Chronic obstructive pulmonary disease, or COPD, is characterized by abnormalities in the lungs that make it difficult to exhale normally. Generally, two distinct diseases are involved: emphysema and chronic bronchitis . According to the World Health Organization (WHO), 75% of deaths from COPD that occur in developed countries are directly related to smoking tobacco. Emphysema and chronic bronchitis cause excessive inflammatory processes that eventually lead to abnormalities in lung structure that permanently obstruct airflow (hence the term "chronic obstructive"). Asthma usually can be controlled, but some asthmatics have permanently narrowed airways and suffer COPD. Incidence and Prevalence The American Lung Association and the World Health Organization track respiratory disease and mortality rates related to tobacco use.

Differences in prognosis among the major types of COPD have become clear, and identification of airway inflammation has dramatically altered medical therapy. Dr Martinez, who coordinated this symposium, here explains the importance of judiciously applying findings from history taking, physical examination, laboratory studies (particularly pulmonary function testing), and radiographic studies to all aspects of disease management. The remaining four symposium articles address specific avenues for treatment of emphysema and chronic bronchitis. C hronic obstructive pulmonary disease (COPD) is characterized by airflow obstruction from various causes that markedly impairs quality of life. In 1991, 85,544 deaths in the United States were attributed to COPD, ranking it as the fourth leading cause of death (1).

    Click here for drugs used to treat rhinitis and sinusitis Go Directly to Drug Classification COMMENT ON CLASSIFICATION. There are many ways to classify drugs for asthma and COPD (chronic obstructive pulmonary disease). The classification offered here is based on their mechanism of action in improving the basic problem (which is airway obstruction) and by their route of administration (oral or pill form, by inhalation, or by injection). Our lungs contain many branching airways that deliver fresh air to the blood. The phrase "drugs for asthma and COPD" means drugs that help open up the airways when they become narrowed due to disease. (Note that these are generally different from medications used to treat upper airway problems (rhinitis and sinusitis); see the link above for a table of those drugs.) Other drugs may be used in people with asthma and COPD, such as antibiotics for infection, but they are not used to directly "open up" the airways. When it comes to drugs for "opening up" the airways, all available medications can be classified by one of two basic mechanisms of action: bronchodilation and anti-inflammatory.

SUMMARY OF KEY POINTS
  • Asthma is an inflammatory airways disease and all but the mildest attacks require treatment with corticosteroids; this is also true for asthmatic exacerbations of chronic obstructive pulmonary disease (COPD).
  • For exacerbations of asthma or COPD, preferentially start an inhaled bronchodilator drug (beta-adrenergic or anti-cholinergic) along with the oral or intravenous corticosteroids. Theophylline is no longer a first-line drug in the treatment of asthma or exacerbations of COPD; it is a second or third-line drug for this indication.
  • The role of anti-leukotriene drugs in chronic asthma management is not yet clearly defined. These drugs are not to be used for therapy of acute asthma exacerbations.
  • When air flow improvement is expected with treatment, monitor some measurement of air flow: peak flow or spirometry. Arterial blood gases are not necessary to manage most asthmatics; they are most useful when peak flow is repeatedly less than 35% of predicted.

  • Definition . A generalized increased resistance to airflow during expiration that includes chronic bronchitis, emphysema, chronic asthma, and bronchiolitis.     Chronic Obstructive Pulmonary Disease
    Chronic Obstructive Pulmonary Disease (COPD) is a disease in which the airways and air sacs inside the lungs are partially obstructed or destroyed. The result is that it gets hard to breathe. There are varying degrees of this illness, and different names for them, but it all comes back to damaged airways and air sacs. This disease occurs when a person breathes in lung irritants of some kind: smoke, chemicals, pollution, dust. It makes sense then, that the most common cause of COPD is smoking.

    Chronic Obstructive Pulmonary Disease (COPD)
    Chronic obstructive pulmonary disease (COPD) refers to chronic lung disorders that result in blocked air flow in the lungs. The two main COPD disorders are emphysema and chronic bronchitis , the most common causes of respiratory failure. Both result from damage to the lungs over many years and predominantly affect former and current smokers. Emphysema occurs when the walls between the lung's air sacs become weakened and collapse. Chronic bronchitis occurs when the airways in the lungs become swollen and partially clogged with mucus. Chronic bronchitis may also involve muscle spasms in the airways. Many people with COPD have both emphysema and chronic bronchitis.
