Antidepressants for diabetic neuropathy and postherpetic neuralgia
Clinical bottom line Antidepressants are effective treatments for diabetic neuropathy (NNT 3.4, 95% confidence interval 2.6 to 4.7) and postherpetic neuralgia (NNT 2.1, 1.7 to 3.0) compared with placebo. The NNH for any patient to have a minor adverse effect with antidepressant was 2.7 (2.1 to 3.9) and for a major adverse effect was 17 (10-43). For over thirty years the management of neuropathic pain has involved the use of both antidepressants and anticonvulsants, but which drug class should be first line choice remains unclear. The dogma that the character of the pain was predictive of the response, burning pain responding to antidepressants and shooting pain to anticonvulsants, was shown to be incorrect in diabetic neuropathy, where patients experiencing both burning and shooting pain responded to tricyclic antidepressants. Most studies on neuropathic pain have involved diabetic neuropathy and postherpetic neuralgia, because these conditions represent the majority of patients with neuropathic pain.

Diabetic neuropathy is a nerve disorder caused by diabetes. Symptoms of neuropathy include numbness and sometimes pain in the hands, feet, or legs. Can Diabetic Neuropathy Be Prevented?
People with diabetes can develop nerve problems at any time. Significant clinical neuropathy can develop within the first 10 years after diagnosis of diabetes and the risk of developing neuropathy increases the longer a person has diabetes. Some recent studies have reported that: 60 percent of patients with diabetes have some form of neuropathy, but in most cases (30 to 40 percent), there are no symptoms. 30 to 40 percent of patients with diabetes have symptoms suggesting neuropathy, compared with 10 percent of people without diabetes. Diabetic neuropathy appears to be more common in smokers, people over 40 years of age, and those who have had problems controlling their blood glucose levels. The symptoms of diabetic neuropathy vary. Numbness and tingling in feet are often the first sign. Some people notice no symptoms, while others are severely disabled. Neuropathy may cause both pain and insensitivity to pain in the same person. Often, symptoms are slight at first, and since most nerve damage occurs over a period of years, mild cases may go unnoticed for a long time. In some people, mainly those afflicted by focal neuropathy, the onset of pain may be sudden and severe.

Diabetic Neuropathy or Nerve Damage
Diabetic neuropathy almost always starts in the feet because these are the longest nerves in the body. They are also fed by the longest blood vessels in the body. The peripheral neuropathy that is characteristic of diabetes is found only in the presence of high blood sugar levels. Neuropathy represents the most common complication of diabetes. Neuropathy appears to be caused by damage to small blood vessels. Nerves depend on multiple tiny vessels that carry nutrients and oxygen to keep each and every segment of these very long nerves intact. Damage to one small segment can result in loss of feeling, pain or burning sensations that bother the feel and legs.

Symptoms and Diagnosis
A "neuropathy" is a condition in which the peripheral nerves (the nerves in your body, aside from your spinal cord and brain) are damaged or not working correctly. There are hundreds of different types of neuropathies and many different ways to categorize them, including by the type of nerve damaged, the causes of the nerve damage, or the pattern of nerve damage. A "polyneuropathy" is a neuropathy pattern, whereby the nerve damage initially starts in both feet and may progress to involve the feet, calves, and fingers/hands. Another word for this pattern is a "Stocking and Glove Neuropathy." Many patients with polyneuropathy may not even have any symptoms; in this case the diagnosis is made by a physical examination or a laboratory test (electromyography (EMG) and nerve conduction velocity test (NCV)). Some patients with polyneuropathy have only numbness, "tingling," and/or "pins and needles." Less often, some unlucky patients with polyneuropathy experience pain.

This is just one form of diabetic neuropathy but probably the most common and well known form.     Diabetic neuropathy is a nerve disorder caused by diabetes. Symptoms of neuropathy include numbness and sometimes pain in the hands, feet, or legs. Nerve damage caused by diabetes can also lead to problems with internal organs such as the digestive tract, heart, and sexual organs, causing indigestion, diarrhea or constipation, dizziness, bladder infections, and impotence. In some cases, neuropathy can flare up suddenly, causing weakness and weight loss. Depression may follow.     What is a peripheral neuropathy?
    The term peripheral neuropathy describes a problem with the functioning of the nerves outside of the spinal cord. The symptoms of a neuropathy may include numbness, weakness, burning pain (especially at night), and loss of reflexes. The pain may be severe and disabling. What causes a peripheral neuropathy?
    There are many possible causes of peripheral neuropathy, including:
  • Some of the most common causes include repetitive activities such as typing or working on an assembly line. In this case, the neuropathy may be isolated to the upper extremities, such as with carpal tunnel syndrome.
    Pressure on a nerve can cause a peripheral neuropathy. For example, pressure on a nerve that comes out from the groin to the skin in front of the upper thigh (the lateral femoral cutaneous nerve) can cause burning and tingling in this location. This particular problem is called meralgia paresthetica and can be caused by wearing a tight belt or other restrictive clothing. Additionally, it can result from being overweight or pregnant.
