What Is Traumatic Brain Injury?
Traumatic brain injury is sudden physical damage to the brain. The damage may be caused by the head forcefully hitting an object such as the dashboard of a car (closed head injury) or by something passing through the skull and piercing the brain, as in a gunshot wound (penetrating head injury). The major causes of head trauma are motor vehicle accidents. Other causes include falls, sports injuries, violent crimes, and child abuse. The physical, behavioral, or mental changes that may result from head trauma depend on the areas of the brain that are injured. Most injuries cause focal brain damage, damage confined to a small area of the brain. The focal damage is most often at the point where the head hits an object or where an object, such as a bullet, enters the brain. In addition to focal damage, closed head injuries frequently cause diffuse brain injuries or damage to several other areas of the brain. The diffuse damage occurs when the impact of the injury causes the brain to move back and forth against the inside of the bony skull. The frontal and temporal lobes of the brain, the major speech and language areas, often receive the most damage in this way because they sit in pockets of the skull that allow more room for the brain to shift and sustain injury. Because these major speech and language areas often receive damage, communication difficulties frequently occur following closed head injuries. Other problems may include

A speech-language pathologist is trained to diagnose and treat communication disorders such as those involving deficits in comprehension or generation of language, speech or voice production, resonance, or speech fluency. Speech-language pathologists are also trained to diagnose and treat swallowing disorders. A master's degree is required as the professional entry-level degree for speech-language pathology. An audiologist is trained to diagnose and manage disorders of hearing and auditory processing, to provide couseling and habilitation methods, and to apply a variety of technologies such as hearing aids and assistive listening devices to enhance hearing and auditory processing in persons diagnosed with impairment. A doctoral degree will be required by 2007 as the professional entry-level degree for audiology. Educational requirements of the American Speech-Language-Hearing Association (ASHA) specify that students receive a broad-based educational training involving both classroom instruction and clinical practicum. At UConn, clinical experience in speech language pathology and audiology are offered in the department's speech and hearing clinic as well as at many external sites. Sites within the UConn area include acute and rehabilitation hospitals, schools, out-patient centers, nursing homes, extended-care facilities, clinics, and private practive settings.

What Is Meant By "Communication Disorders"?
The term COMMUNICATION DISORDERS encompasses a wide variety of problems in language, speech, and hearing. Speech and language impairments include articulation problems, voice disorders, fluency problems (such as stuttering), aphasia (difficulty in using words, usually as a result of a brain injury), and delays in speech and/or language. Speech and language delays may be due to many factors, including environmental factors or hearing loss. Hearing impairments include partial hearing and deafness. Deafness may be defined as a loss sufficient to make auditory communication difficult or impossible without amplification. There are four types of hearing loss. Conductive hearing losses are caused by diseases or obstructions in the outer or middle ear and can usually be helped with a hearing aid. Sensorineural losses result from damage to the sensory hair cells of the inner ear or the nerves that supply it and may not respond to the use of a hearing aid. Mixed hearing losses are those in which the problem occurs both in the outer or middle ear and in the inner ear. A central hearing loss results from damage to the nerves or brain.

CD majors follow a prescribed program leading to eligibility for national certification in Speech-Language Pathology by the American Speech-Language- Hearing Association. Apraxia of speech is a motor-speech programming disorder resulting in difficulty coordinating the oral-motor movements necessary to produce and combine speech sounds (phonemes) to form syllables, words, phrases and sentences. Oral-motor weakness or dysarthria may coexist but must not be a primary concern. Apraxia of speech is usually treatable with the appropriate techniques. It is not just a simple articulation disorder nor a "phonological disorder" but a motor-speech programming disorder. Traditional therapy techniques are usually unsuccessful. "Minimal pairs" techniques are also not as successful of an approach. Speech pathologists should be knowledgeable about oral-verbal motor skills, and adult/acquired apraxia to assist the child best. They should be well-versed in "cueing," "pacing" and successive approximations. The Kaufman treatment method for developmental verbal apraxia is a highly effective program with young children. ( See schedule of workshops ). Children must be seen one-on-one, at least in the early stages of treatment, even by age two.
