Cyclic vomiting syndrome
Definition
Cyclic vomiting syndrome (CVS) is a rare idiopathic disorder characterized by recurring periods of vomiting in an otherwise normal child or adult. It was first described in 1882 by a physician named Gee. CVS is sometimes called abdominal migraine because it may be caused by some of the same mechanisms in the central nervous system that cause migraine headaches. Description Children in the pre-school or early school years are most susceptible to CVS, although it can appear anywhere from infancy to adulthood. One doctor reports that 43 of the 233 patients with CVS that he has treated were adults when their symptoms began. The average age of patients at onset is 5.2 years, but CVS has been diagnosed in patients as old as 73. This disorder was identified over a century ago, but its cause is still unknown. Episodes can be triggered by emotional stress or infections (particularly sinusitis), can last hours or days, and can return at any time. Abdominal pain is a frequent feature. CVS appears to affect all races equally. The female: male ratio has been reported as 11: 9.

General Discussion Cyclic vomiting syndrome is a rare digestive disorder that affects children and adults. This disorder is characterized by chronic nausea, vomiting, extreme fatigue, motion sickness, abdominal pain and, in some cases, dizziness (vertigo) that may last for hours to days. These episodes of symptoms seem to be similar in onset and duration for each affected individual. The exact cause of cyclic vomiting syndrome is not known. Hi, have the doctor's suggested Cyclic Vomiting syndrome? My daughter was diagnosed with it about 4 years ago. She would vomit everyday for about 3 weeks and stop for about one. She would also run a low grade fever before the cycle started. We have been to hell and back with this situation. She also suffers a lot with migraines now. I have a son with even a worse migraine problem. If you read material on CVS, you will find they mention treatment with a drug I can't remember the name of now, but it was unfamiliar. I began to search it out and found out it is Dramamine. Hope any of this helps. Feel free to email me if you would like to talk more. My daughter's stomach is extremely sensitive now and can hardly take any medications. Tina Next Article

My 17 year old son has suffered from cyclic vomiting syndrome (CVS) since he was a toddler. He has had all the standard tests to rule out other causes, the most recent being a HIDA scan of the gallbladder and biliary system. The scan showed normal structures, and no evidence of stones or obstructions, but a contraction rate of only 30%. He has never had RUQ pain associated with his vomiting episodes, nor at any other time. Nevertheless, his doctor has scheduled him for a cholecystectomy (gall bladder removal). My son is understandably willing to try anything that might resolve his CVS, but I question whether there is any reason to think this surgery will help. I am also concerned that he may have problems after the cholecystectomy, since he will have many years to live without the normal services of his gallbladder.
It seems to me that his doctor is just grasping at straws to find something that might help the CVS. However, I have searched the internet for many hours, and I can't find a single reference to link poor contraction rate of the gall bladder to CVS or vomiting, nor can I find any references that indicates cholecystectomy is in order when RUQ pain is not present, and the gall bladder has always been asymptomatic.

(as defined by the The National Digestive Diseases Information Clearinghouse (NDDIC) in Publication No. 99-4548 (e-text last updated: February 2000) Cyclic vomiting syndrome is a childhood disorder characterized by bouts of vomiting that last from a few hours to several days. The vomiting episodes occur regularly: Some children have them every few days, others every few months. Although researchers do not know what causes CVS, many patients can identify particular events that seem to trigger the vomiting. The three most common events are distress, excitement, or illness. Diagnosis can be difficult because CVS does not appear to leave clues that can be found by medical tests like x-rays or blood tests. The only way doctors can diagnose CVS is to rule out other diseases and wait to see whether a pattern to the vomiting can be identified.

Background In 1994, the 1st International Symposium on Cyclic Vomiting Syndrome was held. Researchers there proposed relatively strict guidelines for the diagnosis of Cyclic Vomiting: 1. A child has recurrent, severe episodes of vomiting which may last for hours or days but have intervals of complete normal health in between vomiting episodes. 2. No other known cause of the vomiting. The reason for having these criteria is that there are children who have vomiting for unknown reasons but do not have normal healthy intervals in between episodes. These children may be vomiting every day or every other day as opposed to children with Cyclic Vomiting who tend to go for weeks without vomiting episodes. This disorder is also known as abdominal migraine . At the 2nd International Scientific Symposium on CVS in 1998, one of the major conclusions of the meeting was the recognition of the close relationship between CVS and migraine. Some children with CVS may evolve to classic migraine. Stress may be an important precipitant of vomiting for some children and psychological assessment and treatment may be helpful for children and families. During treatment of an acute episode of vomiting, close attention to the amount of fluid that is replaced is important as too much fluid as this may stimulate some of the chemicals which will perpetuate vomiting. It is important to ensure a good supply of sugar during this process.
