Home Ptosis of eyelid DESCRIPTION Ptosis is also called "drooping eyelid." It is caused by weakness of the muscle responsible for raising the eyelid, damage to the nerves which control those muscles, or laxity of the skin of the upper eyelids. Drooping eyelid can be caused by the normal aging process, a congenital abnormality (present before birth), or the result of an injury or disease. Ptosis is of two types, namely congenital ptosis and acquired ptosis. Congenital ptosis is more common than acquired ptosis. Ptosis may involve one or both the eyes and ranges from mild to severe.
  • Congenital Ptosis - Present at birth, the reason for congenital ptosis is the poor development of the eyelid lifting muscle called the levator muscle. A child may have only ptosis or may have other associated eye abnormalities like eye movement disorders, refractive disorders and neurological disorders. Acquired Ptosis - Acquired ptosis may occur at any time after birth, but more commonly in old age. The usual cause for this is weakening of the eyelid lifting muscle, the levator. Weakening of the levator muscle and the resultant droop of upper eyelid may occur due to factors like old age, injury, surgery, muscular disease, and neurological disease, including myasthenia gravis, chronic progressive external ophthalmoplegia, Horner's syndrome, third cranial nerve weakness, etc..

CPEO
(Chronic progressive external ophthalmoplegia) CPEO tilhører gruppen af mitokondrie-sygdomme CPEO er en kronisk fremadskridende lammelse af de ydre øjenmuskler, mens de indre øjenmuskler er intak-te. Hvis lammelsen forekommer dobbeltsidigt, kan det medføre et stift ansigtsudtryk med hængende øjenlåg. Dertil kan man også have ændret pigmentering af nethinden (retinitis pigmentosa), svage lemmer og generelt være plaget af forøget træthed. CPEO viser sig efter 20-års-alderen. Med tiden kan man udvikle døvhed, miste sine reflekser, få forhøjet mælkesyreindholdet i blodet, få koordinationsbesvær og noget der ligner slagtilfælde. CPEO beskrives ofte som en mildere grad af Kearns-Sayre Syndrom, hvor der er flere mitokondrier med muteret DNA. Og i mod-sætning til Kearns-Sayre syndrom, har CPEO ofte et godartet forløb.
Diagnosen stilles bl.a. ved hjælp af arbejdstests, hvor man bl.a. måler kroppens oxygenoptagelse, blodprøver, muskelbiopsier og genetiske analyser. Muskelbiopsier giver ofte entydige svar i den diagnostiske udredning af CPEO. Der findes ingen helbredende behandling, kun symptombehandling.
CPEO skyldes sædvanligvis en spontan mutation, og man vil derfor sjældent have oplevet syndromet tidligere i familiens historie.

June 2000 Written by Dr D.Hilton-Jones, Consultant Neurologist, Oxford for the Muscular Dystrophy Campaign It has been recognised for many years that some patients with muscle disease have particular problems with the muscles around the eyes, although other parts of the body can also be involved. Whilst research is continuing, it appears that most of these patients have either oculopharyngeal muscular dystrophy (OPMD) or mitochondrial chronic progressive external ophthalmoplegia (CPEO). The main features of these disorders are discussed below. Some of the symptoms and signs are common to both disorders. Medical terms frequently used include: ptosis, external ophthalmoplegia, diplopia and dysphagia. Ptosis - this describes drooping of the eyelids due to weakness of the muscle that normally lifts up the eyelid. External ophthalmoplegia - this means weakness and restriction of muscle movement around the eye (external to the eye). It shows as slowness and incomplete range of movement of the eyes, and includes the eyelid muscle weakness that causes ptosis. These problems typically progress very slowly, hence the term 'chronic progressive external ophthalmoplegia'.

BOUZA E, GÓMEZ DE LIAÑO P, MERINO P, GONZÁLEZ M, CONEJERO J RESUMEN Objetivo/método: Discutir el resultado quirúrgico obtenido en un caso de oftalmoplejía externa crónica progresiva (OECP) asociada a estrabismo con diplopía. Se presenta a una paciente con miopatía mitocondrial y OECP que fue tratada mediante retroinserciones ajustables con anestesia local. Resultados/conclusiones: Transcurridos cinco meses desde la intervención se mantenía la mejoría en las ducciones, la ausencia de diplopía y prácticamente de tortícolis. Somos partidarios de intervenir estos casos de OECP asociada a estrabismo. El uso de suturas ajustables permitió un resultado satisfactorio en el postoperatorio inmediato. Palabras clave: Oftalmoplejía externa crónica progresiva-cirugía, estrabismo, diplopía. SUMMARY Purpose/methods: The purpose is to discuss the surgical result in a case with chronic progressive external ophthalmoplegia (CPEO) associated with strabismus and diplopia. The authors present a patient with mitochondrial myopathy and CPEO who was treated using adjustable recessions with local anaesthesia.
