Dandy-Walker Syndrome
The Dandy-Walker syndrome is a malformation of the brain that involves the maldevelopment of the cerebellum (hindbrain), associated with a cystic enlargement of this area, and frequently hydrocephalus. This malformation occurs in approximately one in 25,000 babies. It accounts for approximately 1 - 4% of cases of hydrocephalus and is seen more frequently in females than in males. There are a spectrum of abnormalities in this area that resemble the Dandy-Walker malformations. This may help explain why some of the newer descriptions of this abnormality refer to posterior fossa cysts and cyst-like formations rather than the classic Dandy-Walker malformation. The normal structures in the back of the brain include the cerebellum which lies on top of the brain stem and encloses a space referred to as the fourth ventricle. This space connects the ventricles of the upper brain (the cerebrum) through a narrow canal called the aqueduct of Sylvius. Normally, cerebrospinal fluid (CSF) flows from the interior of the cerebrum through this canal through the fourth ventricle to bathe the outside of the brain and the spinal cord. In the Dandy-Walker malformation, the cerebellum is poorly developed, and a large cyst of CSF develops in this area. This cyst may lift up and displace the back portion of the brain as well as cause an internal obstruction of the normal flow of CSF with resultant hydrocephalus. These patients may have other abnormalities in the development of the brain, including decreased connections from one side of the cerebrum to the other side. Developmental malformations outside the nervous system may occur in approximately 30% of these patients. These other malformations include cleft lip, cleft palate, cardiac malformation, orthopedic and respiratory problems, and urinary structural abnormalities.

Dandy-Walker Syndrome is a congenital brain malformation involving the cerebellum (an area at the back of the brain that controls movement) and the fluid filled spaces around it. The key features of this syndrome are an enlargement of the fourth ventricle (a small channel that allows fluid to flow freely between the upper and lower areas of the brain and spinal cord), a partial or complete absence of the cerebellar vermis (the area between the two cerebellar hemispheres), and cyst formation near the internal base of the skull. An increase in the size of the fluid spaces surrounding the brain as well as an increase in pressure may also be present. The syndrome can appear dramatically or develop unnoticed. Symptoms, which often occur in early infancy, include slow motor development and progressive enlargement of the skull. In older children, symptoms of increased intracranial pressure such as irritability, vomiting, and convulsions, and signs of cerebellar dysfunction such as unsteadiness, lack of muscle coordination, or jerky movements of the eyes may occur. Other symptoms include increased head circumference, bulging at the back of the skull, problems with the nerves that control the eyes, face and neck, and abnormal breathing patterns. Dandy-Walker Syndrome
Dandy-Walker syndrome is a congenital brain malformation involving the fourth ventricle and cerebellum. It is defined as an enlargement of the fourth ventricle, an absence (partial or complete) of the cerebellar vermis (the narrow middle area between the two cerebral hemispheres), and cyst formation in the posterior fossa (the internal base of the skull). Hydrocephalus (increased intracranial pressure) may also be present. Symptoms which often occur in early infancy include slow motor development and progressive macrocrania (an abnormally enlarged skull). In older children symptoms of increased intracranial pressure such as irritability, vomiting and convulsions, and/or signs of cerebellar dysfunction such as ataxia and nystagmus (jerky eyes) may occur. The syndrome can appear dramatically or be totally asymptomatic. Other symptoms include increased head circumference, bulging occiput (the back of the head), cranial nerve dysfunction, and abnormal breathing patterns.

The disorder was first described in 1914 by W. Dandy and K. Blackfan and was designated as Dandy-Walker syndrome in 1954 by C. Benda, who also reported familial occurrence. The Dandy-Walker syndrome is hydrocephalus associated with a posterior fossa cyst and abnormal development of the cerebellum. What causes Dandy-Walker Syndrome? This malformation is thought to occur secondary to atresia (failure to open) of the foramina of Luschka and Magendie. This results in agenesis (failure to develop) of the cerebellar vermis with a large posterior fossa cyst communicating with an enlarged 4th ventricle. Hydrocephalus occurs in 90% of cases. How is Dandy-Walker Syndrome diagnosed? The clinical presentation of the Dandy-Walker syndrome depends to some extent on the combination of the developmental anomalies in the infant. If no other anomalies are present the sole manifestation may be increasing head size secondary to hydrocephalus. The normal signs of increased intracranial pressure may be absent in infants secondary to the ability of the head to increase in size. In older children, cerebellar dysfunction (mainly affecting balance), mental impairment, and double vision may be found. The definitive diagnosis depends upon radiological intervention. Ultrasound, MRI, and CT scan will show the malformation.

Dandy-Walker Malformation is a form of "Obstructive" or "Internal Noncommunicating Hydrocephalus," meaning that the normal flow of cerebrospinal fluid is blocked resulting in the widening of the ventricles.

A congenital brain malformation involving the fourth ventricle and cerebellum, marked by partial or complete absence of the cerebellar vermis, and posterior fossa cyst continuous with the fourth ventricle. Congenital hydrocephalus is caused by blockage of the foramina of Magendie and Luschka (the foramina of the fourth ventricle of the brain), through which the cerebrospinal fluid passes. Because of the blockage, the cerebrospinal fluid cannot pass into the subarachnoid space and in consequence accumulates in the ventricles. Symptoms are vomiting, hyperirritability, convulsion. There is progressive enlargement of the head, congested veins in the scalp, bulging of anterior fontanelle, separated cranial sutures. Other complications may include papilledema, blepharoptosis, bradycardia, bradypnea and sixth cranial nerve paralysis. Develops in utero.
