Q: Chondromalacia I have been suffering with pain in front of my knee that my doctor has diagnosed as chondromalacia. I'm in my early twenties and thought that only teenagers got this? Pete Dr Rob Hicks responds Teenagers more commonly develop this condition but young adults of your age can suffer with it too. Often doctors will call it by its other names of chondromalacia patella and patellofemoral pain syndrome In chondromalacia the cartilage that lines the back of the kneecap becomes damaged. For teenagers it's often the extra pressure the knees have to cope with during the growth spurt that sets the problem in motion. For adults and teenagers strenuous exercise can be the trigger for the damage to start, as can repeated injuries to the knee. Why the condition occurs at all though is not really understood. The knee , since it is usually only one knee that's affected, feels stiff after sitting down and it may click when it's moved. Any action where the knee is bent and straightened, running or going up and down stairs for example, will bring the pain on.

This disorder occurs in adolescents and young adults. The typical site of involvement is the medial facet of the patella, particularly about the ridge that separates the medial and odd facets. Swelling and oedema of the cartilaginous surface are present. The changes are difficult to observe on images, and histologic evaluation is needed for diagnosis. MR imaging is perhaps the best noninvasive diagnostic technique for assessment of cartilage loss, but arthrography, particularly in association with CT scanning, can be valuable ( Fig.1 ). On arthrograms, absorption or inbibition of contrast material by the patellar cartilage can be observed, as may nodular elevation, fissuring and loss of surface cartilage ( Fig.2

The pain is usually most marked during athletic activity, when rising from a chair or walking down stairs. There is fraying and fissuring of the retropatellar articular cartilage which predominantly involves the medial facet. The changes are not visualized on conventional radiographs but may be demonstrable on double contrast CT arthrography as an area of surface irregularity of the articular cartilage with associated contrast adsorption ( Fig.1 ). Likewise chondromalacia patella can be demonstrated on cartilage specific MRI sequences as heterogeneous increased signal within the retropatellar articular cartilage. Occasionally, on radiographs, one can see irregularity of the posterior surface of the patella with bony fragmentation. This is a variant of chondromalacia patellae, which, in addition to the cartilage destruction, also has an osteochondritis ( Fig.2

It is caused by direct trauma or chronic injury to the knee joint. Although it is often referred to and treated as a discernible ailment, chondromalacia patellae is best thought of as a symptom. The patella contains the thickest layer of cartilage in the body. It has five facets or ridges: superior; inferior; lateral; medial; and odd. The "odd" facet is most frequently the first part of the patella to be affected in chondromalacia patellae Chondromalacia patellae is the softening and subsequent roughening of the patella's hyaline cartilage. This malady presents itself as grinding beneath the patella and may cause related swelling and pain. A definitive diagnosis is made through visual inspection during arthroscopy. Chondromalacia patellae is most often, if not always, the result of biomechanical changes affecting the lower extremity. As such, it may be treated symptomatically by acupuncture.

Patellofemoral Syndrome Or Chondromalacia
Patellofemoral syndrome causes pain in the knee. It is a common problem of young adults, especially female adolescents. It is sometimes called chondromalacia. The long bone of the thigh is called the femur. The femur has a groove in which the kneecap (the patella bone) moves. The kneecap has to be able to move freely up and down when we bend or straighten our knee. If there has been an injury or overuse of the knee, the structures that allow it to move freely can wear down. Over time, the undersurface of the kneecap becomes worn out and inflamed. Signs and symptoms that should be evaluated by a healthcare provider include:
  • knee pain that gets worse when kneeling or walking up and down stairs and knees ache if sitting in one position too long, like at a movie, or on a plane.

My doctor said that the cartilage is so damaged that I will need another surgery in a couple of years to smooth it out again. Yesterday, I rode the stationary bike for a short period of time and afterward began having pain just below and on the inside of the kneecap. It is a burning pain that is unlike the pain that I have previously experienced. Is this related to the Chondromalacia? What is the best treatment?

    Degenerative Chondromalacia The term chondromalacia comes from two Greek words that essentially mean cartilage softening.1 Chondromalacia describes the changes that occur in cartilage covering the ends of bones as it breaks down and degenerates. This condition is not the same as osteoarthritis, and one does not necessarily lead to the other. Osteoarthritis results from long-term wear and tear that causes the lining of the joints to become rough and painful. Chondromalacia results from an injury to otherwise healthy cartilage or a response to abnormal pressure on the cartilage. Cartilage gets its strength from an interlocking mesh of collagen fibers and proteins. When viewed through a microscope, the cartilage in a joint with chondromalacia has fewer collagen fibers and has a lower protein content, which means the interlocking mesh has begun to unwind. The lubricating fluid inside the joint also shows elevated levels of some enzymes that can dissolve proteins. The causes of chondromalacia are different for different joints. A baseball pitcher can develop it in his elbow due to overuse, and someone who constantly works with his or her arms overhead, such as an electrician can develop it in the shoulder. However, the most commonly affected joint by far is the knee. Several factors contribute to the development of knee pain due to chondromalacia. Occupations involving repeated kneeling, squat-ting, and climbing stairs or ladders can lead to knee pain. Malalignment of the leg bones causes some people to develop this problem. For example, people who are "knock kneed," or have what is called valgus alignment, have increased stress on the area between the patella (kneecap) and femur (thigh bone). This abnormal pressure causes a breakdown of the cartilage and knee pain. People who have loose joints and are hyperflexible also have abnormal stress on their patella and can develop these changes. Any injury to the knee that bruises the cartilage can lead to softening. Even a mild injury can easily interrupt the blood supply the cartilage receives from the underlying bone and can lead to death of the cartilage cells. Aging may also play a part.
