The Dandy-Walker syndrome is a malformation of the brain that involves the maldevelopment of the cerebellum (hindbrain), associated with a cystic enlargement of this area, and frequently hydrocephalus. This malformation occurs in approximately one in 25,000 babies. It accounts for approximately 1 - 4% of cases of hydrocephalus and is seen more frequently in females than in males. There are a spectrum of abnormalities in this area that resemble the Dandy-Walker malformations. This may help explain why some of the newer descriptions of this abnormality refer to posterior fossa cysts and cyst-like formations rather than the classic Dandy-Walker malformation. The normal structures in the back of the brain include the cerebellum which lies on top of the brain stem and encloses a space referred to as the fourth ventricle. This space connects the ventricles of the upper brain (the cerebrum) through a narrow canal called the aqueduct of Sylvius. Normally, cerebrospinal fluid (CSF) flows from the interior of the cerebrum through this canal through the fourth ventricle to bathe the outside of the brain and the spinal cord. In the Dandy-Walker malformation, the cerebellum is poorly developed, and a large cyst of CSF develops in this area. This cyst may lift up and displace the back portion of the brain as well as cause an internal obstruction of the normal flow of CSF with resultant hydrocephalus.

yes no This is a condition whereby there is abnormal development of the foraminae (holes) through which the cerebrospinal fluid (CSF) exits from inside the brain to the outside surface. The foraminae, not having opened during fetal development cause a blockage of the flow of the CSF and so produce an abnormal enlargement of the fourth ventricle (fluid cavity between the brainstem and the cerebellum). This results in failure of parts of the cerebellum to develop correctly. A large cyst is then visible on scans of the back of the brain. These are severe abnormalities with hydrocephalus being present in 90 per cent of cases. Dandy-Walker causes 2-4 per cent of all hydrocephalus cases.
There are often many associated brain abnormalities including agenesis of the corpus callosum in 17 per cent of cases, occipital encephalocoele (failure of fusion of the bones at the back of the head) in 7 per cent, spina bifida syringomyelia (cysts in the spinal cord), or small heads and bone abnormalities of the neck (Klippel-Feil syndrome). Other associated abnormalities include facial, ocular and cardiac.

Hydrocephalus is a frequent but inconstant finding. Clinical features: The clinical manifestations of Dandy-Walker syndrome are mainly those of hydrocephalus. Therefore, the diagnosis is made in 75% of cases after 3 months of age and often only by the first birthday, as hydrocephalus usually becomes apparent by 1 year of age. Remarkably, there is no cerebellar sign or disturbances of stance. Mental retardation is present in 30-50% of patients. Prognosis: Prognosis is guarded and the mortality is about 27%. Other cerebral and visceral anomalies: it is the presene or absence of other cerebral and viseral abnormalies that determines the prognosis of individuals. About 68% of all cases have other developmental abnormalities of the CNS. Facial abnormalities may also be seen. Pathogenesis: agenesis of the vermis and cystic dilatation of the fourth ventricle may be the two primary events. Obstruction of the foramina of Magendie and Luschka may not be present in every cases. Other features may be secondary to the primary events.

Dandy-Walker Malformation is a rare malformation of the brain that is present at birth (congenital). It is characterized by an abnormally enlarged space at the back of the brain (cystic 4th ventricle) that interferes with the normal flow of cerebrospinal fluid through the openings between the ventricle and other parts of the brain (foramina of Magendia and Luschka). Excessive amounts of fluid accumulate around the brain and cause abnormally high pressure within the skull, swelling of the head (congenital hydrocephalus), and neurological impairment. Motor delays and learning problems may also occur. Dandy-Walker Malformation is a form of "Obstructive" or "Internal Noncommunicating Hydrocephalus," meaning that the normal flow of cerebrospinal fluid is blocked resulting in the widening of the ventricles.

Background: Dandy-Walker malformation is a rare congenital malformation and involves the cerebellum and fourth ventricle. The condition is characterized by agenesis or hypoplasia of the cerebellar vermis, cystic dilatation of the fourth ventricle, and enlargement of the posterior fossa. A large number of concomitant problems may be present, but the syndrome exists whenever these 3 features are found. Approximately 70-90% of patients have hydrocephalus, which often develops postnatally. Dandy-Walker malformation may be associated with atresia of the foramen of Magendie and, possibly, the foramen of Luschka. Dandy-Walker malformation first was described by Dandy and Blackfan in 1914. Since the original description, additional studies have reported on the various morphologic features of the syndrome. Not until 1954 did Benda first emphasize that atresia of the cerebellar outlet foramina is not an essential feature of the condition and suggested the now widely accepted term Dandy-Walker malformation. Studies by D’Agostino in 1963 and Hart et al in 1972 further defined the characteristic triad of Dandy-Walker malformation as consisting of (1) complete or partial agenesis of the vermis, (2) cystic dilatation of the fourth ventricle, and (3) an enlarged posterior fossa with upward displacement of lateral sinuses, tentorium, and torcular herophili. The triad typically is found in association with supratentorial hydrocephalus, which should be considered a complication rather than part of the malformation complex.

Soulioti Leeds General Infirmary, Ophthalmology Department, Leeds, England We report a case of congenital anophthalmos and glial hamartoma in a child with Dandy-Walker syndrome. This is the first reported case of anophthalmia and glial hamartoma occurring in conjunction with Dandy-Walker syndrome, and may have some implications with regard to the pathogenesis of this syndrome.
