yes no Cystic Hygromas are non-malignant malformations of lymphatic vessels, usually occurring around the head and neck region. They are usually evident by 3 years of age. They may be single or multiple. If they are large they may be detected on antenatal ultrasound scans. They are usually slow-growing and may spontaneously resolve or reduce in size. Rapid growth does occasionally occur if there is internal bleeding into the cysts or if there is a complicating infection. Large cysts present at birth may necessitate a tracheostomy if there is pressure on the airway, though this is rare. Small cysts may not require any specific treatment except to wait and see how they develop. Larger cysts which continue to grow may need to be actively treated. Laser treatment to cystic hygromas in the mouth can help to reduce their bulk to help eating. Surgical reduction of these lesions in either the mouth or neck may also be helpful. An alternative treatment has been to use an attenuated bacteria (OK432) as an injection into the cysts, which causes a local sclerosing reaction which may help to reduce the size of the lesions. No hereditary association has been identified Prenatal diagnosis
In some cases the condition has been diagnosed through ultrasound scanning while in others it has remained undetected. This is because there is a wide variation in the extent of the condition which ranges from a small growth on the neck to a very extensive mass of growths.

Differential Diagnosis
The differential diagnosis for neck masses includes encephalocele, meningomyelocele, and teratoma. Teratomas are uncommon and are generally solid. Encephaloceles have associated calvarial abnormalities and contain brain tissue. Meningomyeloceles are uncommon in the cervical region and have associated spine abnormalities.

Discussion
Cystic hygromas, which usually occur in the neck, are most likely due to abnormal lymphatic development. Approximately half of these fetuses have monosomy X (Turner syndrome), while 10-15% of cases have trisomy 18, 21 or 13. Cystic hygromas are midline, posterior, septated, cystic masses without associated bone defects. They frequently contain a characteristic appearing midline nuchal ligament.

Philip A. Matorin, M.D. The nature of lymphatic malformations (LM) has sparked great interest since they were first described by Redenbacher in 1828. Many of the early researchers believed that lymphatic malformations were neoplasms. Currently, most researchers agree that LM are not neoplastic and have adopted the term "lymphatic malformation" to emphasize this fact. Two major theories of the development of the lymphatic system have been proposed to explain the origin of LM. Sabin proposed that the lymphatic system develops from five primitive sacs which sprout from the venous system. In the head and neck, endothelial outbuddings from the jugular sac spread centrifugally to form the lymphatic system. McClure and Huntington proposed that the lymphatic system develops from mesenchymal clefts in the venous plexus reticulum and spread centripetally toward the jugular sac. LM develop from sequestration or congenital blockage of the primitive lymphatic anlage.

Expectations (prognosis) Return to top The outlook is good if surgery can totally remove the abnormal tissue. In cases where complete removal is not possible because of involvement of neighboring nerves or blood vessels, recurrence is common.

Cystic hygromas (lymphangiomas)
are multiloculated cystic structures that are benign in nature. They form as the result of budding lymphatics and, thus, may occur anywhere in the body, although they are most frequently encountered in the neck (75%) and axilla (20%).

General Discussion
Cystic hygromas are non-malignant malformations of the lymphatic system characterized by single or multiple fluid-filled lesions that occur at sites where the lymphatic system connects to the venous system, most commonly the back of the neck. Typically, this disorder becomes apparent in infancy or early childhood. Ninety percent of cases are symptomatic by the age of two years.
Symptoms
Cystic hygroma is characterized by the presence of a sac, resembling a bag of water, usually at the nape of the neck but occasionally elsewhere. This sac, which is thin-walled and compressive, expands under the skin and is filled with lymph fluid and lymph cells.
