Remember, the adrenal gland has a center gland (medulla) which produces adrenaline , and an outer gland (the cortex) which produces several hormones such as cortisol an aldosterone. This page is about cortisol overproduction by the adrenal cortex.
In 1932, a physician by the name of Harvey Cushing described eight patients with central body obesity, glucose intolerance, hypertension, excess hair growth, osteoporosis, kidney stones, menstrual irregularity, and emotional liability. It is now known that these symptoms are the result of excess production of cortisol by the adrenal glands. Cortisol is a powerful steroid hormone, and excess cortisol has detrimental effects on many cells throughout the body. Although some of these symptoms are common by themselves, the combination of these suggests that a workup for this disease may be in order. Keep in mind that Cushings syndrome is rare, occurring in only about 10 patients per one million population. On the other hand, simple obesity can be associated with some of these symptoms in the absence of an adrenal tumorthis is related to the slightly different mechanism by which normally produced steroids are metabolized by individuals who are obese. Note: The most common cause of excess steroids in the blood and its side effects, however, is long-term use of steroid medications for other disorders.

Cushing's Syndrome is well known in humans, cats and especially in dogs. It is also becoming more prevalent in horses, and yet there is little written about identifying it, and caring for horses who suffer from it. It is regarded as incurable in horses, though it is possible that horses can live reasonably happily for a long time whilst suffering from it.
CUSHING'S DISEASE IS HYPERADRENOCORTICISM
"Cushing's disease" is a term used in human medicine that refers to pituitary dependent hyperadrenocorticism. According to veterinary and human medical experts the disease is characterised by excessively high levels of steroids in the body. Steroids are compounds that circulate naturally in our bodies and our pets' bodies. Two major centres in the body regulate the levels of these steroids.
The main centre is a gland in the brain called the pituitary. This gland controls most of the hormones in the body. This disease is called hyperadrenocorticism.; (hyper = too much; adreno = adrenal gland; cort = cortex, the outer layer of the adrenal gland; ism = process). Blood tests often help the diagnosis of Cushing's Syndrome, and they indicate high levels of blood glucose. This would be similar to diabetes, which is very rare in horses, but is sometimes found in horses with Cushing's.

This can be caused by drugs (e.g. prednisone, depo-medrol, dexamethasone, betamethasone) often prescribed for the treatment of many diseases; this syndrome is known as iatrogenic Cushing’s syndrome. It can also be caused by an excess of the body’s own cortisol, caused by a pituitary tumor or adrenal tumor. Iatrogenic Cushing’s syndrome is caused by administration of drugs that suppress the body’s own cortisol production. Clinical signs and complications are similar to the naturally occuring disease. The dog can become dependent upon the drugs with chronic use, and abrupt withdrawal can lead to signs related to lack of cortisol, such as lethargy, depression, vomiting, and diarrhea. For these reasons, chronic steroid use is not recommended except in cases of life-threatening immune-mediated disease or chemotherapy protocols. The lowest possible dose of steroids should be used for therapy. Pituitary dependent Cushing’s disease is caused by a functional pituitary tumor that stimulates the adrenal glands to produce excess cortisol. The tumor is usually microscopic and benign (a microadenoma). Sometimes the tumor can be large enough to cause pressure on the nerves to the eyes and the part of the brain called the hypothalamus, causing behavior changes, lack of appetite, and blindness, in addition to the other signs of Cushing’s syndrome. These tumors are known as macroadenomas. Pituitary-dependent Cushing’s disease makes up about 85% of spontaneous Cushing’s disease cases.

