I'm not a doctor. (I don't even play one on T.V.) I'm a 37-year-old woman suffering from chondromalacia patella, an obstinate desire to keep playing competitive volleyball, and possibly even a mid-life crisis. Consult your doctor or sports medicine specialist if you are suffering from knee pain. ( Update 18-Feb-2002: I find it terribly ironic that I've been sidelined from volleyball since late 1999 for torn wrist ligaments that could only be partially repaired by surgery. Creaking around on bad knees is one thing; doing without the use of one hand is another! Until technology gives me a fully-repaired wrist, I'll be watching my daughter, the Wild Child, a chip off the amazon block, slam volleyballs down short chicks' throats, instead of playing myself. She even stuffs those silly hitters who try to dink over herjust like Mama! :-D That's my baby! PS: I'm now almost 40! Egads!)

Chondromalacia Patella, Patellofemoral Syndrome
What is chondromalacia Patella?
The term chondromalacia patella refers specifically to a pathological condition resulting in structural changes in the cartilage surface of the kneecap. These changes may be as mild as softening and discoloration of the normally firm, white glistening surface. if the process continues, fissuring and fraying may occur with the more advanced stages resulting in full thickness cartilage loss with exposure of the underlying bone. The process may be localized to one small area or it may involve the entire surface of the patella. Because of these variations in the cartilage changes, the symptoms experienced by patients with chondromalacia may vary. The patella aids the quadriceps muscle in extending the knee by improving it's mechanical advantage. The articular surface of the patella is the thickest in the body (four to six millimeters). Walking on level ground exerts a patellofemoral force equivalent to one-half the body weight, climbing stairs between 3-8 times the body weight, and rising from a full squat can generate forces eight times the body weight.

Knee Problem: Chondromalacia Patella
This disease is also known as softening of the cartilage of your knee cap or "anterior knee pain". This is a big medical word for a condition where the shiny cartilage surface of your patella (knee cap) is softened due to many factors including abnormal pressure across the joint surface or hormonal changes in your body. This pain is usually located over the front of your knee and is described as a deep aching pain. It is sometimes associated with swelling and is usually worse when your knee is bent for long periods of time such sitting in a car or bus. The pain is also worse with such activities as squatting, kneeling or stair climbing (either up or down stairs). The pain is sometimes improved with rest and the use of pain medication. It is sometimes associated with mild or moderate swelling of the knee and some people report a grinding feeling in their kneecap. It is more common in younger females especially after a growth spurt where the knee must carry more weight.

Chondromalacia Patella: Treatment
Most orthopedic surgeons agree that chondromalacia should first be treated conservatively. This means without surgery. However, each person is different and your doctor may feel you need surgical treatment immediately. If you are told that you will require conservative care you can remember the plan easily by remembering the words:
Control, Avoid, and Rehabilitate. Control: Get your pain under control.
  • Cryotherapy (ice, cold therapy) is an excellent way to control your pain. Do not apply ice directly to your skin for more than 5 minutes as it can cause burns. A device specially designed to cool your joint (see our

The key message with chondromalacia is that one must understand that this is not a disease. It is a word which means 'joint cartilage softening' and this diagnosis can only be made during surgery when the cartilage is probed or maybe on MRI where the cartilage may be seen to be abnormal. Too often patients are told they have 'chondromalacia' by a PT or medic just on the basis of a combination of symptoms - anterior knee pain (pain in the front of the knee), crunchy noises, pain on going down stairs. This fills them with unnecessary fear and they may start a totally inappropriate rehabilitation programme or, worse still, stop looking for the cause of their problem. A number of conditions of the knee can cause joint surface softening such as abnormal position or tracking of the patella or a plica. Arthritis Surgeons will grade the joint surface degeneration for the record. Although there is more than one classification, the principles are:

  • What Is It? The joints in your body are cushioned with a type of connective tissue called articular cartilage. This tough, rubbery tissue covers the ends of bones inside a joint. As the joint moves, the cartilage helps to cushion the bones. It also allows the ends of the bones to glide smoothly against one another.

Runner’s knee (Chondromalacia patellae) www.Studenthealth.co.uk Symptoms
The main symptoms of runner's knee (chondromalacia) are aching around the knee cap. This ache may be worse on climbing stairs or cycling. Runners may experience knee pain after running a certain distance when their knee seizes up and forces them to stop. Cause
Chondromalacia is caused by disproportionate development of some muscles around the knee. These muscles then pull the knee cap in the wrong direction. The back of the knee cap then starts rubbing against other parts of the knee joint and ache develops. Some patients have a predisposition to knee pain because the knee cap is small or anatomically misplaced. Treatment
This condition often gets better on its own in time, but can persist for years. You can speed up the recovery process if you follow this advice. Modify your exercise to e.g. cycling in low gears, gentle jogging, swimming etc. Do the following; Knee exercises:
  • Sitting on a bed, keep your leg flat and bend your toes towards your head as much as possible, keep this position for ten seconds and release. Repeat this ten times.

    Definition
Chondromalacia patellae refers to the progressive erosion of the articular cartilage of the knee joint, that is the cartilage underlying the kneecap (patella) that articulates with the knee joint. Description Chondromalacia patellae (CMP), also known as patello-femoral pain syndrome or patello-femoral stress syndrome, is a syndrome that causes pain/discomfort at the front of the knee. It is associated with irritation or wear on the underside of the kneecap, or patella. In a normal knee, the articular cartilage is smooth and elastic and glides smoothly over the surface of the thighbone, or femur, when the knee is bent. Erosion of the cartilage roughens the surface and prevents this smooth action. CMP is most common in adolescent females, although older people may also develop it. An average of two out of 10,000 people develop this condition, many of them runners or other athletes. Causes and symptoms CMP is the result of the normal aging process, overuse, injury, or uneven pressures exerted on the knee joint. In teens, CMP may be caused by uneven growth or uneven strength in the thigh muscles. Growth spurts, common in teens, may result in a mildly abnormal alignment of the patella, which increases the angle formed by the thigh and the patellar tendon (Q-angle). This condition adds to the damage. Symptoms include pain, normally around the kneecap, and a grinding sensation felt when extending the leg. The pain may radiate to the back of the knee, or it may be intermittent and brought on by squatting, kneeling, going up or down stairs, especially down, or by repeated bending of the joint.

You may strengthen and stretch certain muscles to influence the tracking of your knee cap. The inside muscle of the quadriceps is the vastus medialis. This is activated when your knee is tightened in the straight position. That is why straight leg raising with Leg weights are very beneficial for Chondromalacia. Strengthening through a bending position activates the larger, lateral muscle of the quadriceps, when this muscle is strong it further pulls the knee cap more laterally, thus further aggravating the condition. There is more compression to the knee cap the further you bend, therefore deep squatting is not advisable. If the lateral hip muscles are tight, they will have an affect on the knee cap. So stretch the hip and hamstrings. Poor foot mechanics such as excessive foot pronation will adversely affect the position of the knee cap. Goode Wraps provide the muscle relaxation to properly stretch the muscles and relieve runners knee.
