Within the last two decades, rapid advances in health care provision have been made in the Kingdom of Saudi Arabia.1 An example of this remarkable progress is the provision of excellent surgical care facilities for children with congenital heart disease (CHD), such that cardiac surgery is now a routine event in designated centers in the Kingdom. A complete medical examination, which is a prerequisite for cardiac surgery, entails the detection and evaluation of any associated non-cardiac defects (NCD). In this regard, a knowledge of the general incidence and types of such NCD in a particular region would be useful since it could serve as a guide during the preoperative assessment of individual patients. We have therefore studied the NCD in Saudi children who presented to us with CHD and report our findings.
Patients and Methods

    Definition
    Congenital heart disease, also called congenital heart defect, includes a variety of malformations of the heart or its major blood vessels that are present at birth.
    Description
    Congenital heart disease occurs when the heart or blood vessels near the heart do not develop properly before birth. Some infants are born with mild types of congenital heart disease, but most need surgery in order to survive. Patients who have had surgery are likely to experience other cardiac problems later in life. Most types of congenital heart disease obstruct the flow of blood in the heart or the nearby vessels, or cause an abnormal flow of blood through the heart. Rarer types of congenital heart disease occur when the newborn has only one ventricle, or when the pulmonary artery and the aorta come out of the same ventricle, or when one side of the heart is not completely formed. Patent ductus arteriosus Patent ductus arteriosus refers to the opening of a passageway-or temporary blood vessel (ductus)-to carry the blood from the heart to the aorta before birth, allowing blood to bypass the lungs, which are not yet functional. The ductus should close spontaneously in the first few hours or days after birth. When it does not close in the newborn, some of the blood that should flow through the aorta then returns to the lungs. Patent ductus arteriosus is common in premature babies, but rare in full-term babies. It has also been associated with mothers who had German

    What is Congenital Heart Disease? Congenital Heart Disease is a term used to describe heart defects that develop before birth. These defects result from a failure of the heart or the blood vessels near the heart to develop normally. Approximately one in 100 infants is born with a heart defect. In some cases, heredity, alcohol consumption or other drug intake during pregnancy, or an infection during pregnancy increase the incidence of heart defects. However, in most cases, the cause of the heart defect is not known. How is it diagnosed?
    There are many different types of heart defects and no two are exactly alike. Some defects are diagnosed before birth while others are not detected until birth or weeks or years after birth. If your primary physician suspects that your child has a congenital heart defect, he or she will most likely refer your child to a pediatric cardiologist for a specialized evaluation. In addition to providing a thorough physical examination, your child's cardiologist will use diagnostic tests such as electrocardiogram, echocardiogram, chest x-ray or cardiac catheterization to determine the type and severity of your child's heart defect.

    What is it?
    Congenital heart disease is a general term that is used to refer to a group of different heart abnormalities that a baby can be born with. The different abnormalities have different effects, but the kind of problems that they can lead to include a slow growth rate, difficulties with feeding and an abnormal breathing pattern. Babies and children with congenital heart disease may be particularly prone to infections. They also tend to get tired very easily.

    Consultation Services in Ambulatory Care Deborah's clinical cardiology services provide consultations for all patients with acute and chronic heart diseases. The scope of care includes congenital and acquired heart diseases including coronary artery disease, rheumatic heart disease, other forms of valvular and myocardial disease, cardiomyopathy and hypertensive heart disease. Included are disorders of the coronary and non-coronary circulation systems, myocardial function, cardiac conduction system and cardiac valves. Approximately 30 to 40 percent of cardiology patients seen at Deborah have valvular heart disease. Deborah physicians also focus on issues relating to women and heart disease and risk factor evaluation and modification. After assessment, the Deborah cardiologist always consults with the referring physician to determine the most appropriate therapy for the patient.

    Congenital heart diseases are abnormalities of the heart's structure and function caused by abnormal or disordered heart development before birth.
    Causes, incidence, and risk factors: Congenital heart disease (CHD) is a broad term that can describe a number of different abnormalities affecting the heart. Congenital heart disease is, by definition, present at birth although its effects may not be obvious immediately. In some cases, such as coarctation of the aorta , it may not present itself for many years and a few lesions such as a small ventricular septal defect VSD ) may never cause any problems and are compatible with normal physical activity and a normal life span.
    According to the American Heart Association, approximately 35,000 babies are born each year with some type of congenital heart defect. Congenital heart disease is responsible for more deaths in the first year of life than any other birth defects. Many of these defects need to be followed carefully; though some heal over time, others will require treatment Some congenital heart diseases can be treated with medication alone, while others require one or multiple surgical interventions. There has been significant improvement in the risk of death from congenital heart disease surgery, from approximately 30% in the 1960s and 1970s to approximately 5% today.

    Adult Congenital Heart Disease Program Advances in cardiac surgery in infants and children have resulted in a larger adult population living with congenital heart disease. The Joan and Michael Schneeweiss Center for Adult Congenital Heart Disease cares for patients who have had cardiac surgery earlier in life and those with newly diagnosed congenital heart disease. These patients often need interventions to address recurrent or residual lesions, in addition to ongoing treatment of the mechanical problems caused by congenital heart disease. Columbia University Medical Center has assembled a multidisciplinary team of physicians who treat myocardial malfunction, arrhythmia, and other problems with heart function. This group has refined procedures for D-transposition of the great arteries, single ventricle, Ebstein's anomaly, and aortic valve disease.
