Cervical cystic hygroma was first described clinically in 1828 by Redenbacker and pathologically in 1843 by Wernher. They are frequently seen in the neck (75%) and axilla (20%). Most mediastinal cystic hygromas are extensions of cervical lesions, and cystic hygroma confined solely to the mediastinum is rarely encountered. We report a case of a female child with a purely extensive mediastinal cystic hygroma, outlining aspects of its unusual clinical features and management. DuBose, M.S., RDMS, RT(R), FAIUM Director, Diagnostic Medical Sonography Program University of Arkansas for Medical Sciences, Little Rock, Arkansas, USA December 98 - Student Case Study This case study presents a 35-year-old white female who was referred to the UAMS High-Risk Obstetrical Clinic due to advanced maternal age. The patient and her husband received genetic counseling. This was the patient's first pregnancy. She presented with no obvious complications. A routine obstetrical ultrasound exam was performed by an ARDMS certified obstetrical sonographer using a 3.5 MHz transabdominal transducer. The sonogram showed a single intrauterine pregnancy with an estimated gestational age of 17 weeks, 4 days (7.5 weeks). On examination of the fetal head a large cystic mass presented around the posterior of the neck. The nuchal fold measured 9.5mm and the nuchal ligament could be seen within the cystic mass. These findings are consistent with the diagnosis of cystic hygroma. Cystic hygromas are caused by a delay or absence of the communication between the jugular lymph sacs and the internal jugular veins during fetal development. Late communication of the sacs to drain into the internal jugular vein results in the dilation of lymph sacs into cystic spaces, increased posterior nuchal measurement, and neck webbing. Complete obstruction of the jugular lymphatic is often associated with fetal hydrops, which is frequently fatal (Sanders, 1996). Another probable cause of cystic hygroma is abnormal budding of lymphatic endothelium. Increased nuchal thickness (nuchal lucency) can be detected at 10 weeks (Stevens 1998).

The cyst may be unilocular or multilocular and of variable size. The cyst wall is of variable thickness, and can be fibromuscular and contain mural thrombi. The fluid within the cyst is proteinaceous and may be haemorrhagic especially in combined lymphaticovenous malformations. 7080% of lymphangiomas occur in the neck, usually in the posterior cervical triangle. Other sites include the axillary region (20%), superior mediastinum ( Fig.1 ), mesentery, retroperitoneum, pelvis and lower limbs. Cystic hygromas can be present antenatally and are associated with a worse prognosis. They are known to be associated with Turners syndrome Noonans syndrome , trisomies, cardiac anomalies, fetal hydrops, fetal alcohol syndrome and familial pterygium colli. There is an 8090% mortality rate for antenatal cystic hygromas associated with fetal hydrops. Cystic hygromas grow with the child but may increase in size suddenly due to infection or haemorrhage. Spontaneous shrinkage may also occur. Clinically they appear as smooth soft tissue masses, usually before the age of 2. Imaging Plain films may show associated skeletal hypertrophy or distortion. Two types of cystic hygroma can be demonstrated at ultrasound. A macrocystic lesion appears as a multiloculated cystic mass with flow seen in the septa. Fluid levels may be seen following sudden enlargement. Microcystic lesions are hyperechoic with no flow on Doppler studies. CT demonstrates a low attenuation cystic mass with enhancement of the cyst wall following intravenous contrast administration. Higher attenuation values are present if haemorrhage has occurred and fluidfluid levels may be seen. MRI, the best technique for complete mapping (
