To demonstrate a vaginal approach to improve the support of anterior compartment prolapse (ACP) after failed cystocele repair with the interposition of Prolene GyneMesh (polypropylene mesh). Methods:
All patients underwent multichannel urodynamics with their prolapse reduced to ensure there is no concomitant stress urinary incontinence. The patient is placed in a modified lithotomy position using Allen stirrups. The vagina is cleansed with betadine solution and draped in a sterile conventional manner. The midline of the anterior vaginal wall (AVW) is infused with 10 ml of 1 % lidocaine with 1:200000 adrenaline solution. The AVW is incised vertically one centimeter proximal to the external urethral meatus. The cystocele is mobilised and reduced. A 1-0 vicryl stitch is placed at the bladder neck (Kelly suture). The mesh is trimmed to adequately reduce the cystocele. A series of 2-0 vicryl stitches incorporate pubocervical fascia and the mesh. Redundant AVW is excised. The AVW is closed with continuous locking 1-0 vicryl suture. The bladder is drained with a Bonnano suprapubic catheter for 24-48 hours. Result:
This vaginal procedure provides excellent and expedient reduction of recurrent cystocele. With one year's experience we have not encountered any recurrent cystocele, urethral or bladder injury, mesh erosion, de novo urgency or urinary retention.

Many women in the menopausal and perimenopausal age range, require surgery for pelvic support defects such as cystocoele (bladder dropping), rectocoele (rectum protruding), bladder neck dropping (stress incontinence), and uterine or post hysterectomy vaginal vault prolapse. There is not much information about these subjects in the usual health media so it is difficult for women to know what to expect as far as success rates, complications and recovery in general. At a past meeting of the Society of Gynecologic Surgeons, some of the scientific papers provided answers to questions that women may have about these subjects.
How often do normal perimenopausal women lose urine with cough, sneeze or increase in abdominal pressure? In one study by Robinson D et al., women aged 45-55 who did not present for incontinence or evaluation of pelvic relaxation, were questioned to see how often they had episodes of urinary loss with intraabdominal straining. Some degree of urinary loss was reported by 2/3's (66%) but daily loss was only reported by 8%.

    What is a cystocele?
    Cystocele is the name for a hernia-like disorder in women that occurs when the wall between the bladder and the vagina weakens, causing the bladder to drop or sag into the vagina.
    What are the results of a cystocele?
    In addition to discomfort, the resulting dropped bladder can cause two kinds of problems to occur, including the following:
    • urine leakage incomplete emptying of the bladder
    The dropped bladder stretches the opening into the urethra, and urine may leak when a woman does any action that causes pressure on the bladder, such as coughing.

    to a friend Search this site When the supports to the pelvic floor are injured by childbirth or other causes, only one pelvic organ may prolapse. However, more often than not several types of pelvic organ prolapse are involved. These pelvic support defects are similar to hernias and include the following:
    • Rectocele (herniation of the rectum) - occurs when the tissue layers between the rectum and the vagina weaken, allowing the rectum to herniate and cause a bulging into the back of the vagina. Cystocele (herniation of the bladder) - occurs when the tissues between the bladder and the vagina weaken, allowing the bladder to bulge into the front of the vagina. Enterocele (herniation of the colon and small bowel) - occurs when the colon and/or small bowel herniate into the upper part of the vaginal wall.
    You may have one of these types of hernias alone or several at the same time. Depending on their severity, these defects can cause abnormal functioning of the bladder, rectum or vagina. As a result, you may experience urinary incontinence, difficult bowel movements, and pelvic pain including pain during sexual intercourse. Like other hernias, a rectocele, cystocele or enterocele can remain stable in size or get larger. These pelvic floor defects require surgery to repair.

    A patient information handout on using the pessary, written by the authors of this article, is provided on page 2729. The pessary is an effective tool in the management of a number of gynecologic problems. The pessary is most commonly used in the management of pelvic support defects such as cystocele and rectocele. Pessaries can also be used in the treatment of stress urinary incontinence. The wide variety of pessary styles may cause confusion for physicians during the initial selection of the pessary. However, an understanding of the different styles and their uses will enable physicians to make an appropriate choice. Complications can be minimized with simple vaginal hygiene and regular follow-up visits. (Am Fam Physician 2000;61:2719-26,2729.) T he pessary is one of the oldest medical devices available (Figure 1) . The type of pessary that is appropriate for each patient depends on the condition being treated.     It is well accepted that damage incurred during vaginal delivery predisposes to the development of prolapse. There is less agreement on the nature of the damage.     I am a 36 year old woman who recently had 2 children. After giving
    birth to the first, I began to notice a swelling in my vagina. I
    have had problems with sensory urgencyfeeling the need to urinate
    when standing or exercising. I have no pain from the vaginal swelling,
    but wonder if this is the cause of my urgency? I have seen a
    gyneocologist who did urodynamic testing and determined that I have
