Conversion Disorder Project (restricted access) Conversion Disorder is a common problem in clinical neurology. In Motor Conversion Disorder there is partial or total paralysis of one or several body parts; the clinician judges that the symptom is involuntary and there is no evidence of conventional neurological pathology on examination or investigation. Although the diagnosis of Conversion Disorder should be made with care, recent studies indicate that the rate of misdiagnosis is low in contemporary practice. There must be some neurobiological explanation for paralysis seen in Conversion Disorder. At the very least there is a failure to activate cortical motor regions which directly drive movement. But whether this results from impaired activity in premotor centres, or via suppression of motor processing, is unclear. This question should be susceptible to study with functional imaging. Conversion disorder is an Axis I disorder and is one of seven disorders categorized under somatoform disorders in the fourth edition of the Diagnostic and Statistical Manual of Mental Disorders .  The others include somatization disorder, undifferentiated somatoform disorder, pain disorder, hypochondriasis, body dysmorphic disorder, and somatoform disorder not otherwise specified.  All of the somatoform disorders share the common feature of the presence of physical symptoms that suggest a medical condition but are not completely explained by a medical condition, another mental disorder, or the effects of a substance.  Conversion disorder is a disorder in which symptoms of voluntary motor or sensory dysfunction suggest a neurological or medical condition but cannot be explained by them.  Since biological reasons do not seem to explain the symptoms, psychological factors are often considered to be associated with them (DSM-IV, 1994, p.445).            There are four subtypes of conversion disorder, which DSM-IV (1994) categorizes based on the primary type of symptom or deficit that is presented.  The four subtypes are:

It also appears with greater frequency in populations where knowledge about medicine and psychology has not made its way into the public's mind. For example, rates are higher in developing countries than in industrialized ones. When this disorder occurs in a more sophisticated person, the symptoms tend to be subtler, while the symptoms are likely to be more far-fetched in a person who is medically naive.     Office Location The Department of Child and Adolescent Psychiatry provides an array of child and adolescent psychiatric services both here at Connecticut Children's Medical Center and at the Institute of Living On the Connecticut Children's campus, consultations are provided to inpatients and outpatients with special medical needs. Children in crisis are seen in either our ambulatory practice at the Institute of Living or in the emergency department. On the campus of the Institute of Living there are inpatient psychiatric services, partial hospitalization programs, a child guidance clinic and a special education school. Throughout the Hartford Hospital/IOL spectrum of care, children and adolescents are seen with Attention-Deficit Hyperactivity Disorder, Oppositional Defiant Disorder, Conduct Disorders, Anxiety Disorders, Mood Disorders including Depression, Developmental Disorders such as Autism, Pervasive Developmental Disorders and Asperger's Syndrome, and Somatoform Disorders such as Conversion and Somatization, and Psychological Factors Affecting Medical Illness. The Department will triage referrals to the appropriate clinician based on his/her area of expertise and availability. There is also a special Pediatric Psychiatric hotline particularly devised for referring community pediatricians at 860.545.7493 and a 24-hour emergency service available at 860.545.7200.
