Diabetic Retinopathy
A Complication of Diabetes Involving Abnormal Blood Vessels Which Nourish the Retina of the Eye D iabetic retinopathy is the leading cause of blindness among adults. Approximately, 25% of current diabetics have some form of the disease. The risk of developing diabetic retinopathy increases with the age of the diabetic person and the duration of the disease. It is estimated that 90% of diabetics may experience some form of diabetic retinopathy over the course of their life. However, only a small percentage of those developing diabetic retinopathy have serious vision problems and even a smaller percentage become blind.
What is diabetic retinopathy?
Diabetic retinopathy is a complication of diabetes mellitus which causes abnormalities in the tiny blood vessels nourishing the retina. These vessels weaken, leak fluid and blood, and fail to provide nutrients necessary for good health in the retina. Left untreated, diabetic retinopathy can result in severe visual loss, including blindness.

The Problem of Diabetic Retinopathy
What is diabetic retinopathy?
Diabetic retinopathy is a complication of diabetes that affects the blood vessels of the retina. Growth of new blood vessels, known as proliferative retinopathy , may lead to blindness through haemorrhage and scarring. A deterioration of retinal blood vessels causing loss of blood vessels and leakage into the retina is known as maculopathy and leads to visual impairment and may progress to blindness. Top of page
Epidemiology of diabetes
. Without taking into account improved detection, the prevalence of both type 1 and type 2 diabetes will increase over the next two decades . Type 2 diabetes is more common in the elderly population. It has been estimated that the prevalence of type 2 diabetes in the UK will more than double between 1997 and 2010 . Diabetes is much more common in people of Asian Indian and Afro-Caribbean origin . In a Newcastle study Top of page
The burden of diabetic retinopathy
Diabetic retinopathy is the biggest single cause of registered blindness in the UK amongst working age people . Significantly greater numbers suffer visual impairment and handicap due to the effects of diabetic retinopathy. The rising prevalence of diabetes will ensure that diabetic retinopathy remains a major health and economic problem in the UK. Top of page

  • STRUCTURE AND FUNCTION OF THE EYE The eye is similar to a camera. It has a clear lens focussing light to a light sensitive membrane (the retina) which creates nerve signals. These signals form the basis of vision and are sent to the brain via the optic nerves. The brain processes the signals and eyesight is perceived by the individual. The retina is a thin transparent membrane spread over the inside of the back of the eye. It contains specialised cells called rods and cones and it is these cells that register the light. The cone cells are responsible for clear, straight ahead viewing and colour vision. The rod cells provide night and side vision. The retina is kept alive by blood vessels both within its structure and underneath it. The blood vessels are known as retinal arteries, capillaries and veins. Clearly anything that interferes with the function of the retina will have some impact on eyesight. The most important part of the retina is where the cone cells are and this is called the macula. It represents the centre of vision.

    http://www.vrmny.com/PDR.htm
Proliferative Diabetic Retinopathy By K. Bailey Freund, MD Proliferative diabetic retinopathy is a major cause of visual loss in diabetic patients. Patients at risk for this complication require frequent eye exams and often immediate treatment once the disorder is recognized. As retinal blood vessels become damaged and close off in diabetic retinopathy, the peripheral portions of the retina lose their source of nutrition and and stop functioning properly. This process is know as retinal ischemia and is believed to be the first step in the development of proliferative diabetic retinopathy (PRD). Presumably, the ischemic (nutrient-starved) retina sends out a chemical message which leads to the growth of new blood vessels (neovascularization) in the eye. These vessels often grow on the surface of the retina, at the optic nerve, or in the front of the eye on the iris. Color retinal photograph centered on the optic nerve (left eye). This picture demonstrates proliferative diabetic retinopathy. Abnormal vessels (neovascularization) are growing from the nerve over the retinal surface and into the vitreous jelly (the clear substance which fills the eye). Drawing of proliferative diabetic retinopathy.
