BONE DISORDERS Although we often take for granted the ability to walk, run, use our arms, or climb stairs, we should cherish the intricate workings of our musculoskeletal system that make free movement possible. When a bone problem, muscle disorder, or disease interferes with this delicate machinery, our movement may be painfully limited, our daily activity severely compromised. Musculoskeletal disorders—those affecting the skeleton and its system of joints—disrupt the normal quality of life more frequently than any other type of disease. More than 20 million Americans must cope with limited activity, significant disability, or restricted movement due to orthopedic problems. The human body contains 206 bones constituting about one-tenth of the total body weight (see figure 24.1 ). The bones' framework supports and protects many vital organs. For instance, the skull shields delicate brain tissue. The spinal column encases the spinal cord, the main pathway of nerves carrying messages between the body and the brain. The rib cage guards the heart, lungs, liver, and kidneys. Bones in the pelvic area anchor the spine, encircle the bladder and, in women, protect the reproductive organs and the developing fetus. Many people mistakenly view their skeleton as a static and unchanging entity. In fact, the skeleton continually resculpts itself. Every day of your life, bone tissue cells called osteoblasts and osteoclasts constantly create new bone and break down the old in a process called remodeling.

About three years ago, studies began to describe increased rates of thinning of the bones in HIV-infected men. Subsequent studies have confirmed that compared to HIV-negative men, men with HIV are more likely to have osteopenia or osteoporosis. Osteopenia and osteoporosis are, by definition, two stages of loss of bone calcium. Osteoporosis represents the more severe end of the spectrum. The major risk of osteoporosis is an increased risk of bone fractures, such as broken hips and collapsed vertebra, which can be crippling and even life threatening. Most of the studies to date have been performed in men with HIV infection. However, the risk of osteoporosis is higher in women than in men, and increases dramatically after menopause. As more and more women are living with HIV, and as these women age, it has become increasingly urgent to learn if osteoporosis is a bigger problem for HIV-infected women. Three studies at the 10th CROI addressed bone health in women with HIV. Unfortunately, they did not give a simple answer.

MOLECULAR GENETICS OF BONE DISORDERS DR. DANNY CHAN Department of Biochemistry, The University of Hong Kong Mutation analysis of candidate genes is an important stepping-stone to the understanding of molecular mechanisms of genetic diseases, and studies of genetic diseases can provide a window into normal biological processes. A limiting step in these studies is the ability to rapidly scan for potential mutations in candidate genes. Over the years, numerous methods have been developed to facilitate this process. These include chemical or enzyme cleavage of mismatched hetero-duplexes formed between DNA sequences derived from normal and mutant alleles, and the analysis of subtle conformational changes of hetero-duplex or single-stranded DNA resulted from an abnormal sequence. The approach taken depends on a number of factors. As examples, mutation analysis of collagen defects in osteochondrodysplasias will be used to highlight how different approaches can be best applied to various candidate genes of the collagen family of proteins.
