Conversion disorder
Overview Symptoms Treatment Prevention Definition: The conversion of emotional distress or unconscious conflict into a physical symptom. It is one of the general class of Somatoform disorders.
Alternative Names: Hysterical neurosis
Causes, incidence, and risk factors: The symptom onset of this disorder is usually very sudden and follows a stressful experience. The loss of function may symbolize the underlying conflict associated with it. Psychodynamic theory interprets the cause of the symptoms as a defense mechanism that absorbs and neutralizes the anxiety generated by an unacceptable impulse or wish. Risk factors include a history of histrionic personality disorder or dependent personality disorder

Description The somatoform disorders are a group of mental disturbances placed in a common category in DSM-IV on the basis of their external symptoms. The terms hypochondriasis and hysteria were traditionally designated to these disorders and are still widely used. Somatoform disorders in the DSM-IV are described as follows: The somatoform disorders are distinguished by physical symptoms suggesting a medical condition, yet the symptoms are not fully explained by the medical condition, by substance use, or by another mental disorder.
The symptoms are severe enough to cause patients significant distress or impaired social, occupational, or other functioning.
The physical symptoms of somatoform disorders are not intentionally produced as are those of factitious disorders and malingering, but no medical condition can fully explain the somatic symptoms. The DSM-IV describes 5 subtypes: conversion disorder somatization disorder psychogenic pain disorder hypochondriasis and Body dysmorphic disorder Symptoms, Signs and Treatment of each Subtype

Symptoms
One or more Symptoms or deficits affecting voluntary motor or sensory function that suggest a neurological or other general medical condition. Psychological factors are judged to be associated with the symptom or deficit because the initiation or exacerbation of the symptom or deficit is preceded by conflicts or other stressors. The symptom or deficit is not intentionally produced or feigned (as in Factitious Disorder or Malingering). The symptom or deficit cannot, after appropriate investigation, be fully explained by a general medical condition, or by the direct effects of a substance, or as a culturally sanctioned behavior or experience. The symptom or deficit causes clinically significant distress or impairment in social, occupational, or other important areas of functioning or warrants medical evaluation. The symptom or deficit is not limited to pain or sexual dysfunction, does not occur exclusively during the course of Somatization Disorder, and is not better accounted for by another mental disorder.

Conversion Disorder Symptoms of Conversion Disorder Symptoms Description Weakness Weakness usually involves whole movements rather than muscle groups. Weakness affects the extremities more often than ocular, facial, or cervical movements. Sensory symptoms Sensory loss or distortion often is inconsistent when tested on more than one occasion and is incompatible with peripheral nerve or root distribution. Visual symptoms Visual symptoms include field defects, tunnel vision, and bilateral blindness. Gait disturbances Astasia-abasia is a motor coordination disorder characterized by the inability to stand despite normal ability to move legs when lying down or sitting. Pseudoseizures In contrast to true seizures, pseudoseizures occur in the presence of other people and not when the patient is alone or asleep.
Causes of Conversion Disorder
  • By definition, symptoms in a conversion reaction are caused by previous severe stress, emotional conflict, or an associated psychiatric disorder. Many studies confirm high incidence of depression in patients with conversion disorder. As many as half of these patients have personality disorders or display hysterical traits.
