicy or oily foods.
Prevention
This depends upon effective drinking water sterilization and ensuring food is uncontaminated or cooked thoroughly. Personal hygiene when eating and drinking is also important including hand washing prior to eating and using sterile plates, cups and utensils. What to avoid
  • Water should only be drunk when you are sure of its purity. Don't drink it without boiling, chemical disinfection or using a reliable filter. This also applies to water used for making ice cubes and cleaning teeth. Bottled water is usually safe, as are hot tea and coffee, beer and wine. Milk should be boiled unless you are sure it has been pasteurised.

What causes diarrhoea? Diarrhoea is not a disease; rather, it can be a sign of many different diseases or conditions. Some of the minor causes of diarrhoea include stomach or intestinal bacteria or viruses, intestinal parasites, and dietary sensitivities or indiscretions (such as scavenging rubbish or other offensive materials). Occasionally diarrhoea is a consequence of more serious illnesses, such as liver problems or cancer. Even diarrhoea caused by mild illnesses may become life-threatening if treatment is not begun early enough to prevent severe fluid and nutrient losses. How serious is diarrhoea in dogs? If your dog does not appear systemically ill from diarrhoea, the cause is unlikely to be serious. Most dogs with diarrhoea can be treated quickly and simply with medications given at home. Medications may be used that control the motility of the intestinal tract, or relieve inflammation in the intestinal tract, and often, a special diet is given for a few days. This approach allows the body's healing mechanisms to correct the problem. We expect improvement within 2-4 days. If there is no improvement then further tests may be needed to better understand the problem.

Coping with the complications of HIV/AIDS
Suggestions are made in this chapter as to which foods should be eaten or avoided for the following symptoms:

A person may have two or more of these conditions at the same time so will need to choose suitable foods carefully. Since people react to foods differently, they will have to select those that work for them. Normal mixed diets can be resumed as soon as the problem is resolved.

This loss is even greater if the person is vomiting. In severe cases, diarrhoea causes dehydration, poor absorption of food, significant weight loss and malnutrition, resulting in weakness and further illness. In young children diarrhoea can quickly become serious and, if not treated, lead to death. Diarrhoea can have many causes. It may be a symptom of disease or a side-effect of medicines and is often caused by contamination of food because of food hygiene problems. It can be made worse by eating certain foods.

  • Drink plenty of fluids - at least eight cups per day - to replace lost water. Drink soups, fruit juice diluted with water or an oral rehydration solution (ORS). Eat soft, mashed, moist foods such as soft vegetables and fruit, porridge from cereals, rice, bananas, mangoes, papaya, watermelon, potatoes and stews with refined maize meal, rice, barley or potatoes. Soft vegetables also include squash, pumpkins and carrots, and vegetable soup. Eat refined foods such as white rice, maize meals, white bread, noodles and potatoes. Peel and cook vegetables and fruit so they can be better tolerated. If fat causes problems, reduce fat intake by using less cooking oil, cutting off visible fat or skin from meat and boiling food rather than frying it. However, fat is an important energy source and should not be omitted from a diet unless really necessary. Do not eat foods that make your diarrhoea worse such as spicy foods and unripe or acidic vegetables and fruit.

Exercise-induced Diarrhoea: If you occasionally have trouble with your digestive system when you exercise, you're not alone. Scientific surveys reveal that up to 30 per cent of all runners suffer from abdominal cramping at one time or another, 50 per cent feel an occasional urge to defecate, 30 per cent actually have to defecate, and 25 per cent experience diarrhoea during or shortly after races.
What causes all this digestive-system mayhem during running? Running itself may not be the prime culprit. Research suggests that 15 per cent of runners who experience intestinal problems when they train may actually be suffering from irritable bowel syndrome. Another 15 per cent may be lactose intolerant, which increases the risk of diarrhoea. Finally, 25 per cent may be eating diets which are overly high in fibre.
Even so, that still doesn't explain why the other 45 per cent of defecation-prone runners feel the need to evacuate on the run. Perhaps the most popular theory to account for these cases of 'runners' trots' is that exercise decreases 'intestinal transit time' - the amount of time it takes for food to pass from the mouth to the rectum. Although this theory seems reasonable enough, supportive evidence is somewhat lacking.
One scientific investigation was able to show that individuals who began a regular exercise programme decreased their average intestinal transit time from 35 to about 24 hours. Unfortunately, dietary records weren't kept, so it's possible that the subjects adopted healthier, higher-fibre diets as they became fitter, a change which would have speeded up transit time even if no exercise were completed. In later research in which the food intake of exercisers was tightly controlled, exercise had no effect at all on gut transit time.

There is no evidence to suggest that travellers diarrhoea is caused by jet-lag, pollution or hot weather; almost all cases are caused by an infection, but it is not always possible to find which organism is responsible in every case.
Usually the formal diagnosis of which bug has caused the illness is not made as most cases settle without the need for treatment or hospital investigations.

    19 February 2004 HELEN R. PILCHER Campylobacter jejuni causes food poisoning and has a unique smell. The fumes from stools could help doctors diagnose diarrhoea in an hour. A new method uses the distinctive gases to identify the infectious bugs. Current detection techniques can take days. Many different infections, including viruses, bacteria and parasites, can trigger diarrhoea and cause severe dehydration. The condition can also be the symptom of a larger problem, such as cholera or typhoid. So the exact cause should be diagnosed and treated as quickly as possible, says Chris Probert from the University of Bristol, who helped develop the test The technique works by analysing diarrhoea's odour. Samples are placed in sealed glass vial and heated gently. "Essentially, you've got a fart in a tube," says Probert. The fumes are captured on a fibrous stick, which is fed into a machine for analysis. The gas is split into its constituent chemicals, which are identified by their mass and other properties. Each infection has its own chemical signature, the team found. Patients with rotavirus - the leading cause of childhood diarrhoea - have ethyl dodecanoate in their gas. This compound, which smells of pears, was missing in samples from patients with other diseases.

    What is a normal bowel habit? People vary a lot in how often they open their bowels - usually between three times a day and three times a week in the UK. A stool should be solid, but moist and easy to pass. What does the bowel do?
    • Receives food and liquid we eat and drink. Absorbs most of the nutrients from food and drink into the blood-stream. Secretes fluid from the body into the gut. Propels its contents from the stomach down the length of the bowel to the back passage (anus). Excretes waste material as stool.

    Many parents wonder if their child has diarrhoea because bowel movements in a newborn tend to occur frequently. Your baby's ordinary bowel movements will depend very much on whether she's being breast or formula fed. The bowel movements of breastfed babies are usually yellowish, on the soft or liquid side and may occur up to five times daily. Sometimes a breastfed newborn will have a bowel movement during or immediately after each breastfeeding session. This occurs because as her stomach fills up, the milk stimulates her entire intestinal tract, prompting a bowel movement. Within a month, most breastfed babies are down to one or two bowel movements a day.
