Buerger's Disease
Buerger's disease (Thromboangiitis Obliterans) is an inflammatory disease of the small and medium sized arteries and veins of the extremities. It is often seen in men and appears to be directly related to smoking. There may also be an autoimmune element as well. Raynaud’s phenomenon, ulcers and pain are typically seen. Immediate termination of smoking is critical, but sadly, often difficult to do for these patients.

Vascular disease can happen to anyone. While in many cases your susceptibility to vascular disease may be genetic, several of the risk factors for vascular disease, such as high cholesterol, sedentary lifestyle, and smoking, are within your control.
The Columbia Weill Cornell Division of Vascular Surgery is committed to providing comprehensive vascular care, which includes a focus on prevention. Our physicians can help you to identify and modify your risk factors for vascular disease. This section provides information on programs run by or in association with NewYork-Presbyterian Hospital that can help high-risk patients take significant steps to maintain lifestyles supportive of a healthy vascular system. Buerger’s Disease Thromboangiitis obliterans, very rare disorder causes obstruction of the blood vessels of the hands and feet. It is caused by bands of inflammation and changes in the lining of the small arteries and veins which leads to a thickening of the blood vessel walls caused by infiltration of white cells.. The association cigarette smoking, cannot be overemphasized. Patient may complain of pain in the instep which is worse when walking or exercising. Characterized by an inflammatory response in the arteries.

O mentum has vast network of blood vessels and lymphatics. It is known for its angiogenic properties. We used omentum for revascularization of the ischeamic limbs and eye (retina) and developed a technique (Agarwal technique) to lengthen and brought it up to the toes in legs* and to retina in Eyes.** Omental homografts were also used as freegrafts with success in ischeamic legs . We feel that this type of transplantation is effective because of its angiogenic properties and neovascularisation of ischeamic tissues. Vineburg experiments have shown that omental grafts would develop communication with the local circulation which is ischeamic in 4-8 days after omentopexy , by biological capillary and arteriolar anastomosis wherever the vessels are patent. Reaction to Having the disease: It was a relief in one way, because I had suffered from the symptoms for 6 yrs.and was told numerous reasons for the pain in my arms and legs. It was hard on my father, because his father had this disease. That was scary, because he lost one of his legs, because of it. Dealing with the disease: It seems as though you are always sick. Hardly a week goes by that there is notpain of some sort. Of course, having to give up my career in Accounting when the disease progressed was emotionally hard. Other info about the disease: It is very rare and hard to diagnose, once you are diagnosed, be very comfortablewith your dr. You will spend alot of time in their office and you need to feel as thoughthey understand your pain and are giving you the best care possible. Take any medications given to help your circulation, and by all means try not to smoke. It's hard, I was diagnosed in 1995 and am still fighting the nicotine habit. What you need to know about vascular disease
As the heart beats, it pumps blood through a system of blood vessels, called the circulatory system . The vessels are elastic tubes that carry blood to every part of the body
  • Arteries carry blood away from the heart. Veins return blood back to the heart.
Vascular Disease includes any condition that affects your circulatory system. This ranges from diseases of your arteries, veins and lymph vessels to blood disorders that affect circulation.  The following are conditions that fall under the category of “Vascular Disease:”
Peripheral Artery Disease
Like the blood vessels of the heart (coronary arteries), your peripheral arteries (blood vessels outside your heart) also may develop atherosclerosis, the build-up of fat and cholesterol deposits, called plaque, on the inside walls. Over time, the build-up narrows the artery.  Eventually the narrowed artery causes less blood to flow, and a condition called ischemia can occur.  Ischemia is inadequate blood flow to the body’s tissue.

