Degenerative spine disease is a major cause of chronic disability in the adult working population and a common reason for referral to an MR imaging center. Spinal degeneration is a normal part of aging, and neck and back pain are one of life's most common infirmities. There are many potential sources of pain, and finding the specific cause is often a confounding problem for both patient and doctor. Pain can originate from bone, joints, ligaments, muscles, nerves, and intervertebral disks, as well as other paravertebral tissues. The landmark article by Mixter and Barr in 1934 on the ruptured intervertebral disk provided an anatomic basis for selected cases of back pain and neurologic dysfunction. Most neck and back pain responds to conservative therapy, but if the pain is unrelenting, severe, or associated with a radiculopathy or myelopathy, imaging is indicated to look for a treatable cause.
EXAMINATION TECHNIQUE In the evaluation of degenerative spine disease, multiple anatomic sites need to be imaged, including the intervertebral disk, spinal canal, spinal cord, nerve roots, neuroforamina, facet joints, and the soft tissues within and surrounding the spine. Many pulse sequences are available, and specific protocols vary among different MR sites. There is general agreement that the spine needs to be imaged in at least two planes, and surface coils are used almost exclusively. In the cervical and thoracic regions a T2-weighted sequence is mandatory to assess damage to the spinal cord. Thin sections are required to visualize the neuroforamina, and pulse sequences must be tailored to counteract CSF flow and physiologic motion. The imaging requirements for the lumbar spine are less strenuous because the anatomical parts are larger. Most protocols include a T1-weighted sequence and some type of T2-weighted sequence to give a myelographic effect.

The Problem Spine
Each vertebra is separated by a cushion, which is called an intervertebral disk (also spelled disc). The outer, thick ring on the disk is the annulus: the spongy center is the nucleus. The annulus serves as a gasket with a goal to keep the nucleus moist. Because of the soft, gelatin-like centers, the intervertebral disks serve as shock absorbing pads and prevent the rubbing of one disk against another. Also, the cushions provide flexibility for the spine.
Attached to the back of these vertebral bodies is a bone arch that provides an open space. This space, the spinal canal, houses the spinal cord and nerve bundles. The spinal cord itself is bathed in cerebrospinal fluid (fluid that circulates through your spine and brain) and is covered by several protective membranes. At each vertebral level (i.e., each building block of bone) a pair of spinal nerves exit, one to the left and one to the right, off the spinal cord through a small opening called a foramen. These nerves provide the relay of messages to and from the brain, allowing us to move our body and to feel sensation. Nerves that exit through the lumbar spine affect the hips and legs.
One of the basic underlying factors associated with most spine disorders is the dehydration of the disks. As we age, (starting around 30), the gelatin-like centers dry out and become flattened, causing the vertebrae to lose height and its healthy resilience. The spine weakens. With this degeneration, the vertebrae get closer together and cause nerve irritation, which usually stems from a bulging or ruptured disk, bone spurs, or stenosis.

    How to Treat a Pinched Sciatic Nerve
    The sciatic nerve is the largest nerve in the body, running from the base of the spinal cord to the feet. Sometimes a vertebral disk may bulge out of place and pinch the sciatic nerve. This can cause mild or severe throbbing of the back and leg. Steps: Visit your family doctor or orthopedic surgeon. Take aspirin or ibuprofen to relieve discomfort. Taking two aspirin every day, even when you aren't suffering with sciatic pain, can help prevent pain from recurring. Place at least one pillow under your knees when you lie down. This helps relieve pressure that the disk is putting on your sciatic nerve. Rest on a firm mattress or on the floor when sciatic pain is severe. Stay in bed, getting up only when absolutely necessary, for 3 days. Ask for help to get out of bed or into the bath or shower so you don't strain your back while getting up and down. Apply hot or cold packs to your back and upper legs. Sometimes ice will work better than heat; at other times, a heating pad will do a better job of relieving pain. Try both to see which one is more effective for you.

    Hello Janis, I am an unfortunate expert of sorts on this matter, having
    undergone three spine surgeries in my life thus far to repair herniated
    disks and the damaged / impinged nerves resulting from them.
    Pardon my most basic example, but it works:... The disks between the
    vertebrae are like jelly filled donuts. Sometimes pressure builds up and one of them bulges out a bit, but remains in tact with no jelly leak, and often over time goes back to its normal state. Sometimes an abrupt, sudden pressure can cause one of the donuts to burst and the jelly shoots out. (this is called a herniated disk). If the jelly leaks onto the nerves which attach to the spine, or if a piece of the donut itself breaks off (fragments) and affect the nerves, then you have a problem. Nerve impingement causes great pain, discomfort and all kinds of horrible symptoms down into the legs. Bulging disks do not cause these serious problems, so that's GREAT news for you.

    A bulging disc could cause your symptoms but there are other conditions that give similar syptoms. To name a few: intraabdominal pathologies, kidney problems, sacroiliitis, piriformis syndrome, etc. I would seek additional advice from a specialist. Sherman Nam Tran, M. D.

    My wife( she is 27 years old) , had a car accident two months ago. After the accident she had the following symptoms: 1. numbness of the left arm.2.tingling feeling.3.weakness.4.pain.She has been taking pain killers since the accident,but the pain won't go away. She had an MRI 3 weeks ago and they found.1.Arthritis in the neck.2. Bulging disk C4/C5and C5/C6. The doctor told her that she is NOT supposed to have pain and the symptoms above. Is that true? What options does she have for her symptoms to improve? She's left handed and she has lost ability to write and other simple everyday activities.     Herniated discs can give all the symptoms above. The dilemma is that 30% of people who have no symptoms can have MRIs that show herniated discs as well. The most important thing is to have a complete and thorough examination to make sure that there is no neurologic deficits. You can also have an electromyography (EMG) to make sure there is no continuing nerve damage. Treatment includes a closely monitored rehab program, epidural injections (risky in the neck region), and surgery. Up to 90% of herniated discs without evidence of myelopathy can be managed conservatively with good outcome. A physiatrist with spine expertise is well suited to manage cases like this.
