How do cleft lip and cleft palate occur?
Each of us had a cleft lip and cleft palate during the early weeks of development in our mother's womb. Normally, the tissues that form the palate and the upper lip come together in the middle and join (fuse). You can see the lines of fusion in the "Cupid's bow" under your own nose, and the ridge and pale line in the middle of your palate (the roof of your mouth). If your baby has a cleft, this fusion failed to happen when he or she was developing. Return to top
Why does fusion of the palate fail to happen?
In most cases, we simply don't know why lip and palate development go wrong. About 1 in 600 babies has a cleft lip or cleft palate. Race and gender play a small role. Clefts are most common in Asians. They are less common in whites and least common in blacks. Girls are more often affected than boys.

Signs and Symptoms A cleft lip and palate is noted at birth by the presence of a gap or cleft in the upper lip either on one or both sides of the lip together with a cleft in the palate (the roof of the mouth). A cleft palate can either be open and obvious or be covered over by the lining or mucous membrane of the roof of the mouth, called a submucous cleft, which is not visible to the naked eye. An infant with cleft lip and palate usually needs to be bottle-fed (either breast milk or formula) using special nipples. Last reviewed in June 2003 by experts at UCSF Children's Hospital.

Definition:

  • Approximately 700 babies are born each year in the UK with a cleft of the lip and/or palate Clefts are usually repaired surgically before the child is one year old. Children with a cleft lip and palate are looked after by teams which includes speech and language therapists, surgeons and dentists who will discuss each stage of management with the parents Many children develop speech and language well but some may experience difficulty and need speech and language therapy. This will be organised in a local health clinic Delayed language development often occurs but children catch up quickly Sounds that are more difficult include P, B, T, D, K, G, S and SH Some children may need a further operation after the age of four to help with their speech Speech and language therapy may be helpful to adolescents and adults with cleft palate who are unhappy with their speech

The Development of Cleft Lip and Cleft Palate
Cleft lip and cleft palate are birth abnormalities of the lip and mouth. The lip and palate begin forming during just the first few weeks of gestation, often before a woman even knows she is pregnant. During this time, the sides of the mouth begin developing and eventually unite evenly in most infants. In rare cases, however, the sides of the mouth do not fuse together properly, creating a notch or cleft. Cleft palate – cleft palate occurs when the roof of the mouth does not unite properly. Openings may involve either side of the palate and may extend into the nasal cavity. They may extend from the front of the mouth (hard palate) to the throat (soft palate), and they often include the lip. Cleft lip – cleft lip occurs when the two sides of the lip do not fuse completely. Cleft lip may vary greatly, from a mild notch in the lip to a severe opening up through the nose. A cleft may extend only partially from the lip towards the nose (incomplete) or go into the nasal cavity (complete). Lip clefts may occur on one side (unilateral) or both sides of the mouth (bilateral).

The Cleft Lip and Palate Center brings together several different specialists to provide comprehensive inpatient and outpatient services at Children's Hospital of Wisconsin. We care for patients with cleft lip and palate disorders from birth into adulthood. Before Birth
Prenatal screening by ultrasound may detect cleft lips with or without cleft palates. In these cases, our team is available for consultation. Often, becoming informed of the issues children with cleft lip and palate disorders face helps prepare parents to care for the special needs their child will have after birth, including breathing and feeding. Most of the time cleft lips and/or palates are discovered at birth. Parents should remember that clefts do not hurt and their baby does not know he or she has a cleft. These common facial malformations appear in about 5,000 infants born in the United States each year. Approximately half of these infants have both types of cleft, with the remaining cases divided evenly between cleft lip and cleft palate. Cleft lip and palate can be quite disfiguring, in addition to creating a number of potentially serious difficulties with feeding, hearing and speech. Fortunately, however, cleft lip and palate can be surgically repaired with excellent results, although the process is sometimes lengthy and complicated. Sophisticated therapeutic techniques also can help an affected child learn to eat and speak normally. HOW DO CLEFT LIP AND PALATE DEVELOP?
About the fourth week of gestation, the tissues of the face begin to form. During this time, the primary palate, which eventually forms the upper lip and gums, and the secondary palate, which eventually forms the roof and back of the mouth, start to grow. During the fifth and sixth weeks of gestation, clefts are present in the primary palate on each side of the upper lip; these clefts run into the openings that will become the nostrils. The clefts normally fuse by the seventh week, creating the facial area between the nostrils and mouth. A cleft lip results when these clefts in the primary palate fail to fuse.

