Reflex Sympathetic Dystrophy Syndrome (RSDS) is a rare disorder of the sympathetic nervous system that is characterized by chronic, severe pain. The sympathetic nervous system is that part of the autonomic nervous system that regulates involuntary, bodily functions such as increasing heart rate, constricting blood vessels, and increasing blood pressure. Excessive or abnormal responses of portions of the sympathetic nervous system are thought to be responsible for the pain associated with Reflex Sympathetic Dystrophy Syndrome.
The symptoms of Reflex Sympathetic Dystrophy Syndrome typically begin with burning pain, especially in an arm, finger(s), palm of the hand(s), and/or shoulder(s). In some individuals, RSDS may occur in one or both legs or it may be localized to one knee or hip. Frequently, RSDS may be misdiagnosed as a painful nerve injury. The skin over the affected area(s) may become swollen (edema) and inflamed. Affected skin may be extremely sensitive to touch and to hot or cold temperatures (cutaneous hypersensitivity). The affected limb(s) may perspire excessively and be warm to the touch (vasomotor instability). The exact cause of RSDS is not fully understood, although it may be associated with injury to the nerves, trauma, surgery, atherosclerotic cardiovascular disease, infection, or radiation therapy.

What is causalgia? Causalgia is a rare syndrome that follows partial peripheral nerve injuries. It is characterized by a triad of burning pain, autonomic dysfunction and trophic changes. Severe cases are called major causalgia. Minor causalgia describes less severe forms, similar to reflex sympathetic dystrophy (RSD). RSD has predominant muscular and joint symptoms, with osteoporosis being common on x-ray. What causes causalgia? Causalgia is caused by peripheral nerve injuries, usually brachial plexus injuries. Denervation causes hypersensitivity resulting in increased pain and increased norepinephrine release causes the sympathetic findings. What are the clinical findings? Pain: usually burning, and prominent in hand or foot. Onset in the majority is within 24 hours of injury. The median, ulnar and sciatic nerves are the most commonly involved. Almost any sensory stimulation worsens the pain.
Vascular changes: Either increased blood by vasodilatation (warm and pink) or decreased blood by vasoconstriction (cold, mottled blue).

WHAT IS REFLEX SYMPATHETIC DYSTROPHY? In 1993, the name of reflex sympathetic dystrophy was changed to COMPLEX REGIONAL PAIN SYNDROME (CRPS) mainly for research purposes and to avoid confusion with the many labels that have been attached to RSD in the past. CRPS type one is formerly known as RSD and CRPS type two was causalgia. CRPS type one (formerly RSD) is a debilitating disease which involves the skin, nerves, blood vessels and bone. The sympathetic nervous system reacts to a stimulus for example, an injury. Blood flow may be affected in reaction to a burn, cut, or severe temperature changes. To prevent you from further using an injured limb, the limb swells. Sometimes, and no one knows why, an abnormal or prolonged sympathetic reflex begins in a limb as reaction to a trauma. The sympathetic nerves become overactive and can cause a variety of symptoms that may cause debilitating consequences. There can be many symptoms, but the most common one is burning pain. Some of the other symptoms include: swelling temperature changes colour changes diminished motor function severe sweating These symptoms usually happen in a limb but can occur in other body parts eg. face.. Symptoms may vary with each individual who has CRPS type one (RSD).

Conservative Management of Causalgia
(Reflex Sympathetic Dystrophy) Causalgia may be described as an uncommon pain syndrome characterized by posttraumatic pain most commonly affecting the upper or lower extremities. Clinically, the presenting pain is usually paroxysmally produced by local friction, or even air currents, which affect the nerve supply innervating that given region where the pain syndrome is symptomatically manifested. This pain phenomenon varies in its clinical presentation from immediately post-injury to several weeks later. The character of the injury may be as trivial as a sprain but any type of trauma may be etiological. Although the etiology is idiopathic, it is most often due to complete, or incomplete, laceration of the nerve, or plexus, involved. The cardinal symptom is severe burning pain of paroxysmal character. There may be an intolerance to dryness in the area of pain and possible cutaneous coolness, color changes involving redness, or cyanosis, with glossiness, and edema. The pain is commonly exquisitely hyperalgesic. The symptoms may not present for several weeks following the etiological event, but gradually cutaneous symptoms indicative of vasodilation appear. Later, the skin may become cold, cyanotic, pale, diaphoretic, thin, and glossy with brittle and ridged nails. These cutaneous arteriovenous characteristics usually exist even in the presence of blood flow studies which show equality with the uninvolved extremity. These findings suggest that such nerve damage may affect surface vasodilatation without influencing larger deep vessels. Reduction, or complete elimination, of the pain by sympathetic block is diagnostic.

Sudeck on XXIX Congress of a German society of surgeons in Berlin makes report about the second changes in the distal divisions of upper extremity in the form of heterogeneous progressive spotty osteoporosis, discovered by it roentgenological. P. Sudeck described the stages of disease and gave to it name "acute bone atrophy". Overview
What is reflex sympathetic dystrophy syndrome? Causalgia was first documented in the 19th century by physicians concerned about pain Civil War veterans continued to experience after their wounds had healed. Doctors often called it "hot pain," after its primary symptom. Over the years, the syndrome was classified as one of the peripheral neuropathies, and later, as a chronic pain syndrome. RSDS is currently classified as a variant of causalgia, not necessarily caused by trauma. Who gets it? RSDS can strike at any age, but is more common between the ages of 40 and 60. It affects both men and women, but is most frequently seen in women. Although it can occur at any age, the number of RSDS cases among adolescents and young adults is increasing. Investigators estimate that two to five percent of those with peripheral nerve injury and 12 to 21 percent of those with hemiplegia (paralysis of one side of the body) will suffer from RSDS.
