Dengue fever

Definition
Dengue fever is a disease caused by one of a number of viruses that are carried by mosquitoes. These mosquitoes then transmit the virus to humans.
Description
The virus that causes dengue fever is called an arbovirus, which stands for arthropod-borne virus. Mosquitoes are a type of arthropod. In a number of regions, mosquitoes carry this virus and are responsible for passing it along to humans. These regions include the Middle East, the far East, Africa, and the Caribbean Islands. In these locations, the dengue fever arbovirus is endemic, meaning that the virus naturally and consistently lives in that location. The disease only shows up in the United States sporadically. In order to understand how dengue fever is transmitted, several terms need to be defined. The word "host" means an animal (including a human) that can be infected with a particular disease. The word "vector" means an organism that can carry a particular disease-causing agent (like a virus or bacteria) without actually developing the disease. The vector can then pass the virus or bacteria on to a new host.

International travelers are at risk for dengue infection, especially if an epidemic is in progress. Cases of dengue are confirmed every year in travelers returning to the United States following visits to tropical and subtropical areas. Travelers to endemic and epidemic areas, therefore, should be advised to take precautions to avoid mosquito bites. The principal vector mosquito, Ae. aegypti, prefers to feed on humans during the daytime and most frequently is found in or near human habitations. There are two peak periods of biting activity, in the morning for several hours after daybreak and in the late afternoon for several hours before dark. The mosquito may feed at any time during the day, however, especially indoors, in shady areas, or when it is overcast. Mosquito breeding sites include artificial water containers such as discarded tires, uncovered barrels, buckets, flower vases or pots, cans, and cisterns.

What is Dengue Fever?
Dengue Fever is an illness caused by infection with one of four serotypes of a virus belonging to the genus Flavivirus. Infection with one of these serotypes does not provide cross-protective immunity, so a person living in an area where Dengue Fever transmission occurs can have four dengue fever infections during his or her lifetime.
Who gets Dengue Fever?
Dengue Fever occurs primarily in tropical areas, including the Caribbean and Mexico in North America. Anyone living in or visiting tropical areas may become ill with Dengue Fever.
How is Dengue Fever spread?
Dengue Fever is spread through the bite of certain species of mosquitoes, but only if they are infected with the Dengue Fever virus. Capable species of mosquitoes pick up the Dengue Fever infection by biting a human who is ill with Dengue Fever. Two capable mosquito vectors are present in the Americas, including the United States, Aedes aegypti, and Aedes albopictus. Nearly all cases of Dengue Fever which are diagnosed in the United States are a direct result of recent travel to tropical areas outside the United States.

Dengue fever endemic Map The virus that triggers dengue fever (also known as dandy or breakbone fever) is related to the yellow fever virus. The symptoms are severe pains in the muscles and joints, which cause the patient to make stiff, awkward, "dandy-like" movements. Serious forms of this disease have been observed particularly in cases of repeated infection. It has been observed first and foremost in travelers in southern Asia and the Caribbean. It is transmitted by mosquitos, which is why good mosquito protection is always required. There is no vaccine. Top Print

Texas experiences first dengue outbreak in almost 20 years
October 6, 1999 Web posted at: 2:58 p.m. EST (1858 GMT) From Correspondent Charles Zewe NUEVO LAREDO, Mexico (CNN) The biggest outbreak of "dengue fever" to hit south Texas in almost 20 years has sickened more than a dozen people in the border town of Laredo. Epidemiologists there have confirmed 14 cases of the viral illness, which is spread by mosquitoes and nicknamed "breakbone fever." Over 100 cases have been identified just across the Mexican border and many more are suspected. Dengue is very painful, but rarely fatal.

Fact sheet N°117
Revised April 2002
Dengue and dengue haemorrhagic fever
Dengue is a mosquito-borne infection which in recent years has become a major international public health concern. Dengue is found in tropical and sub-tropical regions around the world, predominantly in urban and semi-urban areas. Dengue haemorrhagic fever (DHF), a potentially lethal complication, was first recognized in the 1950s during the dengue epidemics in the Philippines and Thailand, but today DHF affects most Asian countries and has become a leading cause of hospitalisation and death among children in several of them. There are four distinct, but closely related, viruses that cause dengue. Recovery from infection by one provides lifelong immunity against that serotype but confers only partial and transient protection against subsequent infection by the other three. There is good evidence that sequential infection increases the risk of more serious disease resulting in DHF. Prevalence The global prevalence of dengue has grown dramatically in recent decades. The disease is now endemic in more than 100 countries in Africa, the Americas, the Eastern Mediterranean, South-east Asia and the Western Pacific. South-east Asia and the Western Pacific are most seriously affected. Before 1970 only nine countries had experienced DHF epidemics, a number that had increased more than four-fold by 1995.

Dengue Hemorrhagic Fever Dengue Hemorrhagic Fever is an acute infectious viral disease usually affecting infants and young children. It is characterized by fever during the initial phase and other symptoms like headache, pain in the eye, joint pain, muscle pain, and other pain, followed by signs of bleeding such as petechiae (red tiny spots on the skin), nosebleeding and gumbleeding. If there is blood in the stools and/or blood in the vomitus and accompanied by shock, this is called Dengue Shock Syndrome and is often fatal.
Dengue Video Clip
Aedes Aegypti, the transmitter of the disease is a day-biting mosquito which lays eggs in clear water container, such as flower vases, cans, rain barrels, old rubber tires, etc. The adult mosquitoes rest in dark places of the house.
What are the symptoms of Dengue H-Fever?
1. Sudden onset of high fever which may last 2 to 9 days
2. Joint and muscle pain and pain behind the eyes
3. Weakness
4. Skin rashes - maculopapular rash or red tiny spots on the skin called petechiae

Arbovirus And Arenavirus Diseases
Arboviruses are maintained in nature through transmission between vertebrate and hematophagous arthropod hosts; they multiply in both. Arenaviruses belong to the family Arenaviridae and are usually transmitted by rodents but sometimes from human to human. Arbovirus is an early, ecologically based designation; changes in taxonomy, based on viral morphology, structure, and function, have distributed the arboviruses among several families, most notably the Togaviridae, Flaviviridae, Bunyaviridae, and Reoviridae. Important diseases are listed by clinical syndrome in Table 162-6. The arboviruses ( ar thropod- bo IV ribavirin (only aerosol currently approved in USA) (2-g loading dose followed by 1 g q 6 h for 4 days, then 0.5 g q 8 h for 6 days) is effective in Lassa fever, Rift Valley fever, and Crimean-Congo hemorrhagic fever. For dosage in hemorrhagic fever with renal syndrome, see below.
