Definition: Chondromalacia patellae is the softening and degeneration of the cartilage beneath the kneecap (patella).
Alternative Names: Patellofemoral syndrome; Anterior knee pain
Causes, incidence, and risk factors: Chondromalacia patella that occurs in adolescents and young adults is thought to be related to overuse, trauma, and/or abnormal forces on the knee. It is more common in females. Many affected adolescents have a mildly abnormal alignment of the kneecap. Chondromalacia patella may also reflect arthritis of the kneecap, which is usually seen in older individuals. People who have previously had a dislocation , a fracture , or other trauma to the kneecap may be more likely to get chondromalacia.

Conservative Management of Chondromalacia Patellae
Chondromalacia pattellae may be considered as a softening of the articular cartilage of the patella. It may be of diverse etiology. More recently, three distinct varieties of the syndrome have been proposed: > a short-term variety described as a primary idiopathic form affecting adolescents and young adults with chronic persistent pain and resolving with a minimum to no residual; > a secondary variety described as a secondary adolescent form due to direct patellar trauma with repeated patellar traumatic episodes which contribute to osteoarthritic development as age advances; > an adult form presenting primarily in the second decade with osteoarthritic symptoms developing asymptomatically and progressing as age advances and becoming symptomatic with middle age or beyond. Contact between the patella and the femur is close at the commencement of flexion. This contact slowly passes outward transferring to the medial and lateral facets after flexion is beyond 90 degrees. This area of contact, or friction, between the medial facet and medial femoral condyle, represents the site of earliest osteoarthritic changes where the development of softening and fissuring from the outset is properly considered chondromalacia. It is also noteworthy that the area of patellar contact on the medial femoral angle corresponds with the site of predisposition for osteochondritis dissecans. The location of the patellar cartilage relative to movement is not suited for the high compressive and frictional forces to which it is subjected. Consequently, the cartilage breaks down and with spontaneous degenerative changes which follow this process, the entire patellofemoral region undergoes osteoarthritic change.

Chondromalacia patellae is a painful disorder of the knee in which the cartilage directly behind the kneecap (also known as the patella) becomes soft and damaged. Cartilage is a type of tissue that helps form many important body structures. The type of cartilage affected in chondromalacia patellae is called articular cartilage. The word "articular" means "referring to a joint." A joint is a place where two bones contact each other. The knee is an example of a joint. Thus, since the cartilage affected in chondromalacia patellae is located directly behind a joint, it is called articular cartilage. WHO USUALLY GETS CHONDROMALACIA PATELLAE? Chondromalacia patellae usually happens to young adults after a knee injury. It is particularly common in teenagers and is more common in females. Chondromalacia patellae rarely occurs in older adults (ages 40 and older). When it does, it is known as retropatellar arthritis. People with a history of trauma to the knee such as a break or dislocation, are more likely to get this condition. A dislocation in when any body part (usually a joint) is out of its normal position. Someone can have a partial dislocation (partly out of the normal position) or a complete dislocation (completely out of the normal position).     Chondromalacia patella is a softening or wearing away of the articular cartilage on the undersurface of the kneecap (patella). Articular cartilage is a smooth substance that covers the femur (thighbone), the tibia (shinbone), and the patella. These structures form the knee joint. The articular cartilage cushions these three bones and allows them to move freely and easily where they meet in the knee.     Chondromalacia patella occurs when:
    • The kneecap doesn’t move properly in the groove in the knee where it meets the femur, or When there is a muscle imbalance of the quadriceps (thigh muscles), which control the patella
    This can occur due to:
    • Congenital conditions – a person is born with a misalignment of the kneecap. Weak quadriceps – these muscles assist in proper movement of the knee joint. Weak quadriceps may allow the patella to move out of alignment.

