Dengue (Break Bone) Fever and Dengue Hemorrhagic Fever Dengue fever is a viral disease carried by mosquitoes in the tropics and sub-tropics. These mosquitoes are most often found in urban areas and they bite mostly in the hours after sunrise and before sunset, on overcast days and in shady areas. Symptoms of dengue fever include fever, bone and joint pain, headache (especially severe just behind the eyes), rash, swollen glands and fatigue. Dengue hemorrhagic fever is a relatively rare but life-threatening form of dengue. The risk of this disease is higher in people who have already had at least one attack of dengue fever. As yet, there is no vaccine and no specific treatment for dengue fever. Avoiding mosquito bites is the only protection. **Note: Use Acetamenophen (e.g. Tylenol) to decrease fever. Never use ASA (Aspirin) or antiinflammatory medications since they can cause bleeding. Over the past decade there has been a dramatic increase in dengue fever outbreaks. Dengue is widespread in Latin America and the Caribbean , and also on the Indian Subcontinent and South East Asia . It is found in some parts of South America and Africa, some Pacific Islands, and in north eastern Australia.

Correspondence to: Simon Hales ( shales@wnmeds.ac.nz ); fax +64 4 3895319 Lancet Eprint There is increasing research interest in the potential impacts of global climate change on human health, and in particular, on the transmission of vector-borne diseases. Here we report an empirical model of the current climatic constraints on dengue fever transmission. We use these findings, along with projections of future population and climate, to assess the potential distribution of dengue fever in the next 100 years. We conclude that–other factors being equal–trends in population numbers and global climate would contribute to a substantial increase in the population at risk of dengue. Existing theoretically based models of the potential effects climate change on vector-borne diseases do not account for the effects of social factors such as population increase, or interactions between climate variables.

Dengue Fever What is dengue fever?
Dengue is a viral disease transmitted by Aedes mosquitoes mainly in tropical and subtropical areas of the world, with the greatest risks occurring in:
  • The Indian subcontinent Southeast Asia Southern China Central and South America (except Chile, Paraguay, and Argentina) Caribbean  Mexico Africa Taiwan The Pacific Islands
Dengue fever occurs most often in urban areas, but may be found in rural areas also, particularly in areas with elevation less than 4000 feet. Transmission of the virus, via  Aedes mosquitoes, usually occurs during and shortly after the rainy season. These mosquitoes are most active during the day and are often found near human dwellings, often indoors. What are symptoms of dengue fever?

Dengue Fever Antibodies Dengue fever is caused by several related viruses (four different arboviruses). It is transmitted by the bite of mosquitoes, most commonly Aedes aegypti, found in tropic and subtropic regions. This includes portions of Southeast Asia, the Indonesian archipelago into northeastern Australia, parts of sub-Saharan Africa, and parts of South and Central America. Dengue fever begins with sudden onset of a high fever, often to 104 to 105 degrees Fahrenheit, headache, and slightly later the appearance of severe joint and muscle pains.

The disease is typified by the sudden onset of a severe fever, chills, headache, pain behind the eyes (retro-orbital pain), photophobia, and joint and muscle pain. The victim may appear flushed at the onset of the fever and a rash may appear on the torso after 3-4 days and spread to the rest of the body. Lymph nodes are often inflamed.

Occurence is usually seasonal: during and shortly after the rainy season. Although the risk of acquiring dengue fever during travel to tropical areas is relatively low, outbreaks of Dengue fever have occurred with increasing frequency in recent years in most countries of the Tropics. Aedes aegypti mosquitos, that transmit Dengue fever are most active during the day and are usually found in or near human habitations. Dengue is predominant in urban centers, but may also be found in rural areas; it is rare above 4000 feet/1300 metres of height. No vaccine for dengue fever is available, therefore travelers should avoid mosquito bites by wearing long pants and long-sleeved shirts and using bednets and insect repellents on skin and clothing. Symptoms of Dengue fever include high fever, severe headaches, malaise, muscle and joint pain, vomiting and a rash appearing about three days after the first symptoms. These symptoms are nonspecific and may easily be mistaken as signs of other infections ( eg, flu, measles or malaria). Dengue is often characterized by a sudden onset of symptoms and usually lasts from one to four weeks until complete recovery. More severe forms of the disease ( Dengue hemorrhagic fever = DHF) are very rare among travelers.

