I have a 10 year old daughter who has been recently diagnosed with conversion disorder. For the last three months she has been complaining of pain and is no longer weight-bearing, and is now confined to a wheelchair. We have reached the end of testing for organic cause for this and are beginning to pursue psychiatry. I am trying to make some sense of this situation and would welcome comments from others who have had similar experiences.     Alternate Names : Hysterical Neurosis Definition Conversion disorder is a psychiatric condition in which emotional distress or unconscious conflict are expressed through physical symptoms. Conversion disorder is one of several types of somatoform disorders, in which psychological problems produce physical symptoms. These disorders have typically been highly stigmatized, with physicians telling patients that the problem is "all in your head." Research on the connection between mind and body is steadily increasing our understanding of these disorders and should reduce the stigma, as it becomes clear that these conditions are real, cause real distress, and cannot just be turned on and off at will. The onset of symptoms in this disorder is usually very sudden and follows a stressful experience. Loss of function, such as in inability to move a limb, may unconsciously symbolize the underlying conflict associated with the experience. Risk factors include a history of histrionic personality disorder or

    Conversion Disorder
    DSM IV Criteria The patient complains of symptoms or deficits affecting voluntary muscles, or deficits of sensory function that suggest a neurological or medical condition. The temporal relation of symptoms to a stressful event suggests association of psychological factors. Symptoms are not intentionally produced. Symptoms are not explained by an organic etiology. Symptoms result in significant functional impairment. Symptoms are not limited to pain or sexual dysfunction, and are not explained. Symptoms most commonly are sensory (blindness, numbness) and motor deficits (paralysis, mutism). Abnormalities usually do not have an anatomical distribution and the neurological exam is normal. Patients often lack the characteristic normal concern about the deficit. This characteristic lack of concern has been termed A la belle indifference. Conversion disorder can coexist with depression, anxiety disorders, and schizophrenia. Conversion symptoms often will temporarily remit after the disorder has been suggested.

Somatoform Home In conversion disorder, a psychosocial conflict or need is converted into dramatic physical symptoms that affect voluntary motor or sensory functioning. The symptoms often seem neurological, such as paralysis, blindness, or loss of feeling (anesthesia). Most conversion disorders begin between late childhood and early adulthood, and are diagnosed twice as often in women as in men. Symptoms usually appear suddenly and during times of extreme stress, and last for several weeks. Diagnostic Criteria: A. One or more symptoms or deficits affecting voluntary motor or sensory function that suggest a neurological or other general medical condition. B. Psychological factors are judged to be associated with the symptom or deficit because the initiation or exacerbation of the symptom or deficit is preceded by conflicts or other stressors. C. The symptom or deficit is not intentionally produced or feigned (as in Factitious Disorder or Malingering).