What is Cushing's syndrome?
Cushing's syndrome is an extremely complex hormone condition that involves many areas of the body. Cushing's syndrome is the result of an excess of a hormone called "cortisol" and it's effects on the human body. Ironically one of the most common causes of Cushing's syndrome is the administration of "cortisol-like medications" to patients by doctors for the treatment of various diseases. All the other causes of Cushing's syndrome are the result of excess production of cortisol by the adrenal glands. What causes Cushing's syndrome?
Cushing's syndrome can be caused by any other condition that abnormally increases the production of cortisol by the adrenal glands either indirectly or directly. There are too many to list here but the most common causes are:
  • An abnormal growth of the pituitary gland associated with an excessive production of one of its hormones, adrenocorticotropic hormone (ACTH), which stimulates the adrenal gland. This specific form of Cushing's syndrome is referred to as Cushing's disease;

    are special chemicals which are produced then travel all over the body in the blood to do their work. The Adrenals (Latin for 'Near the Kidney') normally produce a hormone called Cortisol (also called Hydrocortisone). Cortisol is essential for life and health, but too much of it can be very harmful. When the adrenal glands become overactive, the normal healthy control of Cortisol breaks down. When too much Cortisol is produced, Cushings is the result. There are several forms of Cushings, each with it's own particular treatment. a) Pituitary Cushings. This is the most common and accounts for around 60% of all cases of Cushings. As its name suggests, the problem lays in the pituitary gland, a small gland below the brain, just behind the nose. This pea-sized chemical factory does many jobs for the body, one of which is the control of the amount of Cortisol the adrenal glands make. It does this by producing another hormone called ACTH

    There are many clinical signs that might be seen with Cushing’s syndrome. These include increased water consumption and urination, ravenous appetite, pot bellied appearance, thin skin, muscle wasting or weakness, ligament tears (commonly the cruciate ligaments of the knees), hair loss, panting, lethargy, recurrent urinary tract infections or neurological abnormalities. Many pets exhibit several of the listed clinical signs, but rarely does any one pet have all of the signs. Cats with Cushing’s syndrome are often diabetic and also have very thin skin that tears easily. How is Cushing’s syndrome diagnosed? How is Cushing’s Syndrome treated? Ketoconazole is an antifungal medication, which interferes with the production of cortisol. This medication is much more expensive than Mitotane and often less effective, but it is a viable alternative, especially in the short term if the pet requires surgery or if there is an adrenal tumor that is too large or invasive to be removed. L-Deprenyl (Anipryl) is another medication that might be recommended for some pets with pituitary dependent Cushing’s syndrome. This medication may help to alleviate some of the clinical signs by decreasing the amount of ACTH released from the pituitary gland. This medication hasn’t proven to be effective in all pets with pituitary disease and doesn’t suppress cortisol production like Lysodren does, but it has the benefit of being less toxic.

Skin becomes thin, fragile and easily bruised. The bones become thin and fracture easily. Women usually have excess hair on their faces, necks, chests, abdomen and thighs. Their menstrual periods may become irregular or stop. Men have decreased fertility and diminished or absent sex drive. A diagnosis may be made based upon symptoms and by measuring the hormones in blood and urine. CT or MRI scanning of the pituitary and adrenal glands and x-rays of the lungs may also be done.

It is caused by excessive cortisol, which is a naturally produced steroid in the animal's body by the adrenal glands. Cushings is a brain-endocrine disorder that can affect many of the functions of the dogs body and is considered a medically complex disorder While cortisol is paramount to healthy functioning in normal amounts, excess can lead to a debilitating existence for once happy pets. Early detection of Cushings syndrome is important because of the many bodily functions that are affected by the disease. bodyOffer(33094) Because the onset of Cushing's Syndrome is slow, symptoms of the disease are often mistaken for signs of aging. Reduced activity, changes in appetite and hair loss are commonly overlooked by pet owners and veterinarians alike. Other symptoms of the disease include an increased thirst in the animal, increased unrination, the appearance of being overweight, muscle weakness, panting and a loss of housetraining. If Cushings disease is suspected in a pet, the veterinarian will order blood and urine tests. The blood tests will help the veterinarian pinpoint the exact cause of the disease in the dog. The veterinarian will check liver enzymes, white cell counts and red cell levels. The urine test will be used to measure the levels of cortisone and creatinine in the system. An ultrasound of the adrenal glands may also be ordered to see if the glands are enlarged and pinpoint tumors. The treatment the dog receives will depend on whether the disease is ptiuitary or adrenal. Complications of Cushings syndrome can include high blood pressure, congestive heart failure, blood clotting, diabetes mellitus and kidney disease.
