Chronic obstructive pulmonary disease (COPD)
Sebastian Johnston's group works on asthma and chronic obstructive pulmonary disease (COPD) and the role of respiratory viral infections in these diseases, with a particular interest in rhinovirus infections. The group carries out epidemiological investigations of the importance of environmental factors in development of these diseases and the role of viral and atypical bacterial infections in these diseases and in exacerbations. Cellular and molecular studies investigate the basic pathogenic mechanisms of the diseases and the role of viral and atypical bacteria in exacerbations with a major focus being the identification of targets for the development of new treatments. Clinical studies include experimental infection studies with rhinovirus and RSV and clinical trials of new therapeutic agents for asthma COPD and viral infections.

Hi, I have COPD which is Chronic Obstructive Pulmonary Disease. This disease is not very understood by the public. I have a hard time just trying to get air into my lungs and some days it is a real challenge just to get dressed. It seems that people with this disease are not getting the information they need from the medical people to help them cope with this dreadful illness. I joined a support group, online called, EFFORTS, and that was the beginning of my real learning about COPD!!! This group is very supportive and is such a great resource for information. There is even a Respiratory Therapist and a doctor that belong to this group and give us so much correct information. I really believe that the public needs to be very much aware of this disease and what causes it. Smoking is the number one cause, but there is also an inherited kind. For a wonderful source of knowledge on COPD please check out the website!! I know that you will be very impressed with this support group. EFFORTS http://www.emphysema.net

Review Case What type of acid-base disturbance occurred in the Emergency Room? What is the PA02? What is the A-a gradient and what does it tell you? Give at least two mechanisms of hypoxemia in this situation. What does the elevated HCO3 tell you? Spirometry revealed the following:
FEV1 at 0100=0.5 L (pred 2.9 L); at 0800=0.7 L.
FVC at 0100=Unobt. (pred 3.9 L); at 0800=1.7 L.
Hospital Course
    Arterial blood gases are drawn in the Emergency Room and oxygen (2 LPM) by nasal prongs is started. Gases are repeated at 0300. You commence bronchodilators and begin trimethoprim/sulfamethoxazole. A gram stain of sputum reveals many PMN's and some alveolar macrophages. At 0800, the patient feels better, can raise some sputum and is able to sleep. Why begin trimethoprim/sulfamethoxazole? What are the most common pathogens in acute exacerbations of chronic bronchitis? Why did the PaC02 increase at 0300? Should any therapy be altered to diminish the hypercapnia? Should the patient be intubated? Note that the hypoxemia originally presented is easily corrected. What does this suggest as the mechanism of the hypoxemia?