What is it?
If a baby is born with a cleft lip, his upper lip has a split in it, which is usually slightly off-centre. This may just be a little notch or it may extend up to his nose. Sometimes there are two splits. If he has a cleft palate, he has a gap in the roof of his mouth, either in part of it or all the way from the back of his palate to behind his gums. The conditions often, but not always, occur together.

The Cleft Palate/Craniofacial Clinic
Cleft lip, cleft palate and craniofacial surgery involves the comprehensive treatment of children with congenital defects (birth defects) affecting their mouth, face and sometimes skull. A cleft means a "split" or "separation" which can occur anywhere in the body, but in these children occurs at the lip, nose or along the roof of the mouth. Craniofacial birth defects can be varied but usually involve some aspect of an abnormally shaped skull. Clefts of the lip and palate are the most common congenital deformity of the face, affecting approximately one in every six hundred births. We do not know exactly why clefts occur but they do run in families to some extent. They are more common in Asian children and less common in African-American children.
As the child grows, many different parts of the face may be influenced by a cleft or abnormally shaped skull. Therefore, care of a cleft or craniofacial child involves a coordinated group of professionals who follow these children from birth until the completion of growth. That team consists of: plastic surgeons, pediatricians, geneticists, dentists, orthodontists, otolaryngologists, oral surgeons, speech therapists, audiologists, social workers and sometimes neurosurgeons/ophthalmologists. At ECU/PCMH, the Cleft Palate/Craniofacial Clinic organizes this care, coordinating and tracking the treatment and progress of these children through the years.

Your long-awaited baby is here. You planned this day for so long. And there is a problem. Nobody cheered when your baby was born. Everyone in the delivery room grew quiet. Somebody said something about a birth defect, and you didn't hear much after that. And then you saw your baby. And right in the middle of the face, where a beautiful little rosebud lip ought to be, there was a gaping hole. And now you have a hundred questions and a baby you are not sure you can take care of. The following are some of the questions you may be asking, along with some common-sense and straight-forward answers. If your question in not answered here, feel free to call Joanne Green, editor of WIDE SMILES Magazine and mother of three cleft-affected children. The most common questions a new parent has about cleft: Q. What caused my baby's cleft?
A. A cleft is most commonly caused by a combination of several factors. In most cases, genetics plays a role, although sometimes not the only role. There may be a number of environmental factors as well. The most important thing to recognize is that most likely, you did NOT cause the cleft. A cleft happens in one in every 700 births. It happens all over the world. It is the fourth most common birth defect. You will want to see a geneticist who will try to gain a clearer understanding of just what factors contributed you your baby's cleft, but very often the final answer is "We don'r really know." A cleft is formed by the 4th - 8th weeks of pregnancy. Many woman do not even know they are pregnant before the cleft has been formed. A cleft happens when the tissues of the face fail to fuse during gestation. It is a repairable birth defect.

Cleft Lip and Palate Program
One in every 600 to 700 children in Ontario is born with a cleft of the lip and/or palate. This notch or separation in the lip or palate can mar appearance and affect both tooth and bone structure. Because of its effect on physical appearance, it can be stressful for both the child and his or her family. Clefts may be caused by heredity, nutritional deficiencies, infection or various drugs. The problem occurs about the seventh to ninth week of pregnancy, when the unborn baby's face is developing. Through surgery, specialized dental treatment and speech therapy, the problem can be corrected eventually. However, these treatments can be very expensive. To help families of children and young adults who must cope with this expense, the Ministry of Health and Long-Term Care has established the Cleft Lip and Palate Program. Through this program, the ministry will provide 75 per cent of the cost of dental treatment by a certified specialist who is associated with correcting the cleft lip and/or palate. The Program also covers 75 per cent of treatment costs of severe acquired (non-genetic) dental dysfunction for patients up to 22 years of age.
