Pediatric plastic surgery is a medical specialty which aims to correct a child's physical deformity and restore a more normal appearance. As a discipline, this field covers a complex range of conditions. We treat children with birth defects (congenital defects) of the head, face, hands, feet, and other areas: traumatic injuries (accidents, burns); skin and superficial tumors (benign and malignant), and other problems.     Early in our development in the uterus, every single one of us has a cleft lip and a cleft palate, but as normal development continues during the first trimester, the tissues that form the palate and the upper lip come together, or fuse. If that fusion doesn't happen, children are born with a cleft lip and/or a cleft palate. How common? The American Society of Plastic Surgeons (ASPS) considers this birth defect (Read about " Birth Defects ") a common one. ASPS says it affects one in every 700 to 1000 births in the United States. The March of Dimes (MOD) calls this birth defect an oral-facial cleft. An oral-facial cleft, according to MOD, is an opening in a structure around the mouth and face. Clefts, according to MOD can occur on:
    • the lip
    • roof of the mouth (hard palate)
    • back of the mouth (soft palate)
    There are two major types of oral-facial clefts according to MOD:
    • Cleft Lip/Palate - in this case the baby has a cleft lip, usually accompanied by a cleft palate

    The face forms, in the first few weeks after conception, from several elements joining together: sometimes the process is not quite completed and the elements remain separate with a cleft between them, sometimes on one side (unilateral), sometimes on both sides (bilateral). The cleft may be complete or incomplete, and the problem can be picked up on a scan during the pregnancy.
    The palate (roof of the mouth) forms by the two shelves growing inward to separate the nasal space from the mouth. Sometimes this process is not quite completed and a cleft remains. This is not visible on a scan.
    The Team
    Although a cleft is not a serious deformity in terms of the baby's health it affects the face, the jaws and the palate, and may affect appearance, the teeth and the swallowing and speech mechanism: sometimes there are problems with hearing.
    A team of experts is therefore required to work together to provide best care for the child from birth until growth is finished at about 18 years.
    Therefore plastic surgeon, orthodontist, oral and maxillo-facial, and ear nose and throat surgeons work together with nurses, counsellors, anaesthetists and speech and language therapists. A geneticist may advise on the chances of any other child having the same problem. Health visitors and parent support groups are also important.

    Common Information

    Babies may be born with this distressing deformity of the most obvious part of the face; if left unrepaired, beside the abnormality of appearance there would be serious problems with feeding, and later, with speech. Initial surgery is performed at Mount Vernon Hospital within a few days of birth, whereas in other centres it is delayed until much later. Our research is looking at the advantages and possible problems arising from such early operation, and is integrated with a National Research Programme co-ordinated by the British Association of Plastic Surgeons. The funds contributed to RAFT have, in part, been matched by money from The Department of Health. This has enabled the recent full-time appointment of a person to co-ordinate the care of children with clefts, and to provide a comprehensive database of all patients with this condition, so that year by year we can improve the standard of care.

    Question: How do I breastfeed a baby with cleft lip or palate?
    Answer: Most babies born with a cleft lip can breastfeed even before surgery . However, a baby with a cleft palate could have difficulty breastfeeding, depending on the severity of the cleft. In either case, a mom usually needs to experiment with breastfeeding positions so that her breast is supported in the baby's mouth, creating a seal. This is more difficult with a cleft palate, since the passage between the mouth and nose cannot be closed off, resulting in poor suction and preventing your baby from being able to keep your breast in his mouth. He may tire more quickly from working so hard to maintain suction. Try feeding him in an upright position using a modified football or clutch hold or by sitting him in your lap with his legs straddling your abdomen. (Learn more about other
