Cold sores by Dr Trisha Macnair What is it? Symptoms Further help What is it? Cold sores are recurrent blistering spots that appear around the mouth and nose. What causes it? Cold sores are caused by an infection with the Herpes simplex virus. The virus is picked up through contact with someone who has active infection (i.e. from someone who has a cold sore - the virus is passed on in saliva). After an incubation period of 3-5 days the initial infection (sometimes called the primary infection) may cause a variety of problems, although in the majority of cases there are no symptoms and it goes unnoticed. After this the virus is not cleared from the body but lies dormant (inactive) deep in the nerve cells. Certain triggers can then reactivate the virus, causing it to travel down the nerve to the skin surface where it produces a cold sore. Triggers include illness, stress, UV light (i.e. exposure to bright sunshine) and cold. Who is affected?

Cold sores are caused by herpes simplex virus. There are two types, called type 1 and type 2. Either type can be caught on any part of the body: lips and genitals are the most common places. This leaflet is about facial sores.
On the face, herpes simplex type 1 is more likely to recur than type 2. How can it recur?
Once you have caught this virus, it hides in nearby nerve sheaths and can sometimes reactivate. There are other viruses that also hide and come back later: for example chickenpox/shingles, glandular fever and cytomegalovirus (CMV) but they do not cause cold sores. How did I get my cold sores?

Cold sores should not be confused with canker sores , which are small ulcerations in the mouth. Rather, cold sores are fluid-filled blisters that form on the borders of the lips. But for example, those who are malnourished, who are being treated for cancer, or who have a weakened immune sores can cause unusually severe illness. What Causes Cold Sores? Cold sores are caused by the herpes simplex virus 1 (HSV-1). Most people carry this virus in their bodies because they have been infected at some time in their lives, usually before the age of 5. After this primary infection in childhood, which may occur without symptoms or may mimic teething in an infant, the virus lies dormant in the nerves near the affected skin area. Herpes recurrences, or secondary episodes, produce fever blisters or cold sores. Many factors can trigger a cold sore outbreak:
  • Stress Fatigue Some foods Sun exposure Fever Excessive heat or cold Menstruation Pregnancy Trauma or injury to the mouth or lips, even that caused by dental work or shaving
Although the edge of the lip is the most common place for cold sores to appear, they can form almost anywhere inside the mouth, including the gums, tongue, or inside of the cheek.

Cold sores, also known as fever blisters, are small red blisters that crop up near the lips or on them. More rarely, they sprout on the roof of the mouth. (Some people confuse them with canker sores How did my child get a cold sore?
Most children pick up the virus by sharing food with or kissing someone who has a cold sore. They can also get the infection from someone who doesn't have a visible sore but has the virus in his or her saliva. Once your child gets the virus, it stays in his body for good, hiding in nerve cells near the ear. In some people, the virus lies dormant and never causes harm. In others, it periodically wakes up and triggers cold sores. Nobody knows what stirs the virus into action, but stress, fever, colds, and sunburn seem to encourage outbreaks. What are the symptoms?

Cold sores are very contagious, very common and usually appear on the mouth and/or lips. Studies show that herpes simplex virus 1 , the virus responsible for cold sores, is found in 30 to 60 percent of children by the age of 10. Once contracted, the virus remains hidden in the body in a dormant state until it is triggered to reactivate and cause a cold sore. The virus can be passed any time during an outbreak - from the first tingle until the affected area is completely healed. Adjacent lymph nodes may also become swollen.
