In 1987 Dr Costello a paediatrician in New Zealand, published in the Australian Paediatric Journal a report of two children with similar physical characteristics and mild intellectual handicap. He had published a brief description of the children in a New Zealand Medical Journal some six years before. In 199I Dr Der Kaloustian reported a similar child and suggested that this particular pattern of growth, behavioural and physical features be called Costello Syndrome. The word syndrome is derived from the old Greek and merely means "running together." There are many thousands of syndromes in the medical literature and they often have the name of the first person to recognise that particular constellation of physical and behavioural and developmental characteristics are related with a common, even if unknown cause. Dr Costello in 1996 published a follow up of his original cases and reviewed the characteristics of 16 children in the literature with the diagnoses of Costello Syndrome. If one excluded the cases where the diagnoses has been disputed, there are published reports now on over 26 children. Children with Costello Syndrome have normal birth weight, and may be large at birth with birth weights in the upper normal range. The pregnancies have most often been described as normal but there are occasional reports of decreased foetal movements and there is excess fluid around the baby in approximately a third of the pregnancies.

Costello Syndrome By Dr Trisha Macnair Costello Syndrome is a very rare inherited disorder which was first reported in 1987 by a paediatrician in New Zealand who described two children with similar physical features and mild learning disabilities. In Costello syndrome there is a characteristic pattern of growth, behavioural problems and physical appearance. Children with Costello Syndrome are born with a normal or slightly high birth weight. Occasionally the mother has reported decreased foetal movement during the pregnancy and in 1 in 3 cases there is excessive fluid around the baby (polyhydramnios). Some of the typical features, which become more obvious as the child grows, include short stature and delayed bone age, redundant skin around the neck, palms, soles, and fingers, curly hair, papillomata (small fleshy growths) around the mouth and nose, a characteristic facial appearance (epicanthal folds, depressed nasal bridge, large earlobes), pigmented skin, cataracts, and premature greying of hair. Heart problems such as Hypertrophic Cardiomyopathy or abnormal rhythms are common. Another common problem is reflux or flowing back of the contents of the stomach into the gullet, which may cause feeding problems. There may be an increased risk of various cancers. Affected children may have learning difficulties, but many have warm social personalities.

Creutzfeldt-Jakob Disease By Dr Trisha Macnair Creutzfeldt-Jakob Disease (CJD) has hit the headlines in recent years because of the link between cattle infected with BSE (Bovine Spongiform Encephalitis) and a new variant of CJD (vCJD) in young people. But there are several types of CJD and infection isn't the only clue to this mysterious condition - it's not entirely clear how but cases of CJD may be of an inherited, infectious or unknown origin. CJD is a prion disease - prions are infectious disease-causing agents composed of a modified protein that cause a group of invariably fatal degenerative diseases of nervous tissues by a unique mechanism. The gene involved is known as the PRP gene. We all have the gene and it normally produces a protein, but this protein can change shape into a prion protein - a tough and sticky form which destroys the cells. Why it changes into a prion isn't clear but mutations of the PRP gene are an important part of the story. There are several different mutations of the PRP gene - one type first discovered in a group of Libyan Jews causes a specific type of CJD while the typical form of CJD is due to a different mutation. Different mutations in the PRP gene may lead to different prion diseases known as Gerstmann-Straussler-Scheinker disease (GSS) and fatal familial insomnia.

Symptoms
Costello syndrome is characterized by growth delays after birth (postnatal) leading to short stature; the presence of excessive, loose skin (cutis laxa) that lacks elasticity on the neck, palms of the hands, fingers, and soles of the feet; the development of benign (non-cancerous) growths (papillomata) around the mouth and nostrils (nares); mental retardation; and/or a distinctive facial appearance.
Infants with Costello syndrome typically have a normal or high birth weight, but exhibit poor sucking ability, have swallowing difficulties, and fail to grow and gain weight at the expected rate (failure to thrive). Growth delay after birth (postnatally) typically results in short stature during childhood and adulthood. Affected children may also have mild to moderate mental retardation. In some cases, speech development and/or the ability to walk is significantly delayed. Children with Costello syndrome generally have warm, sociable personalities.