The body's intricate network of arteries, veins, and even tinier blood vessels, can suffer from a wide range of medical disorders. Members of the Section of Vascular medicine bring their joint expertise to the diagnosis and treatment of every kind of vascular condition. Section members specialize in the newest, most technologically advanced therapies for vascular disease. They share an international reputation for excellence. In the Forefront of Patient Care
Among the disorders treated are: carotid artery disease, Raynauds disease, deep vein thrombosis, poor circulation, leg cramps, atherosclerosis, stroke, Buergers disease, arterial diseases, hypercoagulable states, venous ulcerations and obstructions, venous insufficiency, varicose veins and spider veins, lymphedema, pseudo-aneurysms, and leg ulcers and wounds. Section members utilize minimally invasive, state-of-the-art therapies, such as angioplasty with stent placement. Angioplasty involves placement of a thin, uninflated, surgical balloon across an area of blockage in a blood vessel. The balloon is then inflated to dilate (widen) the blocked artery back to its normal size. When angioplasty alone cannot dilate the vessel sufficiently, a mesh tube called a stent can be inserted and left in place to hold the artery open to its normal size.

V ASCULAR A SSOCIATES OF B ANGALORE N EWSLETTER
Buerger's Disease Etiology and Pathophysiology:
Smoking is very closely related to Buerger's disease and smoking history is one of the criterion for diagnosing the disease. In general if the patient absolutely abandons smoking the course of the disease will be invariably benign, but if smoking continues any treatment will ultimately be futile. Though "passive smoking" has adverse effect on cardiovascular system, non smokers should never develop the disease. Active smokers can be indentified by measuring levels of continine, the major metabolite of nicotine in urine. Since all smokers do not develop the disease an immunopathogenesis is considered probable. It has been proposed in Japanese that presence of a gene linked to some HLA antigens might control the susceptibility to the disease. Socioeconomic conditions, work environment may also play in etiology as the disease is seen more in out door and manual workers. Hypercoagulable state has been observed in association with the disease. Hepatitis B Virus and rickettssiosis may contribute to pathogenesis, but this role is uncertain. Buergers disease is an inflammatry occlusive disease which involves all layers of medium sized and small arteries of the extremitiles. Involved superficial veins bear a close resemblance to those in the affected artery. Majority of the patients develop critical limb ischemia with trophic lesions are distal to ankle, the anklebrachial doppler index could be normal in early stage. Toe pressures can be measured and if it is less than 30 mm Hg, the healing of ulcers is unlikely. The disease though commences peripherally, may gradually extend proximately occluding the larger arteries.

http://www.brandoncarmichael.com/buergers_disease.html
Buerger's disease, also called thromboangiitis obliterans, is a rare disease of the blood vessels of the arms and legs. It's characterized by a combination of inflammation and clots in the arteries and veins that obstructs blood flow. Buerger's disease usually affects men between ages 20 and 40. It's more common in the Middle East and the Far East.
The cause of Buerger's disease isn't clear. But it's strongly associated with smoking. Stopping smoking usually improves the problem. Also, some research suggests that arsenic poisoning may play a role in the disease.
The disease restricts or blocks blood flow to the arms and legs, beginning with the hands, fingers and feet. This eventually damages or destroys tissues and makes them more prone to infections and gangrene.
Signs and symptoms of Buerger's disease include:
  • Pain or tenderness in the fingers, hands or forearms
  • Pain in the toes, feet, ankles or legs
  • Skin changes (atrophy) or ulcers in the fingers and toes
  • Fingers and toes turning white or blue in response to cold or strong emotions (Raynaud's phenomenon) — this is due to blood vessel spasms resulting in less blood flow to fingers and toes
  • Visible short segments of enlarged, red and tender surface veins

years. Then I asked him “what does this have to do with my toe?” He replied “you may have buergers disease or Diabetes”.

[Read the November 21, 2003 Forum Article on Brandon Carmichael]
I must explain something first. I had been offered cigarettes many times before, but thankfully it was drilled into me that they were very dangerous. So up to that point I had been able to say NO.
It all started when I was 15 years old. I was with two of my buddies. We were at the “free pool” park in West Fargo. One of them pulled out a pack of Marlboro Reds and started smoking a cigarette and my other friend lit his cigarette. Almost simantansly they asked me 'do you want one?' I paused and then I said “yes” not knowing what I had just started.
As time went on I found myself hanging around these guys a lot more than I should of, smoking their cigarettes. I had thought to myself 'cigarettes can't hurt me', but boy oh boy was I wrong.
Smoking up to my senior year in High School brought me to a “Reality Check”. Many of you may be confused by my use of the term “reality check”, but to me it makes perfect sense. Let me explain…
…It started in December of 1999, just a few weeks before Christmas.
