This page is designed to provide information on the research study conducted by Dr. Jacqueline T. Hecht and her associates, and to provide a way for families who are affected by cleft lip and palate to participate in the study. Cleft lip and palate is a serious problem affecting the face that is found in 1 out of 1,000 babies. Multiple surgeries are needed to close the clefts affecting the face and palate. The causes of cleft lip and palate are slowly being identified but a great deal of research needs to be done. We are trying to identify the the genetic (hereditary) causes and what factors in the environment (Smoking, medications, alcohol) play a role. Families in which there are multiple relatives with cleft lip and palate are of particular interest but we are also interested in families where isolated cleft lip and palate has occurred. By identifying the causes we may be able to modify the environment and develop new treatments. Please refer to our Frequently Asked Questions page for information on how to participate.

Surgery for cleft lips takes place in the first few months after birth
A baby which was abandoned by its parents in Portugal was identified partly through a distinctive cleft upper lip. BBC News Online looks at the condition and how it is treated. A 'cleft' or split forms parts of the face which developed separately during early pregnancy, do not then join together properly. A cleft can affect the lip, the palate and the gum. The severity of the condition can vary. A cleft lip creates an opening between the mouth and the nose, and can look as though there is a split in the lip. It can range from a slight 'notch' in the coloured part of the lip to a complete separation in one or both sides of the lip extending up and into the nose. Some babies who have a cleft lip may also have a cleft in the gum. Again, that can either be a small split, or complete division.

Much of the work was based on an analysis of samples from a set of twins in Brazil, where, highly unusually, one boy was born with Van der Woude syndrome, and the other without. In the long-term, it could lead to better understanding of why and how cleft lip and palate occurs, and could lead to treatments during pregnancy that might prevent the development of the condition. Important advance Researcher Professor Michael Dixon, of the University of Manchester, said: "This research is of significant global interest. Every 11 minutes a child is born somewhere in the world with a cleft lip and palate.

Common Congenital Disorders The cleft lip deformity is the most common congenital deformity seen in the United States. The overall occurrence is one in seven hundred live births. Some ethnic groups (eskimos, asians) have a higher number of babies with cleft lip. Sometimes the cleft lip is associated with a cleft in the roof of the mouth (cleft palate). The cleft lip maybe unilateral (one side) or bilateral (on either side of the nose). The cleft lip maybe complete (no bridging tissue across the cleft) of incomplete. The deformity seen in your baby with a cleft lip includes the lip, muscles around the lip, and the nose. All these deformities should be addressed at the initial surgery. The alveolus (the bone where the teeth come out) may also have a cleft, which may be address early if the deformity is not too severe, often the alveolus cleft is corrected at seven or eight years old. The repair of the lip is usually completed around three months of age. In preparation

Cleft Lip and Palate What is Cleft Lip and Palate?
Cleft means 'split' or 'separation'. During early pregnancy, separate areas of the face develop and then join together. If some parts do not join properly the result is a cleft, the type and severity of which can vary. Cleft lip and palate can occur separately or together. A cleft lip (sometimes referred to as a harelip) can range from a slight notch in the coloured part of the upper lip to complete separation in one (unilateral) or both (bilateral) sides of the lip, extending up and into the nose. A cleft palate occurs when the roof of the mouth has not joined completely. The back of the palate (towards the throat) is called the soft palate and the front (towards the mouth) is called the hard palate. If you feel the inside of your mouth with your tongue, you will be able to notice the difference between the soft and the hard palate. A cleft can affect the soft palate, or both the soft and the hard palate. Incidence
About 1 in 700 babies is born with a cleft in the UK (that's about 1,000 each year). The causes are not well understood, though it can sometimes run in families.

