Prescribing a steroid drug, such as prednisone, is usually the first line of treatment. Immunosuppressants, such as azathioprine and methotrexate, may help those for whom prednisone is ineffective. Further treatment with intravenous immunoglobulin was shown to be effective and safe. Physical therapy is usually recommended to preserve muscle function and prevent muscle wasting. What is the prognosis?

Hello. My name is Sherrell and I am the mother of three loving children. My youngest child is a beautiful little girl named Jordan. She was only 10 years old, in 1998, when diagnosed with a rare disease called Juvenile Dermatomyositis. (JDMS) It is so rare, information is hard to come by. I thought I would put her diary on the Internet in hopes of getting more information and also, to let other parents, facing this same nightmare, know that they are not alone. The first months of the diary may be hard to read. It brings tears to my eyes and I wrote it and lived it. But, trust me, there are joyful tears in the months to come. We have been blessed, for God is taking care of our baby. We hope that the information is helpful to each of you. The diary is our personal experience and doesn't necessarily apply to everyone. However, if you share the diary with someone else that has been diagnosed with JDMS or is the parent of a child with JDMS, please encourage them to read the whole diary or at least through December 1998. I just recently found a forum that can connect us and may help answer many questions. You may want to check it out.

What is dermatomyositis?
Dermatomyositis is an acute inflammatory condition of skin and muscle. There are several distinct types of dermatomyositis which may ultimately prove to be different diseases. Most often dermatomyositis presents with increasing fatigue and loss of proximal (thighs and shoulders) muscle strength accompanied by a heliotrope rash. The characteristic clinical history is simply of a child who is not able to 'keep up', or who is asking to be carried more and more often. Difficulty going up and down stairs is often the manifestation which prompts the family to seek medical attention. These findings may occur with other illnesses, but a careful pediatric rheumatologist can often make the diagnosis on the initial examination. This is because many children have vasculitic lesions of the nail beds (' nailfold capillary dilatation) and lesions of the skin overlying the proximal interphalangeal joints ( Gottren's papules). These findings almost invariably signal dermatomyositis or a similar rheumatic disease (scleroderma or progressive systemic sclerosis). Less frequently the onset of dermatomyositis can be rapid with fever, widespread vasculitic rash and profound weakness. Both rapid and gradual

DERMATOMYOSITIS
  • Recognized in many different breeds but especially common in Collies and Shelties.
  • Both the skin and muscle can be involved. It is very unusual to have muscle signs without skin disease. In most cases, only dogs with moderate to severe skin lesions have clinically significant muscle disease. The skin disease may or may not be active when the muscle disease is recognized.
  • Although there appears to be a rare adult-onset variant, most affected dogs develop skin lesions early in life. Typically, skin lesions are first seen in dogs 12 to 24 weeks of age. The lesions may be short-lived or persist and worsen over the next several months.

Although the disorder is rare, with a prevalence of one to 10 cases per million in adults and one to 3.2 cases per million in children, early recognition and treatment are important ways to decrease the morbidity of systemic complications. An association with other connective tissue disorders (overlap syndrome) and malignancy make this diagnosis particularly important to primary care physicians. Patient management includes careful evaluation for underlying malignancy and liberal use of physical therapy, antihistamines, sunscreen and oral corticosteroids. Poor prognostic indicators include poorly responsive disease, delay in diagnosis and the presence of malignancy. The therapeutic goal is to maintain function and prevent or minimize sequelae.     Juvenile Dermatomyositis (JDMS) is a rare childhood auto-immune disease. My daughter, Julia, was diagnosed with JDMS in March of 1998. This web site is here for two primary reasons. First, to chronicle the events surrounding Julia's diagnosis and treatment of Juvenile Dermatomyositis, thereby helping me cope with it. Second, to give other children and their parents in the same situation some much-needed information and support. Juvenile Dermatomyositis is a very rare disease; I have scoured the internet, bookstores, and libraries and there is precious little on JDMS to be found, and much of that is out of date.
