By Ginny Hermann When we think of children and depression, the picture that most likely pops into mind is that of a lonely teenager, diary in hand, languishing in the isolation of her bedroom, or we envision a misfit adolescent, sulking in a corner with only his Game Boy and comic books for company. But depression isn't just a teenage or adolescent problem. The "blues" can strike as early as kindergarten and can progress into an extremely serious illness if left untreated. According to Ellie Epstein, an employee assistance program counselor with Methodist Hospital in Omaha, Neb., childhood depression is more common than you would think. "There are many things that can contribute to a young child's bout with depression," says Epstein. "A stressful life event is usually found to be the trigger in children ages 6 through 9. Since depression has very little to do with biology at this age, the causes are usually found to be environmental, something that has occurred either inside or outside the family unit." The Effects of Tragedy
Unpleasant family events – such as death, divorce, family conflict, illness or physical disability – can turn an otherwise happy-go-lucky youngster into a sad, withdrawn child.

Childhood Depression: Whose Job Is It?
It is estimated that 1.9% of primary school children and 4.7% of adolescents are affected by depression (Weller and Weller, 1991); the consequences of depression in this age group can include social dysfunction, academic underachievement and suicidal behavior. Within the last three decades, the recognition of a behavioral component of the primary care of children, coupled with the introduction of managed care and new psychopharmaceuticals, has changed the role of the primary care clinician (Rushton et al., 2000). Also, while reports of the efficacy and safety of antidepressants in children and adolescents are mixed (Emslie et al., 1999; Hazell et al., 1995), their use has dramatically increased in the last decade (Martin et al., 2000; Zito et al., 2000). It is still unclear, however, how childhood depression and psychiatric disorders fall within the scope of primary care. To assess the current management of childhood depression in primary care, researchers Jerry L. Rushton, M.D., and colleagues (2000) at the University of Michigan conducted a survey of general pediatricians and family physicians in North Carolina. Both groups were surveyed using identical four-page questionnaires designed to assess the patterns of use of four management strategies for childhood depression: medication, referral, counseling and watchful waiting. Adolescent medicine specialists, behavioral pediatricians and medicine-pediatrics physicians were excluded from the study. The mailed survey also gathered information on physician demographics, practice characteristics, general management approaches to depression in children, volume of pediatric patients with depressive symptoms, prescription of selective serotonin reuptake inhibitors for depression, and opinions on the management of depression.

Before that time, it was commonly believed that children could not suffer from depression. What, after all, could a child in a healthy family and community environment be depressed about? DSM-IV , the “Bible” of the psychiatric profession, which identifies symptoms of mental illnesses, recognizes that children can be depressed. Nevertheless, depression is frequently overlooked as a diagnosis by the health care profession. Among possible reasons are these: The historic denia l that depression exists in children. Twenty years is not a long time. Many medical practitioners, especially those who do not specialize in mental health treatment, may not have been trained to recognize depression in children, or are prone to recognize other possibilities first when presented with a depressed child.

J ust 40 years ago, many physicians doubted the existence of significant depressive disorders in children, primarily because they believed that children lacked the mature psychologic and cognitive structure necessary to experience these problems. However, a growing body of evidence has confirmed that children and adolescents not only experience the whole spectrum of mood disorders but also suffer from the significant morbidity and mortality associated with them. Suicide has become a growing public health concern as successive generations have shown a parallel increase of suicide and depression in the pediatric age group. Childhood depression, like the depression of adults, can encompass a spectrum of symptoms ranging from normal responses of sadness and disappointment in stressful life events to severe impairment caused by clinical depression that may or may not include evidence of mania. Epidemiology In the United States, the prevalence of major depressive disorder is approximately 1 percent of preschoolers, 2 percent of school-aged children and 5 to 8 percent of adolescents.

"Depression is not age defined. By the time they're 14 years old, up to nine percent of children have already experienced at least one episode of severe depression." The World Health Organization and the World Bank have determined that depression is the leading cause of disability worldwide in market economies among individuals 5 years and older Mental health problems affect 1 in every 5 children at any given time. Depression is defined as an illness characterized by feelings of depression that persist and interfere with a child or adolescent's ability to function. Children under stress , who experience loss , or who have attentional , learning, conduct or anxiety disorders are at a higher risk for depression. Depression also tends to run in families. The signs of depression in children are very different than in adults. Children process information and feelings differently.

Dr. Goldstein is a psychologist working in the Neurology, Learning and Behaviour Centre in Salt Lake City, Utah. He holds a number of teaching and clinical posts, as well as being Chairman of the Professional Advisory Board of CHADD.   • Depression may affect as many as 10% of all children before the age of 12, but is likely to affect an even higher percentage of children at greater risk, including those with anxiety, attention deficit or conduct disorder, learning difficulties, etc. If their parents are depressed or if they have parents with bi-polar disorder the risk increases. Depression is episodic, and ranges on a continuum from mild to severe. It is a syndrome a cluster of incapacitating symptoms and behaviours affecting the whole body. These symptoms affect both behaviour and emotion. There is a biological difference in each of us, and some of us are more resilliant than others. It is estimated that approximately 20% of ADHD people carry a trait or predisposition for depression. Thus when ADHD is diagnosed, it is important to consider the possibility of co-existing depression.

Millions of American children experience persistent sadness and hopelessness. They may withdraw from friends and family, perform poorly in school, act out in anger or stop participating in activities they once enjoyed. The National Alliance for the Mentally Ill NAMI ) is trying to get the word out that these children may be suffering from a serious illness called depression.
"Children can and do get depressed," said Laurie Flynn, NAMI executive director. "But by intervening early and providing appropriate education and support, we literally can save lives."
As many as one in 33 children and one in eight adolescents may suffer from depression, according to statistics released by the U.S. Center for Mental Health Services. Moreover, if left untreated, the results of depression can be fatal, including suicide the leading cause of death among adolescents. NAMI, in conjunction with many leading national mental health and advocacy organizations, seeks to increase public awareness of depression in youth and serve as a call to action to help eradicate this highly treatable illness.

Recognizing and treating childhood depression "Symptoms of depression in childhood are basically common difficulties children have like temper tantrums, hyperactivity, disobedience and, in girls particularly somatic complaints – headaches, pains, stomachaches," says Dr. Scott Armentrout, director of psychological services at Warm Springs Counseling and Training Institute in Boise. "What makes the symptoms of depression different from what you encounter normally are their frequency, intensity, and duration." Contrary to social thinking, children do not suffer depression because they are bad kids or because they have bad parents.
