Right-sided heart failure - cor pulmonale
Definition
Failure of the right side of the heart caused by prolonged high blood pressure in the pulmonary artery and right ventricle of the heart.
Causes and Risk
Normally, the left side of the heart produces a higher level of blood pressure in order to pump blood to the body; the right side pumps blood through the lungs under much lower pressure. Any condition that leads to prolonged high blood pressure in the arteries or veins of the lungs (called pulmonary hypertension ) will be poorly tolerated by the right ventricle of the heart. When this right ventricle fails or is unable to properly pump against these abnormally high pressures, this is called cor pulmonale. Almost any chronic lung disease or condition causing prolonged low blood oxygen can lead to cor pulmonale. A few of these causes include:

PHYSICIAN ASSISTANT 2002 June;26(6):21-31 Abstract Secondary pulmonary hypertension (SPH) is caused by a number of secondary disorders, with chronic obstructive pulmonary disease as the most common. Many primary disease processes mask SPH, often leaving the condition misdiagnosed. With knowledge of the pathophysiology, clinical presentation, complications, diagnostic approach, and management, primary care clinicians will be ready to reverse the effects of this common condition. Pectus excavatum has never been reported to cause hypercapnic respiratory failure. In this report, we describe the first such case in a young woman with severe pectus excavatum who presented with chronic respiratory acidosis, pulmonary hypertension, and chronic cor pulmonale. An extensive diagnostic workup failed to uncover any other cause of respiratory acidosis, which led us to conclude that the severe chest wall deformity and the resulting severe restrictive defect were responsible for the development of chronic respiratory acidosis and cor pulmonale. (CHEST 2001; 119:1957-1961) Key words: alveolar hypoventilation; cor pulmonale; pectus excavatum; respiratory failure

ABSTRACT Here, are reported two patients with obstruction of superior airways with echocardiographic findings of severe pulmonary hypertension and cor pulmonale, both patients were diagnosed of obstructive sleep apnea by polisomnographic study. Due to this findings and the little response to medical management the patients were taken to surgery resulting in improvement of symptoms. Key words: Obstructive sleep apnea, cor pulmonale, tonsillar hypertrophy.