Dengue Fever About the disease
Dengue fever is a deeply unpleasant, but generally harmless, disease common to large parts of the tropics. It's an arbovirus (Arthropod-BOrne VIRUS), and is spread between humans by mosquitoes. Arboviruses are a group of diseases that are spread between humans and animals by arthropods - a great division of the animal kingdom that includes insects, crustaceans, arachnids, etc. Unlike many arboviruses, dengue fever is not transmitted through animals - just humans. It is extremely common, especially in its usual form, but is also found less frequently in a much more serious haemorrhagic form (more about that later).

What is Dengue Fever?
Dengue Fever (DF) is a mosquito-borne disease affecting an estimated 50 million people each year. DF occurs in all tropical and some sub-tropical areas of the world, usually in an urban setting. Before 1970 only nine countries had experienced DF, by 1995 this had increased four fold. Today 2,5 billion people worldwide live in areas that are at risk of infection. Humans contract the disease when bitten by an infected female Aedes mosquito. Mosquitoes remain infected all their lives and can in turn pass on infectivity to their eggs. In some parts of the world it has been seen that monkeys are also able to pass on the infection to mosquitoes. Endemic areas increase around the world due to trade (particularly tyre trade where mosquitoes breed in pools of water in tyres). After a bite from an infected Aedes mosquito, the virus circulates in the blood for 2-7 days, causing a feverish illness with headache and muscle pains similar to flu. Sometimes presents with a rash.

PREVENTING DENGUE FEVER IN TRAVELERS
Travelers may acquire dengue fever, a potentially life-threatening viral illness, during visits to tropical and subtropical countries. Dengue is transmitted by the bite of infective Aedes mosquitos which are found primarily in urban areas. This disease occurs in most of tropical Asia, the Pacific Islands, the Caribbean Islands, Central and South America, and Africa. There is generally greater risk in urban areas and less risk of dengue in rural areas and at altitudes above 1500 meters (4500 feet). Figure 1 Dengue infected regions of the World
Dengue fever and its symptoms
Dengue is characterized by sudden onset, high fever, severe headache, joint and muscle pain and rash. Nausea and vomiting, loss of appetite, and altered taste sensation are common. A rash may appear 3 to 4 days after onset of fever and may spread from the torso to the arms, legs, and face. The disease is usually benign and self-limiting after about 7 days. Dengue may also present as a severe and fatal hemorrhagic disease, called dengue hemorrhagic fever (DHF). There is no specific treatment for dengue infection.

Background Dengue and dengue hemorrhagic fever (DHF) are caused by one of falicivirus. The mosquito vector is the same that carries the Yellow fever virus. Dengue is primarily a disease of the tropics, and the viruses that cause it are maintained in a cycle that involves humans and Aedes aegypti, a domestic, day-biting mosquito that prefers to feed on humans. Infection with dengue viruses produces a spectrum of clinical illness ranging from a nonspecific viral syndrome to severe and fatal hemorrhagic disease. Important risk factors for DHF include the strain and serotype of the infecting virus, as well as the age, immune status, and genetic predisposition of the patient. In the United States, there have been infections in south Texas and southeastern United States. The increase in air travel has resulted in the spread of this disease beyond the usual geographical areas. Dengue fever usually starts suddenly with a high fever, rash, severe headache, pain behind the eyes, and muscle and joint pain. Nausea, vomiting, and loss of appetite are common. A rash may appear 3 to 4 days after the start of the fever. Overall, the illness can last up to 10 days. A complete recovery can take as long as a month. Older children and adults are usually sicker than young children. Most dengue infections result in relatively mild illness, but some can progress to dengue hemorrhagic fever.
