Bronchitis COMMON CHARACTERISTICS
Bronchitis is an inflammation of the bronchial tubes, or bronchi (the air passages that extend from the windpipe into the lungs). The inflammation may be caused by a virus, bacteria, smoking or the inhalation of chemical pollutants or dust. When the cells of the bronchial-lining tissue are irritated beyond a certain point, the tiny hairs (cilia) within them, which normally trap and eliminate pollutants, stop functioning. Consequently, the air passages become clogged by debris and irritation increases. In response, a heavy secretion of mucus develops, which causes the characteristic cough of bronchitis. ACUTE BRONCHITIS
Brief bouts of acute bronchitis often evolve from a severe cold. This disorder may also follow or accompany the flu, or it may begin without having had an infection. Acute bronchitis usually lasts about 10 days. Severe cases may also cause general malaise and chest pain. CHRONIC BRONCHITIS
Chronic bronchitis is defined as excessive mucus secretion in the bronchi and a chronic or recurrent mucus-producing cough that lasts three or more months and recurs year after year. In diagnosing chronic bronchitis, it is important to rule out heart disease, lung infections, cancer and other disorders that may produce bronchitis-like symptoms. Chronic bronchitis may result from a series of attacks of acute bronchitis, or it may evolve gradually because of heavy smoking or the inhalation of air contaminated with other pollutants in the environment.

A patient information handout on acute bronchitis , written by the authors of this article, is provided on page 1281. Acute bronchitis is a lower respiratory tract infection that causes reversible bronchial inflammation. In up to 95 percent of cases, the cause is viral. While antibiotics are often prescribed for patients with acute bronchitis, little evidence shows that these agents provide significant symptomatic relief or shorten the course of the illness. In a few small studies, bronchodilators such as albuterol have been found to relieve some symptoms of acute bronchitis. Increased attention is being given to the role of Chlamydia species in acute bronchitis and adult-onset asthma. Studies in progress may help to clarify the importance of these organisms in acute bronchitis and to determine whether early treatment can prevent or ameliorate asthma. A cute bronchitis is one of the most common diagnoses made by primary care physicians.

A patient information handout on chronic bronchitis , written by the authors of this article, is provided on page 2376. C hronic bronchitis is one of the principal manifestations of chronic obstructive pulmonary disease (COPD), the fourth leading cause of death in the United States. About 10 million Americans are affected by some degree of COPD; it causes 40,000 deaths annually. Chronic bronchitis is a clinical diagnosis characterized by a cough productive of sputum for over three months' duration during two consecutive years and airflow obstruction. These requisites exclude more transient causes of cough associated with sputum production, such as acute bronchitis. The airflow obstruction in chronic bronchitis is caused by excessive tracheobronchial mucus production and is distinct from the anatomic findings of distal air space distention and alveolar septa destruction, which define emphysema. Cigarette smoking is the most important risk factor for the development of chronic bronchitis. Over 90 percent of patients with chronic bronchitis have a smoking history, although only 15 percent of all cigarette smokers are ultimately diagnosed with some form of obstructive airway disease.

    What Is Chronic Bronchitis? Bronchitis is an inflammation of the lining of the bronchial tubes. These tubes, the bronchi, connect the windpipe with the lungs. When the bronchi are inflamed and/or infected, less air is able to flow to and from the lungs and a heavy mucus or phlegm is coughed up. This is bronchitis. Many people suffer a brief attack of acute bronchitis with cough and mucus production when they have severe colds. Acute bronchitis is usually not associated with fever. Chronic bronchitis is defined by the presence of a mucus-producing cough most days of the month, three months of a year for two successive years without other underlying disease to explain the cough. It may precede or accompany pulmonary emphysema. What Causes Chronic Bronchitis? Cigarette smoking is by far the most common cause of chronic bronchitis. The bronchial tubes of people with chronic bronchitis may also have been irritated initially by bacterial or viral infections. Air pollution and industrial dusts are also causes. Once the bronchial tubes have been irritated over a long period of time, excessive mucus is produced constantly, the lining of the bronchial tubes becomes thickened, an irritating cough develops, air flow may be hampered, and the lungs are endangered. The bronchial tubes then make an ideal breeding place for infections.

There are two types of bronchitis: acute and chronic. Acute bronchitis is the inflammation of the mucous membrane that lines the bronchi (brawnk-eye), or main passages of the lungs, and usually only lasts a few days. It is often caused by a viral infection and may begin after you develop a cold or sore throat. The first symptom is an uncomfortable cough which often produces a thick yellowish or grayish phlegm (flem). Other symptoms include pain in the upper chest, wheezing, and fever. Make yourself comfortable and avoid cold air, tobacco smoke, and air pollution. Inhaling steam from a running shower or vaporizer may help to clear nasal passages. Your doctor may also recommend cough medicine, antibiotics, and bed rest. Chronic bronchitis usually persists for long periods of time and occurs most often in heavy smokers or as the result of recurring episodes of acute bronchitis. People with chronic bronchitis should not smoke and should avoid cold, polluted, and smoky environments. They should also stay away from people with colds, which could lead to another attack of bronchitis. Treatment depends on how far the disease has progressed, but will probably include an inhaler and antibiotics. For more information about bronchitis, contact your health care provider.

The disease is a chronic inflammation of bronchi due to infection, physical and chemical irritations and allergic agents. The majority of the patients are of over middle age. It is characterized by chronic course and recurrent episodes. In some chronic cases, the disease may progress to chronic obstructive emphysema and chronic pulmonary heart disease. In TCM, this disease is related to the categories of "ke sou" (cough), "tan yin" (phlegm retention), "xiao chuan" (asthma) and so on. Main Points of Diagnosis 1. Long-term cough, sputum production and dyspnea which attack repeatedly and fluctuate are worse in autumn and winter. There may be fever, exacerbation of cough and mucopurulent sputum production, and dyspnea is more marked if acute concurrent infection is present. 2. Chronic bronchitis usually persists for many years. It can be classified into 3 stages, acute episodic stage, chronic persistent stage and clinically remissive stage. The last stage may last for a period of time and an attack may be induced by infections or catching a cold. Based on the clinical manifestations, the disease can be divided into simple chronic bronchitis and chronic asthmatic bronchitis. In the former, cough and sputum production are predominant. In the latter, in addition to the above symptoms, there are dyspnea and wheezing, indicating that the spasm of the smooth muscles of bronchi and bronchioli is an important pathogenic factor.

COPD is de verzamelnaam voor chronische bronchitis en longemfyseem. Beide zijn aandoeningen aan de luchtwegen, die het ademen lastig maken. Bij COPD zijn de luchtwegen vernauwd door een ontsteking. Bij een ernstige vorm van COPD zijn de longen beschadigd. Het is een chronische ziekte: mensen die COPD hebben, worden niet meer beter.
Astma, longemfyseem en chronische bronchitis werden vaak met ©©n naam aangeduid: CARA. Ze hebben allemaal te maken met ontstekingen van de luchtwegen. Maar de ontsteking bij astma heeft een andere oorzaak dan die bij chronische bronchitis en longemfyseem. Ook de behandeling is anders. Vandaar dat nu wordt gesproken over astma en COPD (Chronic Obstructive Pulmonary Diseases). COPD is de nieuwe naam voor chronische bronchitis en longemfyseem.
