Issenman MD, Professor of Pediatrics, McMaster University, Chief of Pediatric Gastroenterology, Children's Hospital, Hamilton Health Sciences, Hamilton, Ontario, Canada Introduction Cyclic Vomiting Syndrome was first described about 120 years ago by Dr. Samuel Gee, the erudite British physician. Interest in the syndrome was revived when Kathleen Adams, a parent of an affected child founded the Cyclic Vomiting Syndrome Association in 1993. She enlisted the support of pediatric gastroenterologists Drs. David Fleisher and B U.K. Li, who recognized that better treatment of the disorder would only occur if the syndrome could be scientifically defined for medical researchers. The result of this effort has been three scientific meetings on the Cyclic Vomiting Syndrome. The first international meeting was in 1994 and the latter two in Milwaukee in 1998 and 2000. Cyclic vomiting syndrome

Definition
Cyclic vomiting syndrome (CVS) is a rare disorder characterized by recurring periods of vomiting in an otherwise normal child.
Description
Children in the pre-school or early school years are most susceptible to CVS, although it can appear anywhere from infancy to adulthood. This disorder was identified a century ago, but its cause is still unknown. Episodes can be triggered by emotional stress or infections, can last hours or days and can return at any time. Abdominal pain is a frequent feature.
The cause of CVS is still a mystery. Similarities to migraine suggest a common cause, but as yet no firm evidence has surfaced. Patients can usually identify some factor that precedes an attack. Vomiting can be protracted and lead to complications such as dehydration, chemical imbalances, tearing, burning and bleeding of the esophagus (swallowing tube). Between attacks there is no sign of any illness.
Diagnosis
The most important and difficult aspect of CVS is to be sure there is not an acute and life-threatening event in progress. So many diseases can cause vomitingfrom bowel obstruction to epilepsythat an accurate and timely diagnosis is critical. Because there is no way to prove the diagnosis of CVS, the physician must instead disprove every other diagnosis. This can be tedious, expensive, exhausting, and involve almost every system in the body. The first episode may be diagnosed as a stomach flu when nothing more serious turns up. Only after several episodes and several fruitless searches for a cause will a physician normally consider the diagnosis of CVS.

Mary Tuder: I'm Mary Tuder; I've got a son, Tim, who's 17, who's had cyclical vomiting since he was six.
Tim Tuder: I think the first time I was at a friend's birthday party. I was outside, I felt a bit sick, so I went inside and sat on the couch, and I think I just started vomiting up on their couch, and that was how it all started.
Mary Tuder: He would lie in his bed, as white as a ghost, with big black sunken eyes, and I actually thought he was going to die. Nothing would stop his vomiting at all.
Tim Tuder: I just felt really, really bad, and because we didn't know what was going on, it was even worse.
Rae Fry: Tim Tuder is actually fairly lucky, because unlike most people, he was diagnosed with cyclic vomiting syndrome relatively early. The first time Tim was admitted to Princess Margaret Hospital in Perth, the gastroenterologist on duty was Dr David Forbes. He'd been curious about cyclic vomiting since his days as a trainee paediatrician, so he recognised the patterns of the illness.

Background
Gastrointestinal symptoms such as cyclic vomiting are commonly encountered in children with mitochondrial disorders. The value of acupuncture as a complimentary therapy in controlling nausea and vomiting in this setting is not well known.
Objective To describe the use of acupuncture as a complementary modality for a case of mitochondrial DNA deletion disorder and cyclic vomiting syndrome.
Patient A 11/2-year-old girl with mitochondrial DNA deletion disorder was evaluated for the treatment of severe cyclic vomiting syndrome.

Occasional vomiting is as much a part of childhood as skinned knees, but there are some children who vomit repeatedly for hours, sometimes days, on end. They suffer from cyclic vomiting syndrome, a mysterious disease that affects mostly children and young adults. Johns Hopkins pediatric gastroenterologist Dr. Richard Katz says parents who are concerned that their child may have this disease need to pay careful attention. These can last from one to four days with periodic and repetitive vomiting. The children will often have excessive drooling, will not be awake or to the parents they don't appear to be conscious but they are conscious. The patients have had episodes occurring repetitively with many days or weeks in between. So to the parents, these are occurring as a fairly normal cycle of things, so that needs to be explained very carefully to any health care provider. Cyclic vomiting is difficult to diagnose and needs to be treated by a specialist. Supportive care and medication can often reduce the symptoms.

In about 1/3 of children with CVS, the vomiting episodes resolve as they mature and are replaced by episodes of migraine headaches. 4. Despite the high frequency of emeses, affected children remain very active during episodes. 5. The episodes usually end abruptly, resolution of symptoms is usually preceded by an end-of-episode sleep.
