Polymyositis and dermatomyositis are related diseases, both involving inflammation swelling or irritation of the voluntary muscles, those that normally govern movement, such as the muscles of the arms and legs. Myositis means inflammation of muscle. Polymyositis means inflammation of many muscles, and dermatomyositis means inflammation of muscle and skin. The results of either condition are weakened muscles and often muscle pain. These conditions are seldom fatal, but in the most severe cases a person may need a wheelchair or require assistance with activities of daily living. As early as 1863, a disease involving muscle inflammation and a reddish rash on the face and upper body was described and given the name polymyositis. In 1887, an apparent case of polymyositis without any rash was described by the German physician P. Hepp. Another German, H. Unverricht, then proposed using the name dermatomyositis for cases of muscle inflammation with skin changes and polymyositis for those without them. This terminology remains in use today. Both conditions are among the 40 neuromuscular diseases in the Muscular Dystrophy Association's program.

This National Institutes of Health funded project will identify children with new onset Juvenile Dermatomyositis and ascertain factors that may play a role in disease etiology. This study builds on promising data in previous studies concerning seasonality, antecedent illness, geographical clustering and other epidemiological information. CASE ASCERTAINMENT:
When a new onset JDMS patient is identified (within 6 months of the JDMS diagnosis) the Registry is notified by the physician calling: JDMS HOT LINE:773- 880-3333. The physician's name, address and phone number is recorded as well as some general information about the patient: criteria for diagnosis filled, date of onset, date of birth, race, gender and area of residence.

Dermatomyositis
Symptoms of dermatomyositis are almost identical to those of polymyositis. The diseases differ in that dermatomyositis is accompanied by a skin reaction that is manifested as a rash, commonly seen over the face, chest, and knuckles. The other major difference between the two disorders is that one-third of the cases of dermatomyositis occurring in older individuals is associated with an undiagnosed or a known underlying cancer. Once the diagnosis of dermatomyositis is made, attempt at ruling out the presence of a cancer should be implemented. These procedures include a chest x-ray, mammogram in women, prostate exam in men, flexible sigmoidoscopy, and possibly a CT scan of the abdomen with contrast.
The first step in the diagnosis of dermatomyositis is eliciting a history and physical examination compatible with the features seen in the disease. The history and examination are usually identical to those of polymyositis except for the presence of the characteristic skin manifestations. The same diagnostic steps that were used in polymyositis are also employed in dermatomyositis; specifically, blood for CPK level and an electromyogram. A muscle biopsy is performed which shows, in addition to the inflammation, a characteristic pattern of atrophy of muscle fibers (perifascicular atrophy). In contrast to polymyositis, the inflammation is usually located in the immediate vicinity of blood vessels (perivascular inflammation). Once the diagnosis of dermatomyositis is made and an underlying cancer has been ruled out, treatment is initiated in a similar manner as in polymyositis.

JDMS Kids to Pray For
Since I first published the JDMS Diary in 1998, I've received literally hundreds of email messages from people all over the world. Some are parents who are coping with a new JDMS diagnosis and turned to the Internet for information. Others are from families who have been dealing with JDMS for some time, and want to share with someone who understands what they're going through. Still others are current or former JDMS patients themselves, who want to connect and tell their story to help us cope a little better. And more. Our family says a prayer every night for Julia "and all the kids with JDMS" and we sometimes mention the name of someone who's written to us. I wanted to post a partial list (partial, because sometimes the name isn't mentioned in the email) of the names of those kids. We hold every one of them in our hearts.
