    Cleft lip and palate and other facial deformities Some form of cleft occurs in up to 1 in 500 live births. Most primary surgery is undertaken by plastic surgeons, although other specialities may be involved. However, the overall management of a patient with cleft lip and palate is a team effort involving orthodontists, ENT surgeons, speech therapists, paediatricians and maxillofacial surgeons, as well as plastic surgeons, all of whom should have expertise in this specialist field. Such teams should be centred in larger plastic surgery units so that manpower facilities and expertise gained from treating significant numbers of a relatively rare condition can be concentrated. This surgery (particularly the primary surgery) should not be carried out by the occasional operator and evidence indicates that primary surgery should not be carried out by those performing less than 20 primary operations per year. Facilities need to be available for the continuing care of these patients until adult life. Un-operated Cleft Lip

Cleft palate may occur in isolation, may be unilateral or bilateral and ranges from a bifid uvula to a complete cleft of hard and soft palate. It may also present in a submucous form. Clefts may be part of very many syndromes affecting the first and second branchial arches including the Pierre Robin anomaly.

    Determining surgical outcomes and quality of patient care are some of the primary forces that shape which surgical procedures and medical protocols are ultimately utilized. As insurers and patient-consumers demand more of an evidenced-based approach to medical care to contain costs and provide proven treatments, those technologies that facilitate this measurement are particularly worthy of consideration. Cleft lip and palate is a devastating congenital deformity affecting nearly 1 in 700 of children born in the United States and nearly 1 in 300 in children born in developing Asian countries. The treatment of this deformity requires a team approach utilizing the skills of surgeons, nursing personnel, speech pathologists, social workers, and orthodontists. Each team member contributes uniquely to the care of the individual patient, and must accurately record their observations and treatments. After this data is then entered into a master database, patient progress may be followed and the efficacy of treatments determined. Provider compliance in acquiring data is enhanced by capture systems that facilitate this process, leading to more complete outcomes measures. To this end, I developed a Palm OS database that incorporates the text descriptors specific for Cleft Lip and Palate surgery and a unique diagram that graphically records the anatomic side and site of the cleft lip and/or palate deformity. A cleft palate is an opening in the roof of the mouth in which the two sides of the palate did not join together. Cleft lips and palates can be unilateral or bilateral. Why did this happen? Sometime between the 6th and 11th week when your baby was developing, the parts of the lip and/or palate failed to come together properly. We all have clefts of our lips and palates initially. Some join together and others do not. Why some do not, we may never know. Sometimes clefts can be found in families who have had clefts in other family members. Some clefts can be linked to certain syndromes. Here are some statistics to help you understand the frequency of clefting:
    • Clefts occur in 1 in 700 newborns Cleft palates are not racially influenced Clefts occur in more males than females There is only a 5%, chance that another child will be born with a cleft. If another child is born with a cleft, the incidence increases.

    Cleft Palate Foundation CPF Cleft lip and cleft palate comprise the fourth most common birth defect in the United States. One of every 700 newborns is affected by cleft lip and/or cleft palate. A cleft lip is a separation of the two sides of the lip. The separation often includes the bones of the upper jaw and/or upper gum. A cleft palate is an opening in the roof of the mouth in which the two sides of the palate did not fuse, or join together, as the unborn baby was developing. Cleft lip and cleft palate can occur on one side (unilateral cleft lip and/or palate), or on both sides (bilateral cleft lip and/or palate). Because the lip and the palate develop separately, it is possible for the child to have a cleft lip, a cleft palate, or both cleft lip and cleft palate. Cleft lip and cleft palate are congenital defects, or birth defects, which occur very early in pregnancy. The majority of clefts appear to be due to a combination of genetics and environmental factors. The risks of recurrence of a cleft condition are dependent upon many factors, including the number of affected persons in the family, the closeness of affected relatives, the race and sex of all affected persons, and the severity of the clefts. A child born with a cleft frequently requires several different types of services, e.g., surgery, dental/orthodontic care, and speech therapy, all of which need to be provided in a coordinated manner over a period of years. This coordinated care is provided by interdisciplinary cleft palate/craniofacial teams comprised of professionals from a variety of health care disciplines who work together on the child's total rehabilitation.

    Quick Reference and Fact Sheets Cleft Lip and Cleft Palate
    A cleft is a separation in a body structure. Clefts that occur in the oral-facial region often involve the lip, the roof of the mouth (hard palate) or the soft tissue in the back of the mouth (soft palate). Two major types of oral-facial clefts are cleft lip/palate and isolated cleft palate. Babies with cleft lip/palate have a cleft lip which usually is accompanied by cleft palate. In isolated cleft palate, the cleft palate occurs by itself, without cleft lip or other malformations. These two forms of oral-facial clefts are considered separate birth defects. Here we refer to each specific type of cleft by name, and use the term oral-facial cleft when the information applies to both.
    There also are about 400 syndromes in which babies have some form of oral-facial cleft along with a wide variety of other birth defects. The diverse problems of these babies are not described here.