    Chondromalacia Chondromalacia is a diagnosis referring to damage, and subsequent pain, to the cartilage under your kneecap. It can be caused by several factors, and produces discomfort with such activities as walking up and down stairs, kneeling, squatting, or getting up from a seated position. It can occasionally produce swelling in the knee and a sensation of giving way or "catching" ("locking"). It is a very common problem and is directly related to the amount of pressure between the kneecap and the femur bone beneath it. This pressure is normally 2-3 times your body weight when simply descending stairs; one can imagine the magnitude of the pressure when running! All the following suggestions and treatments are directed at decreasing the pressure between the kneecap and the underlying femur. The usual precipitating cause of pain in the patient with chondromalacia is either trauma (an injury such as a fall on the knee or a car accident) or a developmental abnormality (Malalignment) of the knee, which may predispose a patient to knee cap pain. Some patients suffer from a kneecap that repeatedly dislocates (usually towards the outside of the knee), which not only causes pain, but buckling of the leg and damage to the undersurface of the kneecap. There is a "normal" relationship between the thigh muscle, the patella, and the point of attachment of the patellar tendon. This is described by the Quadriceps Angle (Q-angle). In those people in which the Q-angle is larger than normal, there is a greater tendency for the kneecap to track abnormally in the groove on the front of the femur with knee bending, leading to increased pressure in certain parts of the kneecap-femur joint.

The key element in this injury is due to overpronation resulting in poor tracking of the patella causing a repeated stress of the bone moving abnormally from side to side over the groove of the thigh bone (femur), gradually softening the cartilage under the kneecap. The result is the surface of the joint can become rough. Pain may be just a prelude to further destruction of the surface (traumatic arthritis). This injury can be confused with "Patella tendonitis" which is usually pain beneath the patella bone. In either case it is the "tracking" of either the bone or tendon which needs to be addressed.
    Other causes of this injury can be:

    Symptoms - pain or tenderness close to or under the patellar or knee cap at the front or side of the knee. Pain is gradual, increases over several weeks, usually in one leg. The Patella’s cartilageunder the kneecapwears away; it becomes sandpaper like, often makes a grinding sound as it no longer rides smoothly over the knee. Causes - Running on a camberthe slope at the side of the road, or if a large part of mileage is across the slope of a hill; long runs ; not warming up properly; tight, weak or fatigued quadriceps; tight, overly strong hamstrings; kneeling; going up and down stairs or hills; sitting still for long periods; cycling; overpronation. Reduce the cycling element of your training, or go higher cadence with lower resistance. Do a complete warmup including quadriceps strengthening exercises; don’t allow the quads to get more than 50 percent stronger than the hamstrings. Stretch the hamstrings, quads and calves. Avoid deep knee bends.

    Introduction
    Knee pain affects as many as 1 in 3 teenagers and young adults at some time or other. A common cause of this pain is a complaint known as chondromalacia patellae . This is a softening of the cartilage that lines the back of your kneecap. This booklet deals with the type of knee pain caused by this condition. Your kneecap (patella) lies at the front of the lower end of your thigh bone (femur). Its upper part is attached by a tendon to your thigh muscles (quadriceps) and its lower part by a tendon to your shin bone (tibia). Its function is to enable the quadriceps muscle to contract, straightening your leg, and to stabilise your knee joint when you walk.
    What are the symptoms?
    The main symptoms are pain and crepitus (grinding or clicking). The amount these interfere with everyday activities varies both from person to person and also from time to time for any particular individual.
    Pain
    The pain is felt in the front of your knee, behind the kneecap. It can sometimes be quite severe and everyday activities like going up and down stairs can make it worse. At other times it may be felt as a dull ache after you have been sitting for any length of time. The pain often makes it difficult to kneel or squat.

Chondromalacia (also called chondromalacia patella) is not caused by surgery or anything related to a surgical procedure. Instead, it is caused by an imbalance of the muscles around the knee or a prolonged twisting of the lower leg. This imbalance or twisting causes a wearing out of the cartilage underneath the kneecap (patella), which results in discomfort. Chondromalacia patella can oftentimes be treated by decreasing or eliminating the exercise that is causing the problem (such as running down hills), as well as quadriceps strengthening exercises.