In the early weeks of development, long before a child is born, the right and left sides of the lip and the roof of the mouth normally grow together. Occasionally, however, in about one of every 800 babies, those sections don't quite meet. A child born with a separation in the upper lip is said to have a cleft lip. A similar birth defect in the roof of the mouth, or palate, is called a cleft palate. Since the lip and the palate develop separately, it is possible for a child to have a cleft lip, a cleft palate, or variations of both. If your child was born with either or both of these conditions, your doctor will probably recommend surgery to repair it. Medical professionals have made great advances in treating children with clefts and can do a lot to help your child lead a normal, healthy, happy life. This information will give you a basic understanding of the operation when it can help, how it's performed, and what results you can expect. It can't answer all of your questions, since a lot depends on you individual circumstances. Please be sure to ask your surgeon if there is anything you don't understand about the procedure.

A cleft is a birth defect that occurs when the tissues of the lip and/or palate of the fetus do not fuse very early in pregnancy . A cleft lip, sometimes referred to as a harelip, is an opening in the upper lip that can extend into the base of the nostril. A cleft palate is an opening in the roof of the mouth.
Description
Babies born with cleft lips will have an opening involving the upper lip. The length of the opening ranges from a small notch, to a cleft that extends into the base of the nostril. Cleft lips may involve one or both sides of the lip. Babies born with cleft palates have openings in the palate, which is the roof of the mouth. The size and position of the opening varies. The cleft may be only in the hard palate, the bony portion of the roof of the mouth, opening into the floor of the nose. It may be only in the soft palate, the soft portion of the roof of the mouth. The cleft palate may involve both the hard and soft palate and may occur on both sides of the center of the palate. Babies may have cleft lips with or without cleft palates. Cleft palates may also occur without cleft lips.

CLEFT LIP AND PALATE One in every 700 births results in a cleft lip and/or palate. Many of these congenital deformities occur in families that have no history of clefts. While it is known how cleft deformities occur, it is seldom known why.
Cleft defects develop very early in pregnancy. They result from an abnormality in the genes (units of heredity) or chromosomes (packets of genes). These elements contain the program that determines how the child will be formed. Why and how abnormalities in the genes and chromosomes develop is largely unknown. An individualized genetic assessment may be helpful in identifying the cause of the malformation.

    yes no A cleft of lip or palate occurs when there is a partial or total failure of the lip and palate to fuse during the early stages of pregnancy. Clefts may affect the lip (on one or both sides), the palate, or both. Surgery is carried out in the early weeks of life. Infants with clefts of the lip and/or palate should be referred to a recognised multi-disciplinary cleft team. Following the publication of the Clinical Standards Advisory Group report on cleft lip and/or palate in 1998, the number of cleft teams in the UK is being reduced to between 8-10 centres, which will provide comprehensive care from birth to maturity. Teams include surgeons, speech or language therapists, orthodontists, paediatricians and others. Early referral is encouraged. Inheritance patterns
    Slight genetic predisposition. The risk of recurrence increases with the severity of the cleft, unless the condition is part of another inheritable syndrome. Prenatal diagnosis
    Clefts of the lip are often found on the 18-20 week ultrasound during pregnancy. Referral should then be made to a cleft team for discussion.

Although it isn't usually know what causes a child to have a cleft deformity, about one third of cases are in children with a positive family history and other children in the family with a cleft deformity. It is also known to be more common for children with cleft deformities to have a mother who smokes. Cleft deformities are also associated with a number of syndromes, including Pierre Robin and Stickler syndromes and many others. Summary Each year approximately 227,500 or 7 percent of births in the United States are affected by birth defects of the head and face. The most common of these are clefts of the lip and palate which occur once in every 700 births. Clefts occur in infants of all races with a 2:1 male to female ratio. The incidence of clefts is highest in the Asian population and lowest in African Americans. Of all orofacial clefts, 21 percent present as cleft lip only (unilateral and bilateral), 46 percent present as cleft lip and palate, while the remaining 33 percent have cleft palate alone. What is a cleft? A cleft is a division or separation of parts of the lip or roof of the mouth that is formed during the early months of development of the unborn child. All of the parts of the lip or roof of the mouth are present; they simply failed to fuse in a normal way. Surgical intervention is necessary to align the parts and join them. Often the bones of upper jaw (maxilla) and/or the upper gum are affected. A cleft lip can be incomplete with a variable degree of notching of the lip, or complete, extending through the lip and into the nose.
