Marco Pantani died of a heart attack and showed signs of cerebral edema and lung damage, according to initial reports from an autopsy carried out in Italy on Monday. Officials conducting the autopsy said it may be up to 60 days before they know the exact cause of death for the man who won both the Giro d'Italia and Tour de France in 1998. "We can't rule out any cause of death," said Professor Giuseppe Fortuni after the autopsy. "Today's autopsy is only the first in a long series of tests. It will be a few weeks before we know the exact cause of death." Pantani, a former winner of the Tour de France and Giro d'Italia, both in 1998, was found dead at 34 on Saturday in a hotel room in the Adriatic coast town. "We will make very precise exams in the laboratory in addition to those of the clinical type," Fortuni said. "We will then have microscopic exams and consider all hypotheses excluding none of them. We are just at the first phase." Funeral set for Wednesday
Marco Pantani's family is preparing a funeral for Wednesday, according to the Italian wire service ANSA. A ceremony will be held in the square at the St. Giacomo church in Pantani's hometown of Cesenatico. The town has also called for a day of mourning for Pantani, who died Saturday in Rimini.

Introduction
Every year thousands of people go climbing in the Rockies, trekking in the world famous Himalayas, and skiing in the Alps in search of the ultimate adrenaline rush. The explosion of recreational “extreme sports” such as snowboarding; mountain climbing and trekking; skateboarding; and mountain biking and the subsequent injuries sustained in these sports, have precipitated a need for further research in the extreme sport arena. It is estimated that during the year 1984, 34 million people, including travelers and indigenous populations, traversed terrain above 7,500 ft with numbers increasing each year thereafter. Of this group, 25% of those reaching elevations above 8,500 ft developed manifestations of high-altitude illness. The incidence of acute mountain sickness (AMS) is increasing due to modern society's ability to travel rapidly to high altitudes. Travelers need to be educated on the risk associated with high altitudes since as many as 5% of cases of AMS can develop life-threatening high altitude cerebral edema (HACE).

Clinical Problem: A 65 year old man who suffered a large MCA territory infarct 72 hours ago develops worsening deficits. A CT scan shows increasing cerebral edema. Clinical Question: In a stroke patient with significant cerebral edema does mannitol improve clinical outcome? Clinical Bottom Lines: 1. The clinical effect of mannitol or HS-HES on stroke outcome is not known. 2. In the short term, mannitol reduced post-stroke raised intracranial pressure in 10/14 (71%) events, while HS-HES reduced it in 16/16 (100%) events. 3. Review of RCTs of other interventions (eg. steroids, IV glycerol) show that they are no better than placebo in improving stroke outcome. 4. Early hemicraniectomy was of some benefit in a small cohort study without randomized controls The Evidence: A single centre RCT of episodes of raised ICP in 9 stroke patients in a neuro-intensive care unit. Initial events of raised ICP were randomized to treatment with hypertonic saline hydroxyethyl starch solution (HS- HES ) or mannitol. In subsequent events the 2 substances were alternated

Predicting Cerebral Edema during Diabetic Ketoacidosis
The New England Journal of Medicine January 25, 2001 Vol. 344, No. 4 Cerebral edema is a devastating complication of diabetic ketoacidosis and remains the leading cause of serious illness and death in children with diabetes mellitus. (1) The causes of cerebral edema are unknown, largely because of the lack of large-scale population-based studies. However, several hypotheses relating to possible antecedent risk factors and the effects of various treatment regimens have been proposed. (1) In this issue of the Journal, Glaser et al. (2) report the results of a retrospective analysis of cases of children with diabetic ketoacidosis at 10 centers. Among 6977 such children, the authors identified 61 with cerebral edema. The incidence of cerebral edema was 0.9 percent, a rate remarkably similar to that reported historically in the United States (3) and recently in the United Kingdom. (4) As in previous studies, the risk was highest among younger children with newly diagnosed diabetes. (1) The 21 percent mortality rate associated with cerebral edema was similar to that in the United Kingdom, (4) as was the morbidity rate, with a substantial 27 percent of the survivors having neurologic sequelae. The development of cerebral edema may be the result of the treatment that children receive for diabetic ketoacidosis; treatments such as high doses of insulin and the administration of bicarbonate or large volumes of hypotonic fluid would be the major culprits. (1) However, it is also possible that the condition is an idiosyncratic response to diabetic ketoacidosis. Thus far, no adequate proof for either hypothesis has been advanced.
