TABLE OF CONTENTS A cataract is a cloudy area in the lens of the eye. A normal lens is clear. It lets light pass to the back of the eye. A cataract blocks some of the light. As a cataract develops, it becomes harder for a person to see. Cataract is a normal part of aging. About half of Americans ages 65 to 74 have cataract. About 70 percent of those age 75 and over have this condition. Most people with cataract have a cataract in both eyes. However, one eye may be worse than the other because each cataract develops at a different rate. Some people with cataract don't even know it. Their cataract may be small, or the changes in their vision may not bother them very much. Other people who have cataract cannot see well enough to do the things they need or want to do. This information can help you decide what to do if you have a cataract. Talk about it with your eye doctor so you can make the choices that are right for you.

What Is a Cataract? A cataract is a clouding of the lens of the eye. The lens is a clear oval structure with three layers: the nucleus, the cortex, and the capsule. It may help to think of the lens as a peach, where the nucleus is the peach pit, the cortex is the flesh of the peach surrounding the pit, and the capsule is the peach skin, or elastic covering of the lens. You have a cataract when the nucleus becomes opaque (that is, it is no longer clear) or when small opacities develop in the cortex that block or scatter light. There are three types of cataracts which affect different parts of the lens and cause different symptoms and vision problems.

By Linda Kaspari Cigarette smoking reduces levels of plasma antioxidant, a substance in the blood stream, which protects retinal cells. Smoking causes the protective layer between the retina and blood vessels to erode, resulting in poor circulation, irritation and scarring. Smoking is damaging to the blood supply. Smoking exposes the body to free radicals which causes cellular damage and can cause poor circulation of nutrients to the retina and lens. Carotenoid levels and vitamin C blood concentrations are substantially reduced. Smoking oxidizes cholesterol, causing fat to build up in the arteries. This leads to heart attack, stroke, and probably macular degeneration.

Researchers think smoking contributes to the incidence of some types of cataracts. "What makes us suspect this is the consistency of studies all over the world," says Robert Sperduto, M.D., chief, epidemiology branch of the National Eye Institute. "If there's one thing people can do to improve their general health, including the health of their eyes, it is to stop smoking," says Dr. Sperduto. Carbon monoxide from cigarette smoke creates a hypoxic effect, it is clear that hypoxia will degrade night vision. (Hypoxia is a state of oxygen deficiency in the body which is sufficient to cause an impairment of function. The inadequate oxygen transport or the inability of the tissues to use oxygen causes hypoxia.)

Most cataracts are inherited, and are found in many breeds such as the Cocker Spaniel, Poodle, Husky, Schnauzer, Golden and Labrador Retrievers, and terriers. Other causes of cataracts include: Diabetes, trauma, inflammation, and puppy milk replacers. Many cataracts will worsen to the point of blindness but certain types, especially in the Retriever breeds, can remain small for the entire life of the patient. A common phenomenon occurs in many developing cataracts where the patient can develop an allergic type of reaction to the cataract. This allergic reaction is a LOCAL reaction and can result in many complications such as scar formation and glaucoma.

(or opacities) of the lens of the eye (see
Figure 1). Such opacities may be quite small and interfere little with vision, or they may involve the entire lens causing blindness. Cataracts occur because something interferes with the normal function of lens fibers causing them to degenerate. Causes include inflammatory diseases, hereditary factors, aging changes, toxicities, and metabolic diseases such as diabetes mellitus. DO ALL ANIMALS DEVELOP CATARACTS WITH AGE? In general, no! In fact most animals should live their entire lives without developing cataracts. The lens does become thicker with age and thus appears grayer causing many people to mistake this change for a cataract. This normal aging process is called nuclear or lenticular sclerosis and does not impair vision other than making focusing on close objects more difficult. However some animals do develop cataracts and certain breeds of animals are afflicted with hereditary cataracts so that a significant percentage of the population may develop cataracts. This is especially true in dogs. Miniature schnauzers, cocker spaniels, poodles, Labrador retrievers and golden retrievers are just a few of the breeds which may be affected.

To understand cataracts, it first helps to know how the eye works. Light enters the eye through the cornea, which is the clear outer dome, then moves through the pupil, where the lens is. The lights is then transmitted and focuses images on the retina, which is like film in a camera. The macula is the central receptor part of the retina where we get sharp vision for tasks such as reading and driving. This lens is a small oval like structure consisting of a thin capsule (like clear plastic wrap) enveloping a bag of protein. The protein is crystal clear and colorless at birth, but over the years it discolors and clouds. It first yellows, then turns brown, and finally cloudy. When it becomes so cloudy that it interferes with vision, it is referred to as a cataract. At this time, the lens no longer transmits or focuses light clearly. Having a cataract is a bit like having a dirty windscreen on a car. It can make the view cloudy or foggy. When the sun is behind you, the vision through the dirty windscreen is ok, but when you turn into the sun, the view through the windscreen becomes a ‘white out’. A cataract is similar in that glare from lights, the sun when it is low in the sky and on-coming headlights can all cause problems with the vision. When these problems make normal daily activities difficult, it is time to get the cataract operated on. Cataracts do not permanently impair your vision and you will not go blind from a cataract. A simple operation will restore your sight. Most forms of cataract develop in adult life, and can occur any time after the age of 40. The normal process of ageing causes the lens to harden and become cloudy. Cataracts can also be caused by injuries to the eye, for instance, any cuts, blows or burns to the eye which cause damage to the lens inside the eye. Inside the eye, behind the iris (the coloured part of the eye) is a lens. In the normal eye, the lens is clear or transparent, and helps to focus light rays on to the tissue at the retina at the back of the eye. When a cataract develops, the lens becomes cloudy and prevents the light rays passing into the retina. The picture that the retina receives becomes dull and fuzzy. Cataracts usually form slowly and people experience a gradual blurring of vision.
