DEFINICION Es la cardiopatía relacionada con la sobrecarga de las cavidades derechas ocasionada por una neumopatía crónica, cambios en la circulación pulmonar o alteraciones en la pared del tórax que constituyen una barrera para el vaciado del ventrículo derecho. ETIOLOGIA Las entidades que pueden desarrollar cor pulmonale suelen ser divididas en tres grandes grupos:
  • Enfermedades del parenquina pulmonar y las vías aéreas Afecciones de los vasos pulmonares Patología de la caja torácica
  • En el primer grupo se incluyen la mayoría de las entidades causantes de CP (EPOC, fibrosis quística, enfermedades granulomatosas, resecciones pulmonares y enfermedad de las alturas). En las alteraciones que compromenten los vasos pulmonares, se ubican la hipertensión pulmonar primaria, la arteritis granulomatosa, el tromboembolismo pulmonar y las vasculitis. La caja torácica se compromete por la existencia de cifoescoliosis, toracoplastia y fibrosis pleurales. La hipoventilación alveolar es la causa determinante del cor pulmonale agudo y crónico.     Cor pulmonale is an increase in bulk of the right ventricle of the heart, generally caused by chronic diseases or malfunction of the lungs. This condition can lead to heart failure Description
    Cor pulmonale, or pulmonary heart disease, occurs in 25% of patients with chronic obstructive pulmonary disease (COPD). In fact, about 85% of patients diagnosed with cor pulmonale have COPD. Chronic bronchitis and emphysema are types of COPD. High blood pressure in the blood vessels of the lungs ( pulmonary hypertension ) causes the enlargement of the right ventricle. In addition to COPD, cor pulmonale may also be caused by lung diseases, such as cystic fibrosis , pulmonary embolism and, pneumoconiosis. Loss of lung tissue after lung surgery or certain chest-wall disturbances can produce cor pulmonale, as can neuromuscular diseases, such as muscular dystrophy . A large pulmonary thromboembolism (blood clot) may lead to acute cor pulmonale.
    Any respiratory disease or malfunction which affects the circulatory system of the lungs may lead to cor pulmonale. These circulatory changes cause the right ventricle to compensate for the extra work required to pump blood through the lungs. The right ventricle has thin walls and is crescent-shaped. The resulting pressure causes the right ventricle to dilate and bulge, eventually leading to its failure.

    GENERAL INFORMATION: What is it? Cor pulmonale (kor pull-muh-nall-e) is when the right side of your heart begins to fail. Heart failure means that the pumping power of the heart is weaker than normal. Causes: Cor pulmonale is caused by high blood pressure in the lungs. This high blood pressure may happen if you have lung problems, such as pulmonary embolism (m-bull-ih-zum). A pulmonary embolism is a blood clot closing one of the arteries in the lung. Chronic obstructive pulmonary (pull-mun-air-e) disease or "COPD" can also cause cor pulmonale. Or, other problems with the blood vessels in the lungs can cause right-sided heart failure. Signs and Symptoms: You may have one or more of the following signs or symptoms.

    What is cor pulmonale?
    Cor pulmonale is enlargement of the heart's right pumping chamber (right ventricle) due to high blood pressure in the lungs. The most common causes are chronic lung diseases such as emphysema (also referred to as chronic obstructive pulmonary disease, or COPD), pulmonary fibrosis, chronic bronchitis, and sleep apnea. All these conditions can increase the resistance to blood flow through the lungs. This increases the workload of the right ventricle until it can't quite keep up with the demand. Cor pulmonale usually occurs late in the course of lung disease. Less commonly, cor pulmonale may be caused by small blood clots that repeatedly come from veins lower in the body and lodge in the lung blood vessels. Over time these clots, or emboli, tend to block blood flow and increase the work of the heart's right ventricle.

    Es la insuficiencia del lado derecho del coraz³n causada por una presi³n arterial alta prolongada en la arteria pulmonar y en el ventr­culo derecho del coraz³n.
    Causas, incidencia y factores de riesgo: Normalmente, el lado izquierdo del coraz³n produce un nivel mayor de presi³n sangu­nea con el fin de bombear sangre al cuerpo; el lado derecho bombea a trav©s de los pulmones bajo una presi³n mucho m¡s baja. Cualquier condici³n que lleve a un aumento prolongado de la presi³n en las arterias o venas de los pulmones (denominada hipertensi³n pulmonar) no ser¡ bien tolerada por el ventr­culo card­aco derecho.     Congestive Heart Failure (CHF)
    Congestive heart failure (CHF) is a serious condition in which the cardiac muscle is not pumping well enough to meet the body's demand for oxygen. This decreased cardiac output can be quantified with a measurement called the ejection fraction. Congested heart failure is sometimes referred to as cardiac asthma. Right sided heart failure is often called cor pulmonale. Contrary to its name, CHF does not necessarily indicate that the cardiac muscle has completely failed/stopped, which is the case when someone has gone into cardiac arrest. It means that the cardiac muscle is operating at a decreased efficiency level and, therefore, is working harder to try and make up for the shortcoming in function. For example, the cardiac muscle may pump more frequently to compensate for its weakened pumping ability. The longer the cardiac muscle overworks itself to compensate for its shortcomings, the more its pumping ability is damaged and the more likely that serious pumping failure will result. The increased cardiac workload may lead to changes in various parts of the body, including an
