ABSTRACT
Objective : To discuss the case of a 12-year-old female presenting with a mass in the right submandibular region consulting for chiropractic evaluation. Clinical features : A 12-year-old female, accompanied by her mother presented for chiropractic consultation regarding a diagnosis of cystic hygroma. Her mother stated that a mass had developed in her daughter's right submandibular muscle at approximately 5 years of age. Since the age of nine she had four surgical procedures in an effort to resolve the mass and the severe sinus drainage she experienced. All surgical procedures had failed to relieve her severe sinus drainage or contain the development of the mass. The mass was approximately the size of a golf ball on her first visit to the chiropractor. The subject has also suffered from daily morning neck pain and headaches for the past year. Intervention and Outcome
Conclusion
: A case report of a 12-year-old female with cystic hygroma is presented. Integral to her condition is the physical exam finding of significant left C2 lamina pedical point tenderness and associated paravertebral muscle spasms resulting in loss of vertebral motoricity of the cervical vertebral units. The care and treatment was approached with the hypothesis that reduction of the subluxation complex of the upper cervical region may result in improved sinus function and or lymphatic drainage. The successful resolution of the patient's cystic hygroma while under chiropractic care indicates the need for further study of the role of chiropractic care in the case of non-musculoskeletal conditions.

NEW ORLEANS First-trimester cystic hygroma is considerably more common in the general population than has been appreciated, Dr. Fergal D. Malone said at the annual meeting of the Society for Maternal-Fetal Medicine. This condition, which is characterized ultrasonographically by an enlarged hypoechoic space at the back of the fetal neck and visible septations, is present in approximately 1 in 300 pregnancies at 1014 weeks' gestation, according to the first prospective data analyzing its prevalence in an unselected population. And unlike third-trimester cystic hygroma, first-trimester cystic hygroma is strongly associated with a range of structural and chromosomal abnormalities.

Lymphangiomas account for 0.7-4.5% of mediastinal tumors, occurring most commonly in the superior and anterior mediastinum. While most of hygromas in the neck or lower face occur in the pediatric age group, isolated mediastinal lesions occur more commonly in adults. Most Patients are asymptomatic. A mass is first detected on routine chest radiograph. Pain may result from hemorrhage or infection in the cystic structures. Dyspnea may result from compression of major airway. Although lymphangioma is a benign tumor, its infiltrative tendency sometimes makes complete removal difficult and results in a high recurrence rate postoperatively. Etiology/Pathophysiology:
Lymphangiomas are tumor like congenital anomalies or hamartomatous changes consisting of endothelial-lined spaces that can vary from capillary size to several centimeters in diameter. Pathology:
A collection of dilated lymph vessels is noted. They may be supported by connective tissue stroma of variable thickness that contains lymphoid tissue, round cells, and smooth muscle. These lesions often contain a thin, clear or straw-colored fluid.
