Echocardiography and MRI usually indicates increase in thickness of the right ventricular myocardium, and increase in the size of the right-sided chambers. Echocardiography demonstrates associated pulmonary and tricuspid regurgitation. Doppler echocardiography can be used to estimate the pulmonary artery pressure by measuring the peak velocity of the tricuspid regurgitant jet and applying the modified Bernoulli equation

In children, the chronic lung disease is usually bronchopulmonary dysplasia but chronic hypoxic states also cause a rise in pulmonary artery pressures and, if unrelieved, can progress to cor pulmonale. Such conditions may occur with sleep apnoea Ondines curse , obesity, chronic hypoventilation states associated with brain stem injury or hypotonia and obstruction of the upper airways due to enlarged tonsils, tonsils enlarged , and adenoids, or micrognathia. GENERAL INFORMATION: What is it? Cor pulmonale (kor pull-muh-nall-e) is when the right side of your heart begins to fail. Heart failure means that the pumping power of the heart is weaker than normal. Causes: Cor pulmonale is caused by high blood pressure in the lungs. This high blood pressure may happen if you have lung problems, such as pulmonary embolism (m-bull-ih-zum). A pulmonary embolism is a blood clot closing one of the arteries in the lung. Chronic obstructive pulmonary (pull-mun-air-e) disease or "COPD" can also cause cor pulmonale. Or, other problems with the blood vessels in the lungs can cause right-sided heart failure. Signs and Symptoms: You may have one or more of the following signs or symptoms.
  • Shortness of breath, fast breathing, or breathing that worsens later in the day and at night.
    Cyanosis (sih-uh-no-sis) which is blue fingernails, toenails, or lips.
    Fatigue (tiredness).

Pathophysiology
As described elsewhere in this monograph, COPD is, in most patients, a mixture of two or three related pulmonary diseases. These include asthmatic bronchitis and chronic bronchitis, which involve inflammation of the airways, bronchi, and bronchioles; and pulmonary emphysema, which affects the lung parenchyma through destruction of alveolar walls and their associated capillary circulatory framework. In emphysema, the capillary bed is progressively and irreversibly destroyed, which eventually raises pulmonary vascular resistance and pulmonary artery pressure. Persistent and worsening pulmonary hypertension, in turn, finally results in cor pulmonale. Cor pulmonale usually appears only in the very advanced stages of emphysema.

Cor Pulmonale and Pulmonary Hypertension
Cor pulmonale is right heart failure that results from increased pulmonary pressure. Cor pulmonale is sometimes also called right ventricular hypertrophy, a condition in which the right ventricle is damaged and blood flow is interrupted. The right ventricle is a pumping chamber in the lower part of the heart responsible for exporting oxygen poor blood to the lungs for oxygen renewal. Cor pulmonale may result from pulmonary hypertension can develop as follows: There are thousands of capillaries in the lung that export oxygen rich blood to pulmonary arteries and import ox ygen depleted blood for oxygen renewal. The interior of these capillaries are lined with endothelial cells (cells that prevent blood from leaking out). The outer walls are made up of a muscle type cells that expand and contract to propel blood through the blood vessel. When primary pulmonary hypertension occurs, the endothelial cells are weakened so that they allow leakage. This leakage in turn causes the muscles type cells surrounding the blood vessels to constrict and inhibit blood flow. The constriction gradually worsens, increasing the pulmonary arterial pressure, eventually causing cor pulmonale. The initial symptoms of pulmonary hypertension are generally common complaints. Patients typically experience only a general fatigue or tiredness and in most cases imagine that they are simply "out-of-shape." As the condition worsens and right ventricular hypertrophy develops, patients experience more acute symptoms such as difficulty in breathing, dizziness and even fainting spells. As time passes, more severe symptoms; swelling in the ankles or legs, bluish discoloration of the lips and skin, and chest pain also develop.
