    to find a practitioner using the Footmaxx system near you. CHONDROMALACIA PATELLA - (of Greek origin meaning "softening of the cartilage") What is it? Chondromalacia is a condition wherein the cartilage on the back of the patella (knee cap) is irritated and painful because it rubs against the medial femoral condyle. What are the common symptoms/complaints? Patients complain of pain felt under the knee cap on the medial side of the knee particularly while going up or down stairs and hills. Also pain and stiffness after prolonged sitting. How is it caused? During a normal gait cycle, the femur and the tibia rotate in unison (i.e. internally during pronation and externally during supination). However, Chondromalacia occurs when a person over-pronates because the tibia is locked into the Talus and therefore continues to rotate internally past the end of the contact phase (where supination starts at the beginning of midstance in a normal gait) while the femur receives its orders from the brain and begins to rotate externally at midstance. The resulting counter rotation of the femur and the tibia causes the patella to rub against the medial femoral condyle instead of moving smoothly up and down in its normal track, which causes the pain felt by the patient and the damage to the cartilage.     Chondromalacia patellae, a condition which may occur in runners, is characterized by a softening of the articular cartilage of the patella. It may cause patellalgia, i.e. pain associated with the patella. This pain and aching around and deep to the patella may be caused by imbalance in the quadriceps muscles which may compress the patella and/or pull it sideways. Chondromalacia patellae may also be caused by extreme flexing of the knee while knelling or squatting or by a blow to the knee.

    Causes of Chondromalacia Patellae
    Chondromalacia Patellae occurs in adolescents and young adults, more frequently in women. The cause is thought to be related to overuse, trauma and/or abnormal forces on the knee. Many affected adolescents have a mildly abnormal alignment of the Patellae (knee cap) and femur. Affected people of all ages have knee pain and a grating or grinding sensation when they extend their knee.
    Symptoms of Chondromalacia Patellae
    Knee tenderness Knee pain in the front of the knee that worsens after sitting for prolonged time Knee pain that worsens with using stairs or getting out of a chair A grating sensation in the knee
    Diagnosis of Chondromalacia Patellae
    A physical examination of the knee is not specific but may suggest the diagnosis. The knee may be tender and mildly swollen. The kneecap may not be perfectly lined up with the femur (thigh bone) underneath.

  • Back to Encyclopedia Index Overview First used by Aleman in 1917, chondromalacia patella was the name given to a degeneration of the patellar articular cartilage (Kipnis and Scuderi, 1995). True chondromalacia patella involves the degeneration of the articular facets of the patella, with resulting ragged fronded edges, (Welsh and Hutton, 1990), and is a very common injury amongst both men and women of our sporting population (Williams, 1990). Along with osteoarthritis, chondromalacia patella is a type of cartilage damage resulting in anterior knee pain. It is also named as a cause of the symptoms of patellofemoral pain syndrome (Reid, 1992). When compared to the number of patients who are seen with patellofemoral pain, and those with actual changes to the articular cartilage of the patella, chondromalacia patella is a rare condition though (Nofthall, 1990). The knee is involved in around 10 % of all sporting injuries, (Turner and Avil, 1995). While treatment of knee complaints is not in the realm of podiatry practice, knee pathology can exert an influence on the overall lower limb’s functioning (Turner and Avil, 1995). This may result in biomechanical compensatory mechanisms in the foot, where podiatry can have a positive effect on such a scenario.

    Grading of Chondromalacia
    The ends of bones which form joints are covered by a very smooth material called articular or chondral cartilage. After injury or with disease, the condition of this surface can change. This can progress over time from becoming a bit soft to being completely absent. The loss of articular or chondral cartilage of a joint for any reason, (injury, illness, or natural wear- and- tear with aging), is called chondromalacia. These changes compare to the difference seen between a brand-new Teflon-coated frying pan, one that has been slightly scratched by a steel scrubber, and one where the Teflon surface has flaked off completely. As with a frying pan's Teflon surface, the greater the destruction of the articular cartilage, the poorer the function of the joint. Important considerations when evaluating chondromalacic changes in a joint are the state of the articular surface, the extent of the cartilage involvement and the location of the lesion. These all impact the degree of symptoms.
